So many recommendations have been put in place including inherent safety design recommendations, Safety Management Systems and the Safety case. The behaviours and language of safety culture have also changed yet some mistakes have been made again. In a moving new piece called PIPER ALPHA, from the experienced Andy Powrie of AKT (Deepwater, Fukushima, The Mousehole, NIMROD) the story is re-told with relevance for the workforce who remember the tragedy and those who have never heard of it. “... these ideas about storytelling are directly relevant for accident prevention.
