Local Insulin Device Error Incident Patient discharged on pre-filled insulin pen previously using a re-usable pen
Hospital discharge did not state the type of device the patient had been discharged on which resulted in the GP practice con-
tinuing to supply cartridges for the re-usable pen. When the hospital supply finished, care staff went back to using the medi-
cation issued by the practice. Having become habited to using pre-filled pens, staff assumed practice had sent the wrong item.
(Leading to confusion and delay in patient receiving correct insulin).
This incident occurred due to: 1) discharge letter not clearly stating device patient had been discharged on 2) care staff lack of
knowledge & experience in difference between different devices. This serves as a reminder that not only is it important to
reconcile medication but also devices when a patient is discharged.
NHSE Updates After-care needs of inpatients re-covering from COVID-19

Improving allergy documentation has been highlighted as a key area to mitigate the potential increased antimicrobial resistance that may have occurred during the recent pandemic. If the following information is rec-orded on the patients GP records this can be used to inform decision mak-ing. This will potentially reduce inappropriate use of broad spectrum anti-biotics, lead to better outcomes, reduce resistance and lead to significant cost savings.
When documenting a new allergy include the following information:
Severity
1 What is the nature of the allergy? (is it an adverse reaction or a true allergy?)
2 What are the details of the reaction?
3 How was the reaction managed? What was the outcome?
Timing
4 How many hours after having the first dose of the antibiotic did the reaction occur?
5 How many years ago did the reaction occur?
Tolerance
6 What other antibiotics have been tolerated since the reaction?
