•Establish and implement safety procedures.

•Plan, coordinate and facilitate all safety training and maintain records of training and testing.

•Resolve questions, approve and/or recommend expense items to Executive Team to correct unsafe conditions.

•Conduct annual safety meetings with Emergency Safety Team representatives.

and ensure actions are implemented.

•Review results of emergency drills and recommend any improvements to the process.

•Review critical incident reports and recommend interventions necessary.

•Conduct debriefings if a sentinel event occurs.

Sentinel Defined…

A Sentinel Event is defined as any unanticipated event in a healthcare setting resulting in death or serious physical or psychological injury to a patient or patients, not related to the natural course of the patient's illness.

So What Can You Do to Prevent Potential Physical Risks?

Here’s What You Can Do!

•Inspect your workplace regularly.•Wash your hands often.•Take extra precaution when dealing with sharps – don’t rush!

•Report any known or possible safety hazards to the Emergency Safety Team.

•Report ALL Critical Incidents properly and immediately within 24 hours.

•Be well informed and attend all required training.

•Work together with the Health & Safety Committee and the Emergency Safety Teams to ensure workplace health and safety.

Infection Control

•Infection control is an essential component of any health care delivery.

•Infection control measures can be as simple as hand washing and as sophisticated as high-level disinfection.

•Implementing these measures can prevent transmission of disease in health care settings and the community.

Here’s the Process To Report Critical Incidents?

1. Complete the Critical Incident Form in Filemaker within 24 hours.

2. Click on the Incident Report Form Tab on the home screen.

3. Then click the New Incident Report Tab.4. Complete all of the fields and upload any

supporting documents.5. Then click the tab – Submit to

Supervisor.

Critical Incident Reporting•After you submit the critical incident report

in Filemaker, it is immediately sent and reviewed by your supervisor.

•Your supervisor will complete the Supervisor Recommendations section in Filemaker and then the report is electronically sent to the Executive Team for immediate review.

•The Executive Team will review the report in Filemaker and enter a response within 5 business days.

Critical Incident Reporting

•If the Executive Team determines the incident to be an Adverse Incident it will immediately be reported to the Office of Health Care Quality for incidents occurring in Maryland or the Addiction Prevention and Recovery Administration for incidents occurring in the District of Columbia.

Critical Incident Reporting

•In the event of an Adverse Incident a debriefing will occur with any Staff involved.

•Any policy or procedural changes resulting from an incident will be reviewed and approved by the Leadership Team before being distributed to the entire staff.

What is an Adverse Incident?

•Adverse Incidents are Critical Incidents which are deemed to be reportable to the State or other governing body.

•Some examples include; Child Abuse, Child Neglect, if an ambulance is called.

•Not all critical incidents are adverse incidents.
