Prof.(Dr.) Jitendra Agrawal Award
The award will be given to an ophthalmologist of Indian Origin. Every year Application/nomination for the award will be called by UP State Ophthal-mological Society from the individual or Organization or Institution and the last date for application will be two months before the UPSOS annual con-ference of that year. It will be preferably awarded to an ophthalmologist working in the field of occuloplasty or pediatric ophthalmology .The Award will consist of a citation or certificate and a cash award of Rs.10000/-(Rupees Ten thousands).The award will be presented to the awardee in the inauguration function of an Annual Conference of every year.

Should not exceed more than 200 words Title should not exceed more than 100 characters. The text should include aim of study, material and meth-ods, results and conclusion. It should not show your identity or place in ti-tle.

Duration of presentation is 6 minutes only. Signed hard copy with CD of the paper will have to be handed over to hall in charge before presentations without which presentation will be disallowed.
POSTER
Size-4*3 feet. Poster should include introduction, material and methods, results, discussion(if any) and conclusion.
VIDEOS(Max 3 Min-Mini 3 Min)
Video should not exceed 3 min duration Send one CD of the video by 30th OCT 2021. Do not send the original video, no video will be returned.
INSTRUCTION COURSES(1 hour, each)
