What do I need to do to apply for my newborn’s Birth Certificate?Please bring the following information with you to the hospital on the day you are expected to deliver:n The mother’s/father’s date of birth, place of birth and social security number(s). n If you are legally married and do not have the same last name, bring your

marriage license or certificate of ceremony. n For same sex marriages:

• Bring your marriage license so that the other parent’s name can be added to the Birth Certificate.

• The Department of Health does not accept verbal proof of marriage.

The Registrar’s office or your Nurse will give you a “Mother/Parent” form to be completed. This form will be given to the Birth Registrar’s Office within 48 hours after your newborn is delivered.

What will the Birth Registrar do with this information?The Birth Registrar will put the information into a special computer system. The Bureau of Vital Records for the New York City Department of Health will receive and process your information.

When will I receive my baby’s Birth Certificate?A Birth Certificate will be mailed to you within 3-4 weeks. If you do not receive it after four weeks, you should call 311. They can tell you when you should expect the birth certificate.

How do I add a father’s name to the Birth Certificate if we are not legally married?n If you were not legally married at the time you gave birth, the law will not

recognize the father as the newborn’s legal father. n If you are not married, you will be asked to sign a “Voluntary Acknowledgement

of Paternity” (AOP) form to help you establish parenthood.

n If you decide to fill out this form you will need: • The social security number of one or both parents. • Please let us know if:

– You or the father does not have a social security number. – If you or the father does not live in this county or is out-of-state.

Where can I get an Acknowledgement of Paternity (AOP) form?n Acknowledgement of Paternity forms are available at:

• The Birth Registrar’s office at Forest Hills Hospital • The New York State Department of Health

The Voluntary Acknowledgement of Paternity form should not be signed if:n The mother was legally married to anyone at the time she became pregnant or

during pregnancy up until the baby’s birth.n The mother is separated and not legally divorced.n The man is not sure that he is the father.

Can parents have a DNA test done at the hospital at the time of delivery?n Forest Hills Hospital does not perform DNA testing. n If you would like DNA testing, please call: The Office of Child Support

Enforcement at (212) 480-7564.

Information about other important documentsn Social Security Card:

• To receive a Social Security card for your newborn, please check “yes” to question number 2 on the “Mother/Parent” form.

• A Social Security card will be mailed to you within one month. • If you do not receive the card or want to apply on your own, please call

(800) 772-1213 or visit your local Social Security Office or their website ssa.gov.

n Medicaid Insurance Card: • If the mother/parent has Medicaid insurance, their Medicaid information

will be sent to the Bureau of Medicaid enrollment and the mother/parent will receive a Medicaid insurance card for their baby within 2-3 weeks.

• If you have any questions about Medicaid Insurance, please call the Medicaid office or if you wish to apply for Medicaid for your newborn call, the HRA Medicaid Helpline at: (888) 692-6116.

• Confirmation of Birth: Once you complete the “Mother/Parent” form, the Birth Registrar will give you a letter that says your newborn was born at Forest Hills Hospital.

• Please let the Birth Registrar know when you hand in your form so the letter can be prepared for you before you are discharged.

n You will need proof of your newborn’s birth before you can apply for Medicaid insurance or WIC.

If I did not name my newborn right away, how do I add his/her name to the birth certificate?n If you did not name your newborn at the time of birth, you can add your

newborn’s name by adding it to the Birth Registration. n There is no extra cost for additions made by the hospital as long as they are

sent in within 12 months of your newborn’s birth. n To start this process, please complete a Birth Correction Application and send

it in with the newborn’s original Birth Certificate. You will receive a new Birth Certificate in about four weeks.

How do I correct mistakes made on my newborn’s Birth Certificate?n If the Birth Registrar made a mistake(s) on your newborn’s Birth Certificate, fill out

a “Birth Correction” application. Write down all of the mistakes that were made. • You can find “The Birth Correction” Application at Forest Hills Hospital or

online at: www.nyc.gov/health.

Welcome to your new family member!Congratulations and thank you for choosing Women’s New Life Center at Forest Hills Hospital to deliver your newborn. The information in this booklet will help you and your family understand what you need to do to receive your baby’s Birth Certificate.

It is very important for you to register your newborn’s birth. The Birth Certificate is his or her first legal document. It provides your state with a record of the birth and is important for every newborn because it shows your child’s identity and nationality.

The Birth Registrar is a person who is available in the hospital to help you understand the New York City Department of Health guidelines and laws for reporting your newborn’s birth.

Free interpretation services are available if you prefer to discuss your health care in a language other than English.
