Job assignments: When you are assigned to a job, you will be told what the job duties are, any tools required, the location of the jobsite, the rate of pay, and if you will be required to use safety equipment. You are always free to decline a job for any reason. If you need an accommodation to perform the job duties, please let us know.

Safety is our No. 1 priority. We want it to be yours, too! You must wear and/or use any safety equipment that might be required for the type of work being performed. Please call the branch immediately if the job is unsafe or different from what the branch staff explained. Don’t work in any job you feel is unsafe.

Transportation is your responsibility. You are free to travel to the jobsite in any manner you choose, but remember the importance of arriving at the jobsite on time. If you do not have transportation, we will try to give you an assignment within bus routes or in areas that are convenient.

Wages: The wages you earn are paid according to the Fair Labor Standards Act (FLSA) or applicable state law. You’ll be paid overtime for hours worked over 40 in one week or as required by state law. In addition, you should receive breaks and meal periods if required by state law. If these are not provided, please notify branch staff.

Survey Requirements:

1. If you previously took the Survey and qualified, you will never need to retake the Survey. Inform the staff if you

have already completed the Survey and provide the results.

2. Show valid photo identification to prove your identity.

3. If you need assistance and/or reasonable accommodation to take the Survey, please let the branch staff know

before you start the Survey.

4. You will be notified if you are “qualified” or “not qualified”. If you are “qualified” you are eligible to proceed in the

hiring process. If you are “not qualified” you can retake the Survey 12 months from the date of your non-

qualification.

I have read, understand and agree to cooperate with the company’s application process. I also understand that

the information I provide about me on application forms, surveys, tests, and during interviews will be used in

making hiring decisions, and I consent to it being used for this purpose. I agree to keep the contents of this

Survey confidential and will not share the Survey questions with anyone.

I agree that any disputes arising out of my application for employment or unemployment, including, but not limited

to, any claims related to failure to hire, wage and hour laws, discrimination, harassment or wrongful termination

that I believe I have against TBI and its subsidiaries or its agents or representatives, and all other employment

related issues will be resolved by final and binding arbitration as my sole and exclusive remedy. Except where

prohibited by law, I agree to bring any disputes I have as an individual and I waive any right to bring or join a class

or representative action. I acknowledge that my dispute will be decided by a neutral arbitrator and not by a judge

or jury. The arbitration shall be conducted by the American Arbitration Association under its Employment

Arbitration Rules. The decision of the arbitrator shall be final and binding. I understand that the company also

agrees to arbitrate in the same manner any claims which the company believes it has against me and the

company will pay for the arbiter’s fees where required by law. This agreement is enforceable under the Federal

Arbitration Act.
