The NORMED plating systems offer a variety of possibilitiesfor the user. The wide selection of implants, instruments andaccessories, as well as organizer trays for storage and steri-lization, are clearly arranged and packaged in a streamlinedmodular system.The defined colour coding system allows for easy identifica-tion and storage of implants, drill bits and tools eliminatingany confusion.The systems are available in basic configurations. Additio-nally, kits may be customized to suit the doctor’s individualrequirements.

Rondo-FIX "Four Corner" ist eine ovalförmige, konvex/konkave Titanplatte diespeziell zur Hangelenksteileinsteifung (mediokarpale Arthrodese) entwickeltwurde. Mittels Rondo-Fix Platte und Titan Schrauben erfolgt eine Verankerungund Vereinigung der vier zu fixierenden Knochen (Kopfbein, Mondbein, Ham-merbein und Dreieckbein). Das System besteht aus verschiedenen Rundplatten-durchmessern, sowie den Standard 2,3 mm Schrauben aus dem Normed SystemExtremity.

Rondo-Fix "Four Corner" features oval-shaped, convex/concave titanium plates,especially designed for fusion of the carpal bones (medio-carpal arthrodesis).The titanium fusion plates and self-tapping titanium screws allow a solid an-chorage and union of the four bones (capitate,lunate, hamate, triquetrum).

1) In der Osteosynthesetechnik ist die Zugschraubenosteosynthese ein be-währtes Verfahren. Ihr Einsatz ist jedoch durch den sphärischen Schrauben-kopf begrenzt, der wie ein Keil wirkt.

2) Die Kombination dieser Schraube mit einer bikonkaven Unterlegscheibe ermöglicht eine wesentliche Erweiterung der lndikation von Zugschrauben.

3) Die sphärische Form der Kopfunterseite der Zugschraube erfordert selbst bei senkrecht zur Knochenoberfläche gesetzter Schraube ein sehr kontrol-liertes Anziehen, da sonst die Keilwirkung des Schraubenkopfes ein Zer-sprengen des Knochens bewirken kann.

4) Mit Hilfe der bikonkaven Unterlegscheibe werden die Keilkräfte der sphärischen Schraubenkopfunterseite um 90 Grad umgelenkt; dies führt zurUmwandlung der Keilkräfte in Druckkräfte, die der Knochen weit bessertoleriert.

5) Die herkömmliche Zugschraube kann nur bis maximal 45 Grad abweichend von der Senkrechten zur Kortikalisoberfläche verwendet werden:Um genügend Auflagefläche für den Schraubenkopf zu erhalten, muss tiefgesenkt werden, wodurch die Gefahr des Durchbrechens in den spongiösenRaum besteht. Noch problematischer ist die Schwächung des Dreiecksfrag-mentes; es ist stark ausbruchsgefährdet.

6) Die bikonkave Unterlegscheibe dagegen leitet die Kräfte beim Anziehen der Schraube auf das stabile Hauptfragment und entlastet das bruchgefährdeteDreiecksfragment. Um diese Wirkung zu erreichen, muss die Schraube fernvom Bruchspalt plaziert werden. Ein Durchbrechen in den spongiösen Raumist dadurch ausgeschlossen.

7) Es wurde nachgewiesen, dass Zugschrauben mit bikonkaven Unterleg-scheiben mehr als doppelt so fest angezogen werden können, bis es zueiner Fraktur des Knochens kommt, als Schrauben ohne solche Unterleg-scheiben.

8) Die Vorteile der Zugschraube mit bikonkaver Unterlegscheibe sind offenkundig und vielfältig:Zugschrauben mit bikonkaven Unterlegscheiben sind auf Grund der in keiner Richtung eingeschränkten Einsatzmöglichkeit universell in der Osteo-synthese einsetzbar.

9) Zugschrauben mit bikonkaver Unterlegscheibe können im Idealfall senkrecht zur Frakturfläche ausgerichtet werden. Die Frakturenden werdenohne seitliche Verschiebung frontal zusammengezogen, wodurch eine idealeBruchspaltadaption erreicht wird.

10) Durch die Positionierung der Schraube in Längsrichtung des Knochens und durch den spongiösen Raum hindurch nimmt die Schraube mit bikon-kaver Unterlegscheibe selbst bei seitlicher Belastung die entstehenden Kräfte auf; sie wird dabei nur auf Zug belastet. Dies erklärt auch, warumImplantatbrüche bisher nicht beobachtet wurden.

11) Diese Positionierung bewirkt bei seitlicher Belastung außerdem, dass jeweils eine Frakturseite alternierend auf Druck belastet wird. Dies stellteinen funktionellen Reiz für den Knochen dar und verhindert eine Inakti-vitätsatrophie. Deshalb können solche Zugschrauben auch im Knochenbelassen werden.

1) The technique of lag screw osteosynthesis is an established method whose application however, is limited by the spherical head of the screw which actsas a wedge.

2) The use of biconcave washers with the lag screws has broadened the range of applications of lag screw osteosynthesis.

3) Since the undersurface of the screw head is spherical, it is important that the screw is tightened very carefully, even when positioned perpendicularlyto the bone surface. Otherwise the wedge action of the screw head willcause the bone to crack.

4) With the aid of the biconcave washer the wedge forces of the spherical screw head are redirected by 90 degrees and converted into pressure forces, which the bone is much better able to tolerate.

5) The conventional lag screw can only be positioned at angles between 0 and 45 degrees to the perpendicular to the bone surface. In order to create a

sufficiently large bearing surface for the screw head, the countersinkingmust be performed deeply. Here there is the danger, however, that the con-ventional screw will penetrate into cancellous bone and an even moreserious problem is posed by the weak triangular fragment, which is highlysusceptible to rupture.

6) The biconcave washer guides the forces towards the strong main fragment while the screw is being tightened, thereby alleviating the stress on the fragile triangular fragment. To achieve this effect, the screw must be placedat a considerable distance from the fracture and thus the breakthrough ofthe screw head into cancellous bone is impossible.

7) lt has been demonstrated that lag screws with biconcave washers can be tightened more than twice as much as screws without such washers beforethey cause the bone to crack.

8) The range of advantages of using lag screws with biconcave washers are quite obvious. Since they can be positioned at any angle to the bonesurface they are suitable for all forms of lag screw osteosynthesis.

9) Lag screws with biconcave washers can be positioned perpendicularly to the fracture surface. There is no lateral dislocation when the fracture endsare drawn together - an ideal fracture adaption can be achieved.

10) A bicortical lag screw with biconcave washer, positioned in the longitu-dinal direction of the bone takes up forces, even when there is lateralstress. The screw is only subjected to tension, which also explains why broken Implants have never been observed.

11) As a result of a longitudinally positioned screw, the sides of the fracture are alternately subjected to pressure. This stimulates bone turnover andprevents atrophy as a result of inactivity. Therefore lag screws can remainin the bone.

8) La cantidad de ventajas que aporta la utilización de estos tornillos con aran-delas bicóncavcas son bastante claras. Pueden posicionarse en cualquierángulo de la superficie ósea y por lo tanto se pueden utilizar en todas lasindicaciones de osteosíntesis con tornillos de rosca parcial.

9) Lor tornillos de rosca parcial con arandelas bicóncavas pueden ser colocados perpendicularmente a la superficie de la fractura. No se produce desplaza-miento lateral de los fragmentos ni rotación al reducir la fractura con elapriete del tornillo.

10) Al colocar el tornillo de rosca parcial bicortical con arandela bicóncava en sentido longitudinal al hueso, no es necesario hacer tanta fuerza, inclusocuando hay un estress lateral. El tornillo está sólamente sujeto a tensión, locual también explica porque nunca se han observado implantes rotos.

11) Como resultado de una colocación longitudinal del tornillo, los lados de la fractura están sujetos a una presión alterna. Esto estimula la reconstruccióndel hueso y previene la atrofia que resulta de la inactividad. Por lo tantolos tornillos de rosca parcial, pueden permanecer en el hueso sin necesidadde ser retirados.

5. Entfernung der Kappe und Aufsetzen der Bohrmaschine im Federbereich. Bohren bis zur Frakturlinie, evtl. Reposition. Vorbohren bis zur Gegen-kortikalis. Abnehmen der Bohrmaschine + Lagekontrolle.

5. Remove the end cap, place a wire driver/drill over the assemble k-wire/ cannula and fix the power tool over the spring area. Initially drill up tothe fracture line. Evaluate the position. After evaluation of the correctpath of the K-wire/cannula, drill to opposite cortex. Remove the powertool without affecting the position of the K-wire/cannula.

5. Retirar el casquillo y conectar el motor sobre el montaje de cánula-broca con aguja de Kirschner. Inicialmente brocar hasta la linea de fractura yverificar alineación. Después de haber evaluado el paso correcto de lacánula-broca con aguja, seguir brocando hasta la segunda cortical. Retirar el motor sin cambiar la posicion de la cánula-broca con aguja.

7. Vorbohren des 0.9 mm Führungsdrahtes (geschützt in 2.0 mm Bohrkanüle) bis ins Os Trapezium.

7. Drill the 0.9 mm K-wire (protected in the 2.0 mm drill cannula) into the Trapezium.
