Certain areas are recognized by the industry as inaccessible and/or for other reasons not inspected. These include but not limited to: Inaccessible and/or, insulated attics or portions thereof, attics with less than 18” clear crawl space, the interior of hollow walls; spaces between floor or porch deck and the ceiling below; areas where there is no access without defacing or tearing out lumber, masonry or finished work; areas behind stoves, refrigerators or beneath floor coverings, furnishings; areas where encumbrances and storage, conditions or locks make inspection impractical; and areas or timbers around eaves that would require use of an extension ladder. Slab floor construction has become more prevalent in recent years. Floor coverings may conceal cracks in the slab that will allow infestations to enter. Infestation may be concealed by plaster, sheetrock or other wall coverings so that a diligent inspection may not uncover the true condition. The roof was not inspected due to lack of accessibility, qualification and licensing. These areas are not practical to inspect because of health hazards, possible damage, obstruction or inconvenience and unless specified or described in this WOOD DESTROYING PESTS AND ORGANISMS INSPECTION REPORT. This company shall exercise due care during inspections and treatments but assume no liability for any damage to tiles, slates, shingles or other roofing materials, including patio covers, aluminum awnings, solar heating, plants, shrubbery or paint during any type of treatment. Re: Business and Professions Code, Division 3, Chapter 14, Article 1 and Title 16, Division 19, Article 5. The purpose of this report is to document findings and recommendations which pertain to the absence or presence of wood destroying pests and organisms and or conducive condition(s) AT THE TIME OF INSPECTION. This report should be read carefully and is not to be confused with a home structural survey. The client’s cooperation and compliance to correct and or complete the recommendations documented in this report are obligatory. Without a mutual effort this company can not assure effective or satisfactory results. Notice: The exterior surface of the roof was not inspected. If you want the water tightness of the roof to be determined, you should contact a roofing contractor who is licensed by the Contractor’s State License Board.

Notice: Roof overhang(s) and/or exterior wood members are only visually inspected from ground level. MILLENNIUM TERMITE & PEST assumes no responsibility for infections or infestations in these areas that could not be detected at the time of this inspection due to height. MILLENNIUM TERMITE & PEST does not use ladders in the process of our inspections unless requested at an additional charge. Notice: This company is licensed by The Structural Pest Control Board to perform wood destroying organism inspections; and identifications as well as perform the control measures required by state law to eradicate infestations, infection or adverse condition. No other tradesman has these credentials, or the experience of a pest control operator. Any assertion about the absence or presence of a wood destroying organisms, or conditions deemed likely to lead to an infestation or infection made by a tradesman other than a licensed pest control operator is a violation of Structural Pest Control Act, Business and Professions code, Division 3, Articles 1-10.

ATTIC SPACES: FURTHER INSPECTION 7 Water stains were noted to the backside of the exterior T1-11 siding in attic. See 7 on diagram. RECOMMENDATION: Remove exterior wood siding for further inspection. Remove and replace any visible damaged wood members and treat adjacent wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR). Replace exterior wood siding to match as close as possible. ATTENTION: If additional damage is found extending into areas not visible at the time of inspection, a Supplemental Report will be issued on findings and the owner or their representative will be contacted with additional costs. ATTENTION: If the above noted repairs are completed by parties other than MILLENNIUM TERMITE & PEST an Interim Inspection will be required before any finished products are installed. GARAGES: SECTION 1 FINDING 8A Fungus (dry rot) damage was noted to the side garage door, jambs and stucco moulding. See 8A on diagram. RECOMMENDATION: Remove damaged door, jambs and stucco moulding. Inspect wood members for additional damage and treat adjacent wood members with an approved chemical fungicide (DISODIUM OCTABORATE and TETRAHYDRATE SOLUTION/TIMBOR). Install new pre-hung door using original hardware if possible. ATTENTION: If additional damage is found a Supplemental Report will be issued on findings and the owner or their representative will be contacted with additional cost. SECTION 1 FINDING 8B Fungus (dry rot) damage was noted to main garage door, approximately 14 linear feet of the 2x8 doorjambs,

approximately 14 linear feet of 1x4 trim and approximately 31 feet of 2x6 framing around the main garage door. See 8B on diagram.

RECOMMENDATION: Remove and replace damaged wood members with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. ATTENTION: If the above noted repairs are completed by parties other than MILLENNIUM TERMITE & PEST an Interim Inspection will be required before any finished products are installed. OTHER INTERIOR: FURTHER INSPECTION 10A Water stain was noted to the formal dining room ceiling. See 10A on diagram. RECOMMENDATION: The owner or their representative to contact licensed roofing contractor to make further inspection and any needed repairs to obtain an adequate guarantee. ATTENTION: This inspection by MILLENNIUM TERMITE & PEST includes all visible structural wood members of the roof, excluding the wood shingles. OTHER INTERIOR CONTINUED: SECTION 2 FINDING 10B Water was noted leaking from the 1st story hall bathroom sink plumbing. See 10B on diagram. RECOMMENDATION: Make necessary repairs to correct the above reported leak. SECTION 2 FINDING 10C The toilet was noted loose in the master bathroom. See 10C on diagram. RECOMMENDATION: Remove the toilet and old wax seal. Install new seal and reset the toilet with new bolts. FURTHER INSPECTION 10D Water stains were noted to the living room wall below the master bathroom stall shower. See 10D on diagram. RECOMMENDATION: Remove previous repair patch for further inspection. If no damage, Re-install sheetrock to match as close as possible. ATTENTION: If additional damage is found extending into areas not visible at the time of inspection, a Supplemental Report will be issued on findings and the owner or their representative will be contacted with additional costs. ATTENTION: If the above noted repairs are completed by parties other than MILLENNIUM TERMITE & PEST an Interim Inspection will be required before any finished products are installed. SECTION 2 FINDING 10E Previous leak has caused water damage to the kitchen storage shelf. See 10E on diagram. RECOMMENDATION: Make necessary repairs to damaged storage area. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. OTHER EXTERIOR: SECTION 1 FINDING 11A Fungus (dry rot) damage was noted to approximately 45 linear feet of 2x6 corner trim, approximately 98 linear feet of 2x6 face trim and approximately 30 linear feet of 2x6 window trim. See 11A on diagram. RECOMMENDATION: Remove and replace damaged wood members with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. RECOMMENDATION: Remove and replace damaged corner trim with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. SECTION 1 FINDING 11C Fungus (dry rot) damage was noted to approximately (5) sheets of T1-11 wood siding on the 1st story and approximately

(4) sheets of wood siding on the 2nd story of the structure. See 11C on diagram. RECOMMENDATION: Remove damaged portion of wood siding. Inspect wood members for further damage and treat adjacent wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR). Install new wood siding to match as close as possible. ATTENTION: If additional damage is found extending into areas not visible at the time of inspection, a Supplemental Report will be issued on findings and the owner or their representative will be contacted with additional costs. ATTENTION: If the above noted repairs are completed by parties other than MILLENNIUM TERMITE & PEST an Interim Inspection will be required before any finished products are installed. SECTION 1 FINDING 11D Fungus (dry rot) damage was noted to approximately 48 linear feet of fascia board on left side of the structure and trim off

the end of fascia board on the right back corner of structure. See 11D on diagram. RECOMMENDATION: Remove and replace damaged fascia board with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. SECTION 2 FINDING 11E Stucco voids were noted to the exterior stucco in various areas of the structure. See 11E on diagram.

RECOMMENDATION: Remove the loose stucco, prepare area(s) and re-stucco exposed area(s) to match texture as close as possible.

SECTION 1 FINDING 11F Fungus (dry rot) damage was noted to 6 linear feet of 2x4 rafter tails on the left side of the structure. See 11F on diagram. RECOMMENDATION: Remove and replace damaged rafter tails with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction.

OTHER EXTERIOR CONTINUED: SECTION 1 FINDING 11G Fungus (dry rot) damage was noted throughout the entire 2nd story balcony at the back of the structure. See 11G on

diagram.

RECOMMENDATION: Remove the entire balcony. Pull back carpet in the master bedroom. Remove up to 120 square feet of plywood subfloor. Install (10) 2x10 joist cantilever into house, each joist approximately 15 linear feet. Install new 2x6 decking and handrails. Repair stucco as necessary to match as close as possible. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction.

SECTION 1 FINDING 11H Fungus (dry rot) damage was noted to approximately 40 linear feet of the 2x6 barge rafter in various areas of the structure.

See 11H on diagram.

RECOMMENDATION: Remove and replace the damaged wood members with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction.

SECTION 1 FINDING 11I Fungus (dry rot) damage was noted to approximately 1 foot of the V-rustic roof sheathing at back of the structure. See

11I on diagram. RECOMMENDATION: Remove the exterior roofing to expose damage. Remove and replace the visible damaged roof sheathing and treat adjacent wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR). Re-install exterior roofing to match as close as possible. ATTENTION: If additional damage is found extending into areas not visible at the time of inspection, a Supplemental Report will be issued on findings and the owner or their representative will be contacted with additional costs. SECTION 1 FINDING 11J Fungus (dry rot) damage was noted to approximately 5 linear feet of the 2x3 bottom collar on the column in front of the

structure. See 11J on diagram.

RECOMMENDATION: Remove and replace the damaged 2x3 collar with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction.

OTHER EXTERIOR CONTINUED: SECTION 1 FINDING 11K Fungus (dry rot) damage was noted to approximately (20) pieces of wood shingle siding at the front of the structure above

the garage and the column. See 11K on diagram. RECOMMENDATION: Remove damaged wood shingles. Treat all affected areas with an approved chemical fungicide (DISODIUM, OCTABORATE and TETRAHYDRATE SOLUTION) and install new shingles as needed. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. SECTION 1 FINDING 11L Fungus (dry rot) damage was noted to approximately 10 feet of 2x12 trim above the garage door. See 11L on diagram.

RECOMMENDATION: Remove and replace the damaged 2x12 trim with new material and treat adjacent or exposed wood members with an approved chemical fungicide (BORIC ACID SOLUTION/TIMBOR) to correct the above noted condition. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction.

SECTION 1 FINDING 11M Fungus (dry rot) damage was noted to the 4x12 header beam at the front right of the structure. See 11M on diagram. RECOMMENDATION: Cut off necessary portion of the beam and treat ends with an approved chemical fungicide (DISODIUM OCTABORATE and TETRAHYDRATE SOLUTION/TIMBOR). ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. SECTION 2 FINDING 11N Approximately 12 feet of the gutter was noted deteriorated and rusted causing the gutter to tear and pull apart from itself.

See 11N on diagram. RECOMMENDATION: Remove damaged portion of the rain gutter and replace with new to match as close as possible. ATTENTION: If portion(s) of gutter cannot be replaced, a complete new gutter system will have to be installed and owner or their representative will be contacted with additional costs. ATTENTION: During the performance of repairs at the above mentioned areas, should any additional damage or conditions be found that were not visible or accessible at the time of the inspection without removing lumber or finished products, the owner or their representative will be contacted for direction. NOTED ITEMS: Moss growth was noted on the ends of shingles around the roof overhang. DISCLOSURE: This estimate does not include any fees for plans or permits as required by local building department. Building permits are the responsibility of the owner,

unless otherwise agreed upon in writing on this agreement. Any additional requirements to comply with the local building department or governing bodies will be in addition

to this estimate. Any changes that may have to be made to comply with local governing bodies will be the responsibility of the buyer and will be in addition to this estimate.

Any material selected by buyer that exceeds the budgeted amount will in addition to this estimate. All work is to be completed in a professional workman like manner

according to standard practices. There will be an additional charge for any alteration or deviation from the above specifications. This may include $100 per change order and

any additional cost for labor and materials. Changes will be executed only upon an additional signed agreement and will be an additional charge over and above this original

agreement. Buyer will be responsible for any additional cost incurred when digging and there are unforeseeable objects such as rock, tree stump/roots, hardpan etc. There

will be a 25% return fee for any change in materials once they have been purchased. This will apply only if the purchased material can be returned and 100% refunded by the

supplier. In such case as the supplier will not give a refund the owner will be responsible for 100% of the cost for such material. All material must be available at local

wholesale distributors. Any retail client-requested material will incur additional charges. Once plans/designs have been approved meaning work has commenced, Millennium

will not be responsible for any errors in the plans/designs. Millennium shall not be held responsible and the buyer agrees to assume full responsibility for any damage to

underground utility lines such as telephone, electrical, sewer, gas, cable, existing irrigation, drainage or other unforeseen underground items. Any unforeseen natural disasters

are not covered under this warranty and will void the warranties below.

ATTENTION: While performing these treatments in a professional manner, Millennium Termite & Pest, assumes not responsibility of any damage done to pipes that may be

sealed under the concrete slabs or damage to floor coverings. We will reset carpet and pad if necessary. Millennium Termite & Pest is not responsible for re-stretching

carpet. Owner may desire to contact a carpet company to have it re-stretched at the owners expense.

NOTICE TO OWNER: Materials: All materials used are selected for their efficiency prior to use on your property. The materials used shall conform with Federal and state

ordinances. The work will be performed in a professional manner. Guarantee: If Subterranean termites penetrate the barrier laid by Millennium Termite & Pest, our

technician will , by mutually agreed appointment, retreat the areas of infestation to assure effective results. Your cooperation is necessary to prepare the area for is necessary

to prepare the area for treatment. In case these areas are not prepared, or are unavailable for the recommended treatment listed, the job total is subject to an additional $78.00

per man hour.

DAMAGE CAUSED BY INSECTS: Millennium Termite & Pest is not responsible for any past, present or future damage of the structure(s) or its contents caused by wood-

destroying insects, or any costs or expenses incurred by the customer as a result of any such insect caused damage.

MOLD DISCLAIMER: There may be health related issues associated with the structural repairs reflected in the inspection report referenced by this work authorization

contract. These health issues are not limited to the possible release of mold spores during the course of repairs. MILLENNIUM TERMITE & PEST is not qualified to and do

not render any opinion concerning such health issues or any special precautions. Any questions concerning health issues or any special precautions to be taken prior to or

during the course of such repairs should be directed to a Certified Industrial Hygienist before any such repairs are undertaken. By executing this work authorization contract,

customer acknowledges that he or she has been advised of the forgoing and has had the opportunity to consult with a qualified professional.

MECHANICS LIEN: Notice: The “Notice to Owner” form to be used by branch 1 and branch 3 registered companies in accordance with Section 8513 of the code shall be

that set forth below.

Under the California Mechanics Lien Law any structural pest control company which contracts to do work for you, any contractor, subcontractor, laborer, supplier or other

person who helps to improve your property, but is not paid for his or her work or supplies, has a right to enforce a claim against your property. This means that after a court

hearing, your property could be sold by a court officer and the proceeds of the sale used to satisfy the indebtedness. This can happen even if you have paid your structural

pest control company in full if the subcontractor, laborers or suppliers remain unpaid.

To preserve their right to file a claim or lien against your property, certain claimants such as subcontractors or material suppliers are required to provide you with a

document entitled “Preliminary Notice.” Prime contractors and laborers for wages do not have to provide this notice. A Preliminary Notice is not a lien against your

property. Its purpose is to notify you of persons who may have a right to file a lien against your property if they are not paid.

WARRANTY: All labor and materials will be warranted for one year from the date of completion unless otherwise specified in writing on this agreement. Please see below

for additional warrantees.

* Irrigation and Drainage ~ 1 year from date of completion. Proper care and cleaning of theses systems as described by the manufacturer are the buyer’s responsibility.

Improper maintenance may void all warranties.

* Lighting ~ 1 year from date of completion. Proper care of the lighting system as described by the manufacturer is the buyer’s responsibility. Improper maintenance may

void all warranties. Millennium is not responsible for physical damage or light bulbs that may burn out.

* Concrete ~ 1 year from date of completion. Millennium is responsible for all structural cracks however assumes no responsibility for normal cracks 1/8 of an inch wide

and smaller. Buyer is responsible for all maintenance including sealant when necessary. Failure to due so may void this warranty. Attention: All warranty work is separate

from this agreement and will not delay payment schedule.

Terms: For property in escrow, payment in full is due at the close of escrow, not to exceed 60 days. Customer understands that they are responsible for payment in full

should escrow cancel or go beyond terms stated herein For non-escrow property, a deposit of 1/4 down is required prior to the start of work and the remainder is due at

completion. Should any balance be left unpaid thirty (30) days after work is completed, the entire balance shall become immediately due and payable. Any balance

remaining 30 days after completion of work shall be charged interest at the rate of 1 ½ % per month, or 18% per year. “We shall have the right, at our option, to institute and

pursue legal proceedings for collection of all amounts owed.” The undersigned, jointly and severally, by acceptance of this contract, agree to pay all costs of collections

and/or suit and reasonable attorney fees for the prosecution of the same determined by the court. A $25.00 charge will be made for any returned checks. This Estimate is

Valid for 30 Days from the Date of the Estimate. Permit fees, as necessary, will be paid directly by the owner. All change orders are subject to additional charges, with a

minimum charge of $100.00 per change.
