ยาทควรหลกเลยง เหตผล
ยาลดนำตาลในเลอดChlorpropamideGlibenclamide
• T1/2 ยาวมากในผสงอาย ทำใหเกดภาวะนำตาลตำทรนแรง โดยความเสยงจะเพมขนตามอาย
• อาจทำใหเกดภาวะ SIADH
ยาระบบหวใจและหลอดเลอดNifedipine (กลมทออกฤทธทนท)
• มโอกาสเกดความดนตำอยางมากในครงแรกท ไดรบยา• เพมการเตนของหวใจ (reflex tachycardia) • อาจสงเสรมใหเกดภาวะหวใจขาดเลอด
ยาจตเวชBenzodiazepine ทออกฤทธยาว เชน diazepam, chlordiazepoxide, dipotassium chlorazepate
• อาจทำใหม impaired cognitive function• เพมโอกาสการหกลม • ควรลดขนาดลงชาๆ แลวเลอกใชยากลมอนแทน ยกเวนเปน

Tertiary TCAs
Amitriptyline, clomipramine, doxepin ในขนาดมากกวา 6 มลลกรม
• มฤทธ3 anticholinergic สง ทาใหเกดผลขางเคยง ไดแก
ปสสาวะขด ทองผก ความดนโลหตตEา การมองเหนผดปกต
และ CNS ทางานผดปกต
• ควรเลอกใชยากลมอEนแทนBarbiturates
เชน phenobarbital
• อาจกด CNS รนแรง ทาใหเกดภาวะเศรา
• มโอกาสตดยาสง
• เพEมโอกาสหกลม
• Therapeutic index แคบ
• CYP inducer มโอกาสเกดปฏกรยาระหวางยาไดงาย
• ควรลดขนาดลงชาๆอยางระมดระวง
• เลอกใชยากลมอEนแทนยาบรรเทาปวด
Pethidine
• ทาใหเกด delirium และอาการงวงซมไดบอย
• Metabolite ของยาอาจทาใหชกได
