Azmi Yahaya [email protected] Department of Cardiology, National Heart Institute, Kuala Lumpur, Malaysia Clinical History A 77-year-old gentleman presented with change of voice for 1 year and associated with chest discomfort. CT Aortography revealed arch of aorta aneurysm measuring approximately 5.9cm x 6.5cm x 7.0cm, involving the origin of the left common carotid artery. Objective Describing the technique of thoracic endovascular aortic repair (TEVAR) in combination with chimney grafts (brachiocephalic & left common carotid arteries) and plug occlusion of left subclavian artery. 1 Epic 14x60mm Advanta V12 7x59mm Vascular Plug Valiant Thoracic 36/32x150mm (1 st stent graft) 2 Valiant Thoracic 32/32x100mm (2 nd stent graft) 3 Final aortogram 4 Take Home Message TEVAR in combination with chimney graft technique is an efficacious and safe non-surgical option for patients with thoracic aortic aneurysm. In this patient, Thoracic aortic aneurysm was successfully sealed and the great vessels (brachiocephalic and left common carotid arteries) were preserved with no complications. Review 1 month later revealed regression of the aneurysm and recovery of voice.
