(p.3) Primary Donor & Beneficiary

• This is the person who is setting up, filling out the documents, and submitting the trust fund application.

• This is the person with the developmental disability for whom the trust is being established.

money left in the Beneficiary's Individual Trust Account on his or her death not including unexpected State matching contributions or earnings on matching contributions) (this Disposition Plan may not be amended):
