5. NSAID in niedrigst möglicher Dosierung +/statt Paracetamol. Bei erhöhtem GI-Trakt-Risiko NSAIDs+Gastroprotektion oder COX-2-Inhibitoren. Kontraindikationen
6. Opioide±Paracetamol sind eine sinnvolle Alternative.
7. SYSADOA (Glucosaminsulfat,Chondroitinsulfat,Diacerein,Avocado-+ Sojabohnenextrakte,Hyaluronsäure) wirken symptomatisch und sind wenig toxisch. Caveats: effect sizes, Definition geeigneter Patienten, klinisch relevante Strukturmodifizierung + pharmakoökonomische Aspekte
8. Corticoide i.a. (US- oder Röntgen-gezielt) sind eine Option bei Schmerzen ohne Ansprechen auf NSAID oder Analgetika.
9. Osteotomie und gelenkserhaltende chirurg. Techniken sollten bei jungen Erwachsenen mit symptomatischer Hüft-OA eingesetzt werden, speziell bei Dysplasie oder Varus/Valgus-Deformität.
10. Gelenksersatz muss bei Patienten mit rad. gesicherter Hüft-OA bei Therapie-resistenten Schmerzen und Behinderung erwogen werden.

Clinical trials on hand OA should separately consider the localization (thumb-base, interphalangeal joints) and the stage or type of OA (non-erosive, erosive, nodal) and examine clinical predictors of response.
Thorough evaluation is required of physical treatments, such as ultrasound, laser, TENS, and local application of heat (eg paraffin wax, hot pack).
Studies are required to determine the most appropriate form or combination of exercise (eg strengthening, range of movement) for the different subsets of hand OA.
Further studies are required to better evaluate the symptom and structure modifying effects of SYSADOA.
The benefits of intra-articular injection of either corticosteroid or hyaluronan should be determined both for thumb-base and interphalangeal OA.
Existing slow acting anti-rheumatic drugs and biologic agents (especially anti-TNF therapy) should be investigated in erosive interphalangeal OA, to determine possible symptom benefits and structure modifying effects.
The efficacy and safety (both short and long-term) of paracetamol, weak opiods, and oral NSAIDs need to be assessed and compared.
The potential benefits of surgery compared to conservative management, and the most appropriate surgical procedure for thumb-base OA, remain to be determined.

NSAR bei OA
• Derzeit ist der Einsatz von NSAR zur Behandlung von Schmerzen bei OA empfohlen
• Die Hälfte aller Patienten mit symptomatischer OA verwendet orale NSAR
In der Kurzzeit-Therapie Placebo überlegen, Effektgröße ?
Langzeitsicherheit in der speziellen Patientenpopulation ?? (GIT, renal, cardiovascular)
1. NSAR sind ein unverzichtbarer Bestandteil in der Therapie schmerzhafter Erkrankungen des Bewegungs- und Stützapparates und sind im Kontext des Stufenschemas der WHO (Stufen 1 u. 2) zur Schmerzbekämpfung u. Entzündungshemmung anzuwenden.
2. Unerwünschte Nebenwirkungen werden bei etwa 5% der Patienten unter NSAR beobachtet und betreffen vorwiegend Störungen im oberen und unteren Gastrointestinaltrakt, Störungen der Nierenfunktion mit Wasserretention, Ödemneidung und Blutdruckanstieg, Störungen der Leberfunktion und der Hämopoese, sowie allergische Reaktionen.
3. Bei unzureichender Wirksamkeit bzw. Unverträglichkeit eines NSAR ist der Wechsel auf ein Präparat aus einer chemisch anderen NSAR-Gruppe indiziert, die gleichzeitige Gabe zweier unterschiedlicher NSAR ist zu unterlassen.
4. Mögliche Interaktionen von NSAR mit anderen Medikamenten, z.B. mit Antikoagulantien, Lithium, orale Antidiabetika, Digoxin, Aminoglykosiden, Antihypertensiva, Diuretika, sind bei der Verordnung und im Therapieverlauf zu beachten.
5. COX-2 selektive Substanzen werden seit wenigen Jahren als Coxibe bezeichnet und zeigen im Vergleich zu nicht-selektiven klassischen NSAR signifikant weniger Nebenwirkungen am oberen und unteren Gastro-Intestinal-Trakt.
6. Risikofaktoren für schwerwiegende GI Toxizität sind höheres Lebensalter, höhere NSAR-Dosen, Anamnese von Magenblutungen/Ulzerationen, gleichzeitige Einnahme von Corticosteroiden oder Acetylsalicylsäure, Antikoagulation, und kardiovaskuläre Erkrankungen. Nikotin- und Alkoholabusus sollten ebenfalls berücksichtigt werden. Bei Vorliegen dieser Risikofaktoren sollte eine Primärprophylaxe mit Misoprostol (400 ug/die) oder einem PPI in prophylaktischer Dosierung oder Famotidin (2x40 mg/die) erfolgen. Nur für Misoprostol sind protektive Effekte distal des Duodenums beschrieben.

7. Bei erhöhtem renalen Risiko (Niereninsuffizienz, hohes Alter, Salz- oder Volumenmangel, Leberzirrhose, Herzinsuffizienz) sollten NSAR nur bei absoluter Notwendigkeit, zeitlich begrenzt, in niedrigst notwendiger Dosis verwendet werden; dabei sind prinzipiell NSAR mit einer HWZ unter 4 h zu verwenden.
8. Coxibe sollten bei KHK, Z.n. Myocardinfarkt oder cerebrovaskulären Durchblutungsstörungen wegen des möglicherweise erhöhten Risikos für ischämische Ereignisse nicht eingesetzt werden.
9. Eine vorbestehende niedrig dosierte Acetylsalicylsäure-Therapie (oder andere thrombozytenaggregationshemmende Therapie), aus kardiovaskulärer Indikation, soll unter NSAR weitergeführt werden.
10. Vor Therapiebeginn haben Kontraindikationen (Überempfindlichkeit / Allergie gegenüber Produkt, aktive peptische Ulzeration oder gastrointestinale Blutung, mittelschwere oder schwere Leberfunktionsstörung, fortgeschrittene Nierenerkrankung (Clearance < 30 ml/min), letztes Trimenon der Schwangerschaft und Stillzeit, anamnestisch asthmati sche Anfälle, Urticaria, Rhinitis, welche möglicherweise durch Salicylate / NSAR ausgelöst wurden, entzündliche Darmerkrankung, schwere dekompensierte Herzinsuffizienz ausgschlossen zu werden.
