2. Definitie
We spreken van vroegtijdig ontslag bij ontslag van de pasgeborene vooraleer hij/zij de
leeftijd van 48 uren bereikt heeft (1). Bij ontslag binnen de 24 uur na geboorte,
spreken we van zeer vroegtijdig ontslag.
3. Inleiding
In normale omstandigheden wordt elke pasgeborene binnen de 24 uur en zeker binnen
de 48 uur klinisch onderzocht door de kinderarts. Dit 1ste klinisch onderzoek richt zich
vooral op het vinden van aangeboren afwijkingen en het inschatten van de wijze
waarop de neonaat de overgang van het intra-uteriene naar het extra-uteriene leven
heeft gemaakt. In de daaropvolgende eerste levensdagen voltrekken zich echter een
aantal fenomenen, waardoor aangeboren problemen later duidelijk worden of waarbij
de pasgeborene alsnog in moeilijkheden kan komen (2). Zo heb je het opstarten van
de borstvoeding, die wat moeilijker kan verlopen dan kunstvoeding, met risico op
excessief gewichtsverlies en eventueel deshydratatie. Het afnemen van de Guthrietest,
die op zijn vroegst na 72 uur na geboorte mag afgenomen worden, kan bij vroegtijdig
ontslag “vergeten” worden. Hyperbilirubinemie bereikt zijn maximum rond de 4de
levensdag en vraagt een geoefend oog, of beter nog een transcutane meting, om tijdig
gedetecteerd te worden. Het niet vaststellen van hyperbilirubinemie kan nog steeds
aanleiding geven tot kernicterus. Vroegtijdige infecties, o.a. groep-B-
streptococceninfecties, kunnen soms pas symptomatisch worden na de eerste 48
levensuren. Een aantal studies tonen een trend tot meer heropnamen van
pasgeborenen na vroegtijdig ontslag uit de kraamafdeling (3). In een gerandomiseerde
studie van vroegtijdig ontslag (24 – 48 uur na geboorte) met goede thuisbegeleiding
versus langer verblijf (4 – 5 dagen) in de kraamafdeling was er ook een hogere
heropnamekans (heropname gedurende minstens 24 uur) na vroegtijdig ontslag (12%
versus 4.8%; P=0.004; RR 2.6; 95% BI 1.3-5.1), maar het verschil was statistisch niet
significant gedurende de 1ste levensmaand (4). Het verband met de interventie
(vroegtijdig ontslag of niet) werd hier in vraag gesteld. Een recente observationele
studie vond geen verschil in heropnames tussen neonaten ontslagen uit de
kraamafdeling binnen de 72 uur na geboorte gevolgd door professionele
thuisbegeleiding of na 4 à 5 dagen (5). De meest voorkomende reden van heropname
in beide groepen was virale luchtwegeninfectie. Het moet dus mogelijk zijn, mits

inachtname van een aantal selectiecriteria en voorzorgsmaatregelen, een vroegtijdig
ontslag op veilige manier toe te laten.
4. De Guthrietest De screening naar hypothyroïdie is gebaseerd op het vinden van een hoge thyroïd
stimulerend hormoon (TSH) spiegel. De TSH-spiegel is fysiologisch hoog gedurende
de 1ste 48 levensuren. Afnemen van de Guthrietest gedurende deze periode zal
aanleiding geven tot veel vals positieve resultaten, waarvoor de baby terug wordt
opgeroepen voor bloedcontrole, met de nodige ongerustheid bij de ouders tot gevolg
(2). De meeste stofwisselingsziekten gaan gepaard met een initiële symptoomvrije
periode, die kan variëren van enkele uren tot weken (6). Uitzonderingen hierop
