Themostchallengingaretypicallypoints3and4asthehardwaremaybebrokenand/ortissueplanesdistorted.

Inaddition,thereisfrequentlynobailoutbecausethehardwaremustberemovedinordertorevisethefixationforanon-unionortoaddressinfection.

Althoughconsideredeasyandstraightforwardbypatients,hardwareremovalisalmostalwaysmorechallengingthantheinsertionoftheimplants.1

Specialtyinstrumentscansignificantlyfacilitatehardwareremovalwhileminimizingdamagetothesurroundingbone/soft-tissue.Strykerhasdevelopedacomprehensivesetofinstruments/techniquestoassistthesurgeonwiththesechallengingissues,regardlessoftheindicationortiming.

Additionally,strippedscrews,coldweldedscrewstoplates,strippedIMNailsproximalthreads[usedforremoval]mayrisesurgicaldifficultyandshouldbetreatedtogether.

StrykerExtractionSystemisasetofinstrumentationdevelopedtomeetthesurgeonsneedforimplantextraction.

TheSystemconsistoftwomodulesfortheremovalofalmostanyStrykerTraumaimplant,aswellasmanynon-Strykerplates,nails,andscrews.Ithasevolvedfromourextensiveclinicalexperiencewithtraumadevices,aswellasfromthecollaborationwithexperiencedsurgeonsworldwide.

Module Oneincludesinstrumentationfortheremovalofinternalfixationsystemsandassociatedscrews.

ConicalandSpreadingTipScrewdriversareprovidedinvaryingsizesforthesaferemovaloftorx,hex,andcannulatedscrews.NailandLagScrewremovalisfacilitatedthroughtheavailabilityofdifferentsizedAdaptorsandConicalExtractionRodsthatareeasilyassembled.

Module Twocontainsextractioninstrumentsfortheremovalofstrippedand/orbrokenscrewsandimplants.

StrykerExtractionSystemisasingleinstrumentplatformthatcanbeusedforvariousextractionmethodsaccordingtoeachindividualcase,inaminimallyinvasiveoperatingprocedure,allowingfortheremovalofStrykerTraumaimplants.

Sincebrokenhardwarecanoftenbeidentifiedonpreoperativeradiographs,butmayalsobeencounteredunexpectedlyduringanoperation,Strykerrecommendshavingbothmodulespresentintheoperatingroomtofacilitatesurgicalproceduresandtoreduceoperativetime.

TheCondyleScrewisextractedbyturningtheScrewdriverinacounter-clockwisedirection.

Neveruseawornordamagedscrewdrivertoremovescrews.Reversecuttingflutesarepresentforthisreason.ItisrecommendedthattheSolidScrewdriverbeusedforScrewremoval.TheSolidScrewdriverappliesgreatertorqueandwillreducethepotentialfordamagingthehexagonaltipontheScrewdriver.Strongboneformationaroundtheimplanthasbeenseeninthepediatriccasesusingpartiallythreadedscrews.Thismay

leadtodifficultimplantremovalwithanincreasedriskofscrewheadbreakageorstrippingofscrewhexagonalhead.

Iftheobliquedirectionofthescrew(approximately135°totheshaft)isnotchanged,thenthereverseflutesarenotinanopportunepositiontocutthecortex.Ifthescrewheadisplacedundertractionandtheangleofthescrewisbroughttoaperpendicularpositionrelativetothebone,cuttingthecortexwillprogressandfacilitatescrewremoval.

Be sure to use the Solid Screwdriver in combination with the appropriate sized screwdriver bits or the cannulated screwdriver with the Spreading Screwdriver Bits. Proceed as described above.

Damaged Screw HeadLightlytappingtheConicalExtractorwithaSlottedHammermaybetriedifpurchaseisnotinitiallyobtainedwithmanualpressure.Itisatthesurgeons’discretionifandhowhardtousethehammer.

AssembletheselectedConicalExtractor(male)withtheTeardop-Handleandturnitcounter-clockwisewhileapplyingpressureinlinewiththescrewaxis,extractingthescrewatthesametime.Ifthescrewdoesnotcomecompletelyout,theForecepscanbeusedtocompletetheextraction.

Step 1:Removethescrewheadportioninordertogainaccesstotheremainingpartofthescrewshaft.Thescrewheadcanberemovedwiththeappropriatescrewdriverasdescribedonpage6.

Step 2:UsetheDriftPunchtoextracttheremainingpartofthescrew.

Ifdifficultiesareencounteredwiththeaboveprocess,overdrilltheremainingpartofthescrewwithaCrowndrillbeforeproceedingwiththeDriftPunch.

IncaseofaTornoffScrewHead:

Step 1:OverdrilltheremainingpartofthescrewwithaCrowndrill.UseanotherCrowndrill,onesizelarger,inordertocreatespaceforthefemaleConicalExtractor/DriftPunch.Itmaybenecessarytostartwithlightpressureonthecrowndrillinordertoavoidthedrillwalkingonthecortexsurfacebeforeapplyingadditionalpressuretopenetratethebone.LighttappingwiththeSlottedHammermayberequired.Itremainsatthesurgeons̀ owndiscretionifandhowhardtousetheSlottedHammer.

Alternatively,theoptionalCountersink(6or8mm)canbeusedtocreateapathfortheCrowndrill.

Step 2:RemovetheremainingpartofthescrewwiththeConicalExtractor,(female)ifthescrewisbrokenclosetothefirstcortexorwiththehelpofaDriftPunchifthescrewisbrokenclosetothesecondcortex.

ThisprocedurecanalsobeappliedtobrokenIMlockingscrews.

Toremoveanyplate,firstextractallscrewsbyusingtheappropriatesizeScrewdriverBits.Removetheplatebyusingaregularforceps.

Thedevelopmentoflockingplatetechnologyhasleadto“coldwelding”ofscrewstotheplates.Inthiscase,cuttingtoolsformetalhavetobeusedfortheremovalofthescrews.Inordertoprotectthesofttissuefromexcessiveheatandmetaldebrisaccumulation,irrigationandsuctionshouldbeusedincombinationwithcuttingtools.

CarbideDrillsareusedtodrill-outtheheadsofscrewsthathavebeencold-weldedtoaplate.Theseareavailableinmultiplediametersandpackagedassingle-use,sterileitems.Seeorderinginformationonpage17.

Step 1:UsetheSphericalBurrtochamferthepointofscrewrupture.

Step 2:UseaCenterDrillinordertocreateapathfortheCarbideDrillBits.

Step 3:UseaCarbideDrillBituntilthereisnolongeraphysicalconnectionbetweenthescrewandtheplate.Axialforceisrequiredforefficientdrilling.AligntheaxisoftheDrillBitwiththeaxisofthescrew.

Note:Carbide Drills are brittle. To avoid damage of Carbide Drill Bits, start the drill prior to touching the screw head. Commence drilling with irrigation and suction systems in place. The suction device should be held close to the tip of the Carbide Drill Bit.

Step 4:Removetheplateanduseoneofthepreviouslydescribedtechniquesforbrokenscreworshaft.

Note:Carbide Drills can also be used also for the removal of a stripped Set Screw. Proceed as described above.

WhenremovinganIMnail,itishelpfultoidentifythenailbrandanditsdiameterinordertochoosethecorrectinstrumentation.

Step 1

IM NailsNote:Special care must be taken to check if the nail moves off-center from the entry point when screws are removed. Any attempt to remove a nail that is off-center may result in fractures of the distal condylar region.

TheEndCap,ifused,isremovedusingaScrewdriver.Ifboneingrowthobstructsaccesstothenail,useofeitheraCrowndrill,pick(notsupplied),orcurrette(notsupplied)mayprovideaccess.

Removethedistal/proximalscrewwiththeappropriateScrewdriverBit.Donottakeoutthescrewsofthedriving(proximal)enduntiltheConicalExtractionRodisattachedtothetopofthenailinordertoavoidrotation.

InserttheConicalExtractionRodintothedrivingendofthenail.

LightlyhammertheConicalExtractorinordertofullyengagethecuttingflutes.

ConnecttheStrikePlateandtheUniversalRodeitheraddingtheSlidingorSlottedHammerasshowninthepicture.

RemovenowtheremaininglockingscrewsbeforeusingeitherHammertoremovethenail.

RemovethedistalscrewwiththeappropriateScrewdriverBit.DonottakeouttheproximalscrewsuntilyouhaveattachedtheConicalExtractionRodtothetopofthenailinordertoavoidrotation.

Anincisionismadeovertheproximalendofthenail.TheEndCap,ifused,isremovedusingaScrewdriver.Ifboneingrowthobstructstheaccesstothenail,theuseofeitheraChisel/Pick(notprovided)oraCrowndrillwillfacilitateremoval.

ConnecttheUniversalRodandtheStrikePlatetotheExtractionRod(centerofpage)eitheraddingtheSlidingortheSlottedHammer.

Step 1:InsertthenailExtractionDeviceintotheproximalendofthenail,tighteningitasfirmaspossible.

Step 2:RemovetheproximalscrewsbeforeusingeitherHammertoremovethenail.

Step 3:Afterremovaloftheproximalnailend,exchangetheConicalExtractionRodfortheExtractionHook(asshowntotheleft).InsertExtractionHookintothecannulationofthenailfragment.

Underfluoroscopy,verifythattheHookhaspassedthroughthenailend.MakesurethattheHooktakesaholdattheendofthenailbeforepullingtoremoveit.

Step 4:UsetheSlidingortheSlottedHammertopullouttheremainingpartofthenail.
