Introduction

The Thoratec corporate, product, and service brands are closely related to the quality of our products. The manner and style in which we communicate portray our attitude toward customers and the image we have of the company, its products, and services. Therefore, the standards established for the corporate identity must be on the same high level as those we claim for our products.

The corporate identity is the main element of the company signature. Standardizing the representation of the corporate identity reinforces the overall

image of Thoratec products, services, and company with each use.

The Style Guide concisely and efficiently defines the Thoratec visual image and maintains consistency and quality of the corporate identity across all materials.

The Style Guide aims at ensuring that the corporate identity and the Thoratec logo communicate company values, competence, and strategic ambition in a common style used consistently throughout the Thoratec organizational structure and in all communication materials.

All communication from the company must represent the company according to these specifications.

As the market, goals, and strategies evolve, so, too, is the Style Guide subject to change from time to time.

To all those concerned with the design of communication materials for Thoratec, we wish you success in the creative implementation of these new standards.

Specific Pantone colors have been chosen to be associated with individual products. This may mean using a fifth color with a normal 4-color process job. In the event that budgets do not allow for an additional color, the Pantone Matching System® (PMS) color can be converted to a process color. To convert the PMS colors to 4-color process, please use the formulas indicated on the right.

Screen color, or RGB, is used to display images in electronic systems (eg, televisions and computers).

Special Notes TLC-II ® and TLC-II ® Plus The TLC-II color is an equal design application of PMS 2597 (PVAD purple) and PMS Metallic 877. PMS Metallic 877 must be specified.

Process color values were determined using Pantone Color Bridge® and more accurately reflect the conversion of Pantone solid colors to process colors.

• Place a footnote stating ownership of the respective mark(s) in the document’s address block on the back cover of a booklet or at the bottom of a single- page document

JPG and GIF files are appropriate for onscreen use (eg, PowerPoint ® presentations, Internet), small in size, and easily emailed. Because of their small size, neither JPG nor GIF files are appropriate for professionally printed materials. GIF files retain transparency, while JPG files do not. JPG files produce better photographs, while GIF files are better for illustrations of block color (eg, logos and clip art).

EPS and TIF files are appropriate for professionally printed materials. These files tend to be larger in size (TIF files are generally larger than EPS), can significantly increase the file size of a PowerPoint ® presentation, and are not easily emailed. Fair Balance

The following fair balance statement must appear on all HeartMate II system consumer literature:

IMPORTANT INFORMATION

This information is provided for educational purposes. Individual experiences, symptoms, situations, and circumstances may vary. Please consult your physician or qualified health provider regarding your condition and appropriate medical treatment.

Indications: The HeartMate II LVAS is intended for use as a bridge to transplantation in cardiac transplant candidates at risk of imminent death from non-reversible left ventricular failure. The HeartMate II LVAS is also indicated for use in patients with New York Heart Association (NYHA) Class IIIB or IV end-stage left ventricular failure who have received optimal medical therapy for at least 45 of the last 60 days, and who are not candidates for cardiac transplantation. The HeartMate II LVAS is intended

for use both inside and outside the hospital.

Contra-Indications: The HeartMate II LVAS is contraindicated in patients who cannot tolerate or are allergic to anticoagulation therapy.

Common Side Effects: Death, bleeding, cardiac arrhythmia, infection, respiratory failure, device malfunction, sepsis,right heart failure, renal failure, stroke, neurologic dysfunction, psychiatric episode, peripheral or device-related thromboembolic event, hemolysis, hepatic dysfunction, and myocardial infarction.

General Rules In general, follow established grammar rules for capitalization, such as capitalizing proper names, titles, and the beginning of sentences.

Product-Specific Rules For Thoratec documentation, additional capitalization rules apply. Follow the guidelines provided.

Registered/Trademarked Names In addition to always capitalizing registered and trademarked names of Thoratec products, capitalize the registered and trademarked names of other companies‘ products. When using the registered/trademarked name of another company, be sure append the ® or ™ symbol as appropriate.

Note: Whenever possible, use generic names for non-Thoratec products. For example, use acetaminophen instead of Tylenol®.
