Little Leaguer’s ElbowCase I. C.S., a well-developed

eleven year old fast-baller, pitched theopening game of the Little LeagueSeason. Despite an inadequate warm-up he pitched a no-hitter and allowedonly one walk! However, by themiddle of the game he wasexperiencing pain over the medialepicondyle of the pitching arm withevery throw and could not really “beardown.” For several days afterwardthere was local tenderness andswelling over the right medialepicondyle and pain on extremeflexion or pronation of the forearm.

He vacationed for one month anddid not pitch or otherwise violently

exercise the elbow. The patient wasbrought to the hospital by his fatherwhen attempted resumption of hispitching career reproduced the initialsymptoms.

Roentgenographic study of theelbow revealed separation andfragmentation of the epiphysis of theright medial epicondyle and loss offascial marking about the elbowsuggesting edema or hematoma.

The patient was forbidden to pitch orotherwise strenuously exercise theelbow. One month later all symptomshad disappeared, but a clandestineexperiment on the part of the patientproved that pitching still elicited pain.
