Instructions
Reference Form
The reference requires a written statement endorsing the financial need of the applicant. In addition, please provide supporting
information which can include examples of their leadership abilities and/or their commitment to community or school. Your candid
assessment of the applicant’s academic promise, character and financial circumstances is essential to help the Awards Adjudication
Committee to decide this year’s recipients. The information you provide will remain confidential and only be used by Enrolment
Services for the purposes of the Centennial Scholars Entrance Awards adjudication. This information will not be released to any parties
outside of Enrolment Services.
The reference form is a deciding factor on the success of the student’s application.
• Use powerful and specific statements to indicate the student’s abilities and character
• Address both the student’s accomplishments and activities but focus on the student’s financial circumstances
Feel comfortable saying “no”
Tell the student if you don’t feel comfortable giving them a reference. A weak reference will not help the student.
Help with the package
Offer to read and critique the student’s Description of Circumstances. Your input is valuable in helping them put together an
effective application.
Complete the form using the downloadable PDF form and enclose a signed single‐sided copy in a sealed envelope to the Student.
