Veronica has a strong background of managing, operating and consulting for provider medical office practices. Her experience includes work with Central Ohio Primary Care, The Ohio State University, Northwestern University and Huron Consulting Group, a nationally renowned medical practice consultant. She previously served as Regional Operations Director for Davita Healthcare Partners, Inc., overseeing the overall management of 20 clinical sites. In this role, Veronica will be responsible for providing leadership in developing and executing an integrated approach to operations for our physician practices and medical group, which is in support of the overall business plan and strategic direction of FMC. Veronica shares the FMC leadership vision of a collaborative, team-approach in all patient, provider and practice interactions, in accordance with the established values and common purpose of the FMC. She will partner with Dr. Welsh, Chief of Medical Affairs, to advance the mission of FHP to provide exceptional care and experience for our patients and their families. HR CornerUpdate Personal Contacts in Employee Self-ServiceIf you cover dependents on your medical, dental or vision plan, please make sure you have their date of birth and social security numbers listed in Employee Self-Service (ESS). It is important our carriers have this information on your dependents for proper coverage and eligibility. Go to ESS, click on the Employee tab, then click on Personal Contacts. Add/edit the appropriate dependents.

Consumer Driven Health Plan and Health Savings AccountIf you are enrolled in the Consumer Driven Health Plan (CDHP) and you do not have a Health Savings Account (HSA) bank account on file in Human Resources, please remember to open an account at the banking facility of your choice. Although opening a HSA is optional, in order for FMC to contribute $500 per covered members on your medical plan into your HSA, you must provide account information to us. Please see page 28 in the Employee Benefit Guide or call HR at 740-687-8017 for more information.

When Closing a Direct Deposit Bank Account-Inactivate in ESS and Notify AccountingPlease remember that if you close a bank account that is used for direct deposit, you must inactivate the closed account in your Employee Self-Service (ESS). You will then need to notify Accounting that you closed your bank account in ESS. This includes closing a Health Savings Account. Please call Kevin Effinger at 740-687-8041 or email him at [email protected] to inform him that you closed an account or for more information.

MedBen WellLivingIf you are enrolled in the medial plan, please be reminded the deadline to complete the wellness screenings is December 31, 2016 to receive the incentive beginning in April of 2017. Visit MedBen’s website at medben.com and click on “MedBen Access” to view your WellLivinsg dashboard. A flier also is available in Human Resources to assist with logging onto the website. This dashboard will show the tests you have achieved credit for and the dates for the next expected time to achieve the screening tests. If you have any questions about the program, contact Human Resources or MedBen’s Customer Service at 800-686-8425.

There will soon be a bookshelf full of entertainment for patients and their loved ones. We are asking for donations of gently-used board games for all ages that can be played quietly without dice rolling. Examples are Battleship, Pictionary, Checkers, Sequence and Uno. There will be a box in the Marketing Department for donations. Please turn them in by Sept. 28. A (slightly) fictionalized account: When the Totally 2000’s team held a paper airplane contest on August 24 as a part of their 1916 Campaign fundraising, they expected the competition to be fierce – and their participants delivered. The contest raised $150 for the Fairfield Medical Center Foundation and provided many paper-pilots with an enjoyable mid-day diversion. But now we have to ask ourselves if some people might have taken this a little too seriously.

Medals were presented to contest winners in two categories: Design and Distance. Design was judged by a panel of judges utilizing advanced independent algorithms to assess the overall aesthetics of each entry. The Design winners were: Chad Cupp, Ed Krile and Ryan Kemper. Distance was determined by the measuring linear inches each plan traveled from the starting line to its post-flight point of rest. The Distance winners were: Chad Cupp, Janet McLain and Lynne Shultz. Chad Cupp spoke to reporters following the medal presentation and was misquoted as follows: “Obviously, winning the double gold is a great honor. I worked hard for this, I trained hard for this, and I just wanted it a little more than everyone else. I’m not saying that I’m the best paper airplaner of all time, but some people are saying that… and who am I to argue with them?” The Totally 2000’s team thanks all who participated, and hopes that all employees will continue to have fun while supporting the Fairfield Medical Center Foundation.

It’s time to get your voices ready for the FMC Christmas Choir. The choir performs at our annual Christmas party, scheduled for Dec. 9, and other events, as requested. All are welcome. Our choir has been singing together more than 25 years and we share in the love of making a joyful noise! TWIG 17 Books & More Sale

FMC now has a mandatory influenza vaccination program. This policy is in effect for all departments of FMC and their workforce including contracted staff, students, volunteers and licensed independent practitioners.

ALL FMC HEALTHCARE WORKERS ARE REQUIRED TO RECEIVE AN ANNUAL INFLUENZA VACCINE.

Why are we making the flu shot mandatory?The Joint Commission and CDC recommend an annual influenza vaccination for all healthcare workers. The benefits of mandatory vaccination include: protecting our patients, protecting our healthcare workers and protecting our community.

What is our goal? The most effective way to prevent infection from an influenza virus is through annual influenza vaccination. Annual influenza vaccination protects our patients and the integrity of the Fairfield Medical Center workforce. The goal is to reduce the transmission of influenza viruses in the healthcare setting, while promoting the health and safety of patients, healthcare workers and the community.

Will the flu shot be free to healthcare workers? YES. The flu shot kick-off day will be Oct. 17 in the FMC Assembly Rooms from 6 a.m.-6 p.m.

What Do I Do to Prepare for My Flu Vaccine?Every employee and employed provider must go to ReadySet to complete your influenza survey at: https://fmchealth.readysetsecure.com/rs/ (there is an icon on the desktop of most computers). The ReadySet survey will be available on Sept. 15.

What happens if I don’t receive the flu shot?Employees and employed licensed independent practitioners who fail to report receipt of influenza vaccination or do not file an approved exemption by Dec. 15 will proceed through progressive disciplinary action. Volunteers who do not receive the vaccine may not provide service between Dec. 15 and March 31. Students and independent contractors who are not vaccinated will not be permitted onsite between Dec. 15 and March 31.

What are the exemptions?Medical Exemptions: Individuals who request exemption from the vaccine due to medical contraindication must submit medical documentation stating the reason that the vaccine is contraindicated, signed by a licensed medical practitioner. Religious and Spiritual Exemptions: Individuals requesting a religious or spiritual exemption from the influenza vaccine will be required to submit documentation explaining the specific beliefs that prohibit them from receiving the vaccine.

How Do I Get an Exemption Form?There will be a link in the ReadySet system as you fill out your survey. Pick up a form in Employee Health Monday-Friday from 7:30 a.m.-4 p.m. Documents also will be searchable in the document center through the Intranet. ALICE (Active Shooter) Training for Staff – The Fairfield Medical Center Police will offer ALICE (Active Shooter) Training on Sept. 13 or Sept. 27 from 8 a.m.-1 p.m. in the Accounting Training Room. ALICE (Alert, Lockdown, Inform, Counter, Evacuate) Training provides preparation and a plan for individuals and organizations to proactively handle the threat of an aggressive intruder or active shooter event.
