MyHumana—your gateway to your Humana plan

As a member, you have a secure, online account that is all yours, and all about your Humana plan. It’s called MyHumana.

MyHumana provides you access to all your plan information, including:

• What your plan covers, and your claims status• Doctors and pharmacies in Humana’s network, for possible cost savings• Wellness programs and education resources

MyHumana is simplicity itself—everything about your plan, organized in one always-available, easily accessible place.

Activate your MyHumana account. It’s easy. Visit Humana.com/registration.

A wellness program with rewards: Go365

Our wellness program, Go365 by Humana®, is part of your plan. Your healthy activities earn you rewards, which you can redeem for gift cards in the Go365 Mall. There’s a wide range of qualifying healthy activities, including:

• Exercising—verified workouts with a fitness tracker digitally or by paper

• Having your annual physical exam• Getting your preventive screenings

• Taking health education classes• Social activities, such as volunteering • Even getting your flu shot earns you a reward

Get started with Go365 the same way you did with MyHumana: • Go to Humana.com• Select Go365• Complete your Go365 materials, either online or on paper

with the business reply envelope included• Start earning rewards

Go365 rewards have no cash value and must be earned and redeemed in the same program year. Any rewards not redeemed by December 31 will expire.

Complete your Health Assessment

It’s 9 simple questions about your health. Your answers will help us guide you to tools and resources available in your plan to help you reach your health goals and live the way you want.

To take the Medicare Health Assessment, call our automated voice service at 888-466-5280 (TTY: 711). When you call, you’ll be asked to provide your 8-digit member ID number located on the front of your Humana member ID card. You can also take the survey online in your MyHumana account.

Schedule your Annual Wellness Visit

Your Annual Wellness Visit is not your annual physical exam. It’s not meant to detect any health problems, but rather to help you prevent them.

This is a conversation with your doctor about your life and your health. What’s good? What could be better? Are you trying to avoid a chronic condition, such as diabetes or heart trouble? Are you already managing one and trying to manage it better?

Your Annual Wellness Visit is a valuable benefit, and Medicare typically covers it with no additional cost to you. Call your primary care physician to schedule your appointment.

Step it up with SilverSneakers

You’ve probably heard of the SilverSneakers® fitness program. With participating fitness facilities across the country, SilverSneakers can help you:

• Get more active• Build strength• Lose weight

• Gain confidence• Join in social activities

Not everything calls for a gym or a swimming pool. SilverSneakers also offers online classes and workshops, video on-demand workouts for home use, a fitness app and classes specifically for seniors.

Most Humana Medicare Advantage plans include SilverSneakers at no extra cost to you.
