• Report covers port state control and other issues

• Factors analysed includes – age profile– Class, Flag and P&I (IACS

does better than average)– Negative performance

factors (Collisions and Groundings)

– Ports detaining vessels– Ownership quality – factored

into 10 categories based on Deficiencies per Inspection

• Bulk Carrier losses remain very small in proportion to the overall total of vessels

• But losses due to structural reasons have not yet been eliminated. 0

The Sharing of information- to improve safety

Frequently there are difficulties – commercial confidences and exposures, legal issues, professional integrities to protect, etc.

The Challenge is to overcome these !

Examples of successes:

• Company “NO-BLAME” cultures

• Confidential reporting – for example CHIRP (Confidential Hazardous Incident Reporting Programme)

• Near-Miss reporting – for example with pilotage in straits

• Inter-industry analyses – for example of fires and explosions on Chemical and Product Carrier

• IACS “Early Warning System” (EWS)

BUT More are needed !

CONCLUSIONS

To improve safety:

• Good ships BUT also GOOD PEOPLE

• Greater emphasis on partnerships and cooperation

• Commitment to “continuous improvement”

• Learn lessons (for example through incident and root cause analyses, and accident investigations)

• Share information (greater openness and transparency)
