The Capacity & Flow Team is a dedicated team who provide highly skilled and professional support in directing patient flow across the acute division. Clinical site co-ordination supports patient flow to ensure timely bed availability, admission, discharge, transfer and appropriate placement of patients across Emergency and Planned Care Directorates, Scheduled and Unscheduled care and operates within the delivery of the 4 hour Emergency Access target.

This is an exciting and challenging role for an individual to make a significant contribution to the delivery of hospital site management. The clinical co-ordinator role is significant in the

management of Acute beds and will work closely with primary care colleagues promoting patient flow to downstream hospitals as well as managing transfer to and from tertiary care.

You will be an experienced first level nurse with extensive post-registration experience with proven leadership experience and a focus on high standards of quality, patient centered care delivery.

This challenging post requires excellent interpersonal and decision making skills and you must be able to work autonomously and have refined and effective communication skills.

You will work within the Fife Primary Care Emergency Service to provide a safe, patient centred and effective clinical nurse consultation to the population of Fife and Kinross. You will develop the role of Advanced Paediatric Nurse Practitioner (APNP) by working autonomously and as part of the multidisciplinary team.

The trainee APNP is an experienced and highly educated Registered Children’s Nurse who will manage the complete clinical care for their patient, not solely any specific condition.

The APNP will assess and treat children and young people aged 0 – 16 year, utilising high levels of decision making according to local and national guidelines and the latest evidence based research to ensure patient centred care in partnership with the patient, families and other persons with caring responsibilities for that individual.

The APNP will work collaboratively across healthcare boundaries to develop the service to improve practice, implementing and evaluating evidence based standards, guidelines and policy in regards to management of the ill child. The APNP role encompasses that of expert clinician, educator and auditor.

This is a brilliant opportunity to develop nursing practice within the specialism of community based urgent care. As a Registered Children’s Nurse with substantial post registration experience, you will be educated or are expected to complete at MSc level in an Advanced Practice Paediatric and Neonatal Qualification and Independent Nurse Prescriber qualification. You will also have experience developing the practice of others including teaching, practice supervision and the application of research and audit underpinned by a strong knowledge of current Urgent Care issues. Excellent leadership, team working, advanced clinical assessment and decision making skills are all key to this role.

To meet the needs of Primary Care Emergency Services hours will include a range of shift patterns which include, evenings, nights, weekends, day-time and public holidays. Travel across Fife will be required with PCES centres at Queen Margaret Hospital, Dunfermline, Victoria Hospital, Kirkcaldy, Glenrothes Hospital and St Andrews Community Hospital.

We will support you with access to personal development planning, clinical supervision, reflective practice, mentorship and a structured competency framework.

The Urology Service currently have an exciting opportunity for a Advanced Nurse Practitioner, Band 7, working within Ward 54 ,UDTC and AU2 at Victoria Hospital, Kirkcaldy to work within the urology multidisciplinary team inpatient/outpatient service and to Lead and support in nurse led /triage/ patient pathway management through to discharge .

If successful you would be joining a well-motivated and dynamic urology team, who strive to deliver the highest level of patient centered care at all times.

You are required to be an individual with experience in clinical assessment, a team player with good written and oral communication skills. You will be expected to be accountable for care delivery, work well individually and also as a part of a multi-disciplinary team.

You will have to show a genuine interest in caring for all urological conditions.

A vacancy has arisen in the Children’s Unit at Victoria Hospital, Kirkcaldy for a Trainee Advanced Paediatric Nurse Practitioner. Initially this is a training post being appointed to under Annex 21 which therefore means during the first 12 months prior to completion of training you will be paid 75 per cent of the pay band maximum of the fully qualified Band 7 rate. Trainees are paid a percentage of the pay band maximum for the qualified rate which is dependent on the duration of the training period (see terms and conditions in job pack).

At present we have 2 qualified Advanced Paediatric Nurse Practitioner.

Our unit consists of an outpatient department, ambulatory care unit and an acute paediatric ward. We care for children requiring both medical and surgical nursing whilst working closely with the Neonatal Unit, Accident and Emergency department and Theatre Recovery and the tertiary centres in Edinburgh and Glasgow

The successful candidate is required to be qualified a paediatric nurse with excellent communication skills, have the ability to work on their own initiative and as part of the team. You will be expected to deliver a high standard of care and use a holistic approach to family centred care. You will undertake academic training to become an APNP and will be supported in your training and in gaining clinical experience on the Children’s Ward.

An up to date knowledge of evidence-based practice, and a commitment to learning and the delivery of clinically effective care are vital.

This key role will provide day to day management of the Hub supporting multidisciplinary teams across various clinical areas and maintain patient flow across the systems.

You will be responsible for the efficient running of the Hub and ensuring information is available to support patient flow across the whole system. The day to day deployment and coordination of staff within the Hub.

You will be responsible for liaising within the Partnership, and also external agencies to ensure effective joint working within the Health and Social Care system as the models develop. There will also be extensive development opportunities to support the re-shaping care agenda.

You will have an excellent understanding of partnership working and a proven track record of multi-agency collaboration.

We are a Level 3 Neonatal Unit, consisting of 4 ITU, 2 High Dependency and 14 Special Care cots.

The project will employ 2 staff and be run from Victoria NHS Hospital in Kirkcaldy, Fife. There will be close partnership working with hospital staff to engage patients who are identified as having insecure housing. Workers will follow a five-stage process to engage with the patient, undertake an immediate crisis response, work with them to identify key goals, put this into action focused on helping the person to develop skills and resilience, and finally support them to move on.

The project will focus on emergency admissions at VHK and Out of Hours. The target group will be individuals of any age and families who are homeless or at immediate risk of homelessness.  This project will be evaluated by Healthcare Improvement Scotland, who will work alongside the staff team in identifying outcomes and learning.

A key aim of this project is to share learning and build capacity with key stakeholders across Fife. Applications are thus being sought from staff in Fife Council Housing Services and NHS Fife who are interested in being seconded to staff the project. Key skills required include: needs assessment; experience of casework or care/support planning & delivery; experience of collaborative working. EXCITING SECONDMENT OPPORTUNITY

If you have well developed skills and experience in working with people who are experiencing homelessness, this could be an exciting career development opportunity for you to develop these skills within the community.

The NHS Fife Intervention Project is a highly innovative 18 month programme to test a new approach to providing services in NHS Scotland hospitals for those patients who are homeless. The project will be managed by Shelter Scotland and is supported by a multi-agency Steering Group led by Fife Council Housing Services and the Fife Health and Social Care Partnership. The key aims of this project are to:

improve working links between health and housing services provide homeless individuals with the right support to improve their health

outcomes reduce the re-admission rates among this cohort evaluate the preventative value of this approach in reducing pressures on

NHS Emergency Departments in Scotland.

The project will employ 2 staff and be run from Victoria NHS Hospital in Kirkcaldy, Fife. There will be close partnership working with hospital staff to engage patients who are identified as having insecure housing. Workers will follow a five-stage process to engage with the patient, undertake an immediate crisis response, work with them to identify key goals, put this into action focused on helping the person to develop skills and resilience, and finally support them to move on.

The project will focus on emergency admissions at VHK and Out of Hours. The target group will be individuals of any age and families who are homeless or at immediate risk of homelessness.  This project will be evaluated by Healthcare Improvement Scotland, who will work alongside the staff team in identifying outcomes and learning.

A key aim of this project is to share learning and build capacity with key stakeholders across Fife. Applications are thus being sought from staff in Fife Council Housing Services and NHS Fife who are interested in being seconded to staff the project. Key skills required include: needs assessment; experience of casework or care/support planning & delivery; experience of collaborative working. A significant programme of staff training and development will be a feature of this project.
