What is COPD quality of care program?

COPD : definition

►Chronic Obstructive Pulmonary Disease (COPD) is a

common, preventable and treatable disease that is

characterized by persistent respiratory symptoms and airflow

limitation that is due to airway and/or alveolar abnormalities

usually caused by significant exposure to noxious particles or

gases.

► COPD is currently the fourth leading cause of death in the world.1

► COPD is projected to be the 3rd leading cause of death by 2020.2

► More than 3 million people died of COPD in 2012 accounting for 6% of all deaths

globally.

► Globally, the COPD burden is projected to increase in coming decades because of

continued exposure to COPD risk factors and aging of the population.

Spirometry criteria for airflow limitation

▪ Spirometry is fundamental in the diagnosis, prognosis, and discussion around non-pharmacologic treatment such as surgery.

▪ Post bronchodilator FEV1/FVC < 0.7 confirms the persistence of airflow limitation

Prevention of COPD exacerbations : Non -pharmacological treatment

▪ Smoking cessation

▪ Pulmonary rehabilitation

▪ reduce symptoms, improve functional capacity and increase patient participation Studies indicate that it also may reduce the frequency and duration of subsequent exacerbations

▪ Rehabilitation delivered in the post-acute recovery stage may also reduce the risk of readmissions and even mortality

▪ Influenza vaccine

▪ rate of hospitalizations due to influenza and pneumonia was significantly reduced (adjusted risk ratio 0.48, p=0.008), as was risk of death (adjusted odds ratio 0.30, p<0.001)

► Initial therapy : a single long acting bronchodilator.

► In two head-to head comparisons the tested LAMA was superior to the LABA regarding exacerbation prevention, therefore we recommend starting therapy with a LAMA in this group.

▪ เรมโปรแกรมฟนฟสมรรถภาพปอด (pulmonary rehabilitation) ในผปวยกลมอาการปานกลาง(moderate)ขนไป

▪ ในผปวยกลมอาการรนแรง (severe) ขนไป

- แนะน าทางเลอกในการรกษาโดยการผาตด (lung volume reduction surgery – LVRS) ในผปวยทมขอบงช

COPD : monitoring and follow up ▪ Monitoring disease progression and development of complications and/or

comorbidities

► Measurements. Decline in FEV1 can be tracked by spirometry performed at least once a year.

► Symptoms. At each visit, information on symptoms since the last visit should be collected, including cough and sputum, breathlessness, fatigue, activity limitation, and sleep disturbances.

► Exacerbations. The frequency, severity, type and likely causes of all exacerbations should be monitored.

► Imaging. If there is a clear worsening of symptoms, imaging may be indicated.

► Smoking status. At each visit, the current smoking status and smoke exposure should be determined followed by appropriate action.

COPD : monitoring and follow up

▪ Pharmacotherapy and other medical treatment

▪ In order to adjust therapy appropriately as the disease progresses, each follow-up visit should include a discussion of the current therapeutic regimen.

▪ Monitoring should focus on:

► Dosages of prescribed medications.

► Adherence to the regimen.

► Inhaler technique.

► Effectiveness of the current regimen.

► Side effects.

training a starting intensity should be 80% of the average 6MWT speed

▪ To calculate an appropriate intensity for walking laps: 80% ([6MWD ÷ 6] x prescribed duration)

▪ Six-minute walk distance (6MWD) ÷ 6 = Distance in one minute

▪ For distance in 30 minutes = one minute distance x 30

▪ For distance in 20 minutes = one minute distance x 20

▪ For distance in 10 minutes = one minute distance x 10 etc…

Example:

If the patient walked 220 m in six minutes:

One minute distance = 220 ÷ 6 = 36.7 m.

30 minute distance = 36.7 x 30 = 1100 m.

80% of 1100 = 880 m in 30 minutes.
