Senior Strength Demonstration Project: Overview – A DBSA Pilot Program

• Integrate peer support services to increase patient engagement with PCP and care teams’ whole health plan for the patient

• Integrate certified peer support specialist as key member of the care team

• Provide one-on-one peer support, wellness tools, and senior support group meeting to support patient in identifying and making progress towards peer-defined wellness goals

• Improve overall health outcomes of seniors at risk for depression who are being seen by primary care physicians (PCPs)Goal
