서론
수직피개란 하악 전치에 대해 상악 전치가 피개하는
정도로, 정상 수직피개량보다 큰 교합관계를 과개교합
이라 한다. 과개교합은 다양한 치성, 골격성 부조화에
기인한다. 치성 과개교합은 전치의 과맹출, 구치의 저맹
출, 또는 2가지 조합의 양상을 보인다. 반면 골격성 과
개교합은 수평 성장 패턴, 하악골의 전상방 회전, 짧은
전안면 고경의 특징을 보인다.1,2
과개교합의 치료로는 구치부의 정출 또는 전치부의
압하가 있으며, 치료계획 수립에 있어서 환자의 잔여 성
장량, 골격 패턴, 인접 연조직에 대한 치아와의 관계,
교합 평면 등에 대해 면밀히 고려해야 한다.3,4
잔여 성
장이 남은 성장기 환자의 경우 상·하악 구치의 정출을
통한 전치의 상대적 압하를 유도하는 것으로 하악지의
수직성장과 균형을 이루는 하악골의 후하방 회전을 통
해 치료할 수 있다. 잔여 성장이 없는 성인의 경우에는
구치의 정출보다는 전치의 압하에 의한 개선이 안정적
이며, 심도의 골격성 부조화를 보이는 경우 악교정 수
술도 고려된다. 최근에는 골격성 고정원을 이용한 치아
의 절대적 압하를 통해 비수술적인 방법으로 개선할 수
있는 과개교합의 범위가 확대되었으며, 특히 과개교합
을 동반한 II급 부정교합 환자의 수직적 조절에 있어서
효과적이다.5
본 증례 보고는 교정용 미니 임플랜트를 이용하여 과
개교합을 동반한 II급 부정교합 환자를 치료한 사례이다.
진단
19세 7개월 여자 환자가 웃을 때 잇몸이 많이 보이고
(gummy smile) 입술이 돌출되었다는 주소로 본과에
내원하였다. 임상 검사 시 환자는 긴장이 없는 상태에
서 입술을 다물지 못했으며(lip incompetency), 약간
과한 상악 전치 노출도(5.0 mm)를 가지고 있었다. 구
내 검사 시 II급 구치부 관계 및 큰 수평피개(6.0 mm)
와 깊은 수직피개(5.5 mm)를 보였으며, 상·하악 치열
궁 공간부족(상악 –4.2 mm, 하악 –2.5 mm)이 있었
다. 특징적으로 하악 좌측 제1대구치 결손과 이로 인한
상악 좌측 제1대구치 정출이 있었으며, 좌·우측 제2대
구치에 양측성 가위교합이 존재하였다(Figure 1).
측면 두부계측방사선사진 분석 시 하악이 후퇴된 골
격성 II급 부정교합(ANB 9.0°) 및 수직적으로 과발산
된 형태를 보였으며, 하악골이 시계 방향 회전되어 있
었다(Bjork sum 397.5°, SN-GoGn 38.1°). 또한 상
악 전치부의 설측 경사(U1-FH 104.4°), 하악 전치부
의 순측 경사(IMPA 100.3°)를 보였으며, 상·하순의 돌
출이 보였다(upper lip to E-line 4.7 mm, lower lip
to E-line 4.9 mm). 파노라마방사선사진에서 매복된
제3대구치가 관찰되었으며, 과두의 형태는 정상이었다
(Figure 2, Table 1). 이러한 문제 목록을 바탕으로 수
직적인 골격 패턴과 과개교합을 동반한 골격성 및 치성
II급 부정교합으로 진단하였다.
치료계획
개인 병원에서 악교정 수술을 위해 의뢰되었지만 환
자가 비수술적 치료를 원하여 이는 치료계획에서 배제
하였다. 과개교합의 수정을 위해서는 구치부의 정출보
다는 전치부의 압하를 계획했으며, II급 교합 관계의 수
정을 위해서는 상악 전체 치열의 후방 이동을 계획하였
다. 즉, 수직적으로 과한 상악 전치 노출도를 고려하여
상악 전치부 미니 임플랜트를 이용한 절대적 압하와 함
께, II급 구치 관계 개선을 위해 상악 구치부에 식립된
미니 임플랜트를 이용한 전체 치열의 후방 이동과 더불
어 압하력을 적용하여 하악골의 반시계 방향 회전을 계
획하였다. 이때 과개교합이 심화될 것을 예상하여 하악
전치부의 압하도 추가적으로 계획하였다.
