Welcome to UnitedHealthcare Community Plan

UnitedHealthcare Community Plan is your TennCare health plan, and we will also be managing your

long term services and supports.

The TennCare long term services and supports program is called CHOICES

UnitedHealthcare has a great deal of experience helping long term services and supports members

just like you. We will help you with your long term service and support needs. The CHOICES program

includes nursing facility care as well as care at home.

A list of CHOICES providers are community-based services, including adult daycare, assisted living

facilities, attendant care, community living supports, community living supports family model, home

delivered meals, in-home and in-patient respite care, minor home modification, nursing home facilities,

personal care and Personal Emergency Response System (PERS).

If you need help finding a doctor, using this directory or would like

to request a hard copy be mailed to you, call Member Services at

1-800-690-1606, TTY 711, 24 hours a day, 7 days a week.

We care about you and your family's health. To find a CHOICES provider, find your county in the table

of contents. Choose the type of service listed in your plan of care.

CHOICES services are based on a member's assessed need. Choose a CHOICES provider that meets

your needs.

Using this Provider Directory

To find a provider, choose the county you want in the table of contents. Choose the type of provider you

want. Remember to take your member ID card with you.

These listings are based on the provider's application. We verify certification when a provider joins

our network. We check every three years (or sooner). We verify names, phone numbers, addresses,

languages, limits and gender when a provider joins our network or gives us new information. A preferred

provider meets certain standards or certification and is noted in this directory.

Some network providers may have been added or removed from our network after this directory

was printed. We do not guarantee that each provider is still accepting new members.

Reporting incorrect information

To report incorrect information, email [email protected].

This email box is for members to report potential inaccuracies for demographic (address, phone, etc.)

data elements identified on the online or paper directories. Reporting issues via this mail box will result

in an outreach to the provider's office to verify all directory demographic data.

Individuals can also report potential inaccuracies via phone. UnitedHealthcare Community Plan

members should call the number on the back of their member ID card. Non-UnitedHealthcare

Community Plan members can call 1-888-638-6613.

The restroom is accessible and the doors are wide enough to accommodate a

which allow easy transfer from wheelchair to toilet. The sink is easy to get to and

the faucets, soap, and toilet paper are easy to reach and use.

The entrance to the exam room is accessible, with a clear path. The doors open

wide enough to accommodate a wheelchair or scooter and are easy to open.

The exam room has enough room for a wheelchair or scooter to turn around.

Exam Table/

Scale

The exam table moves up and down and the scale is accessible with handrails

to assist people with wheelchairs and scooters. The weight scale is able to

accommodate a wheelchair.
