Changes are effective October 1 (FY 2013)

A new form just for reporting Indirect Contacts

More streamlined form for Direct Contacts◦ Just one place to record agency, site, or school◦ Visual cues to help with recording

race/sex/ethnicity ◦ Reporting number of youth by grade level

Only include District if the approved site is a school
