AbstractWe report on the case of a 33-year-old male patient, who was brought to theemergency room of our hospital after suffering a high energy trauma due to anautomobile accident. Beside of a scaphoid fracture there were no signs of anyneurological deficits. After several hours without any clinical symptoms the patientdeveloped a dysarthria as the first manifestation of local cerebral ischemia based on atraumatic dissection of the carotid interna artery. Under systemic high-dose heparintherapy a fast and complete remission of all neurological disorders could be achieved. In the course of time it came on to the development of a dissection aneurysm.Temporary anticoagulation with Phenprocomoun was started in the meantime and untilnow no further complications did appear.Beside of presenting this absorbing case this article shall highlight the regime ofdiagnostic and therapy in case of a traumatic dissection of supraaortal arteries to riseto this rare but potential dangerous complication rapidly and sufficiently.
