To present the LHIN’s strategic objectives to expand the use of Telemedicine technology in the Central West LHIN

To outline identified areas of focus for OTN, LHIN and local health service providers

To inform organizations ensuring alignment of organizational telemedicine plans to the LHIN’s strategic objectives

To discuss how organizations can enable Telemedicine to improve access to programs and services for the benefit of Central West LHIN residents

What is Telemedicine?

Telemedicine, is the use of information and communication

technologies to support the delivery of clinical care,

professional education and health-related services. MOHLTC invested in the Ontario Telemedicine Network

(OTN) to provide virtual care to Ontario residents who

would have to travel to receive healthcare services.

Telemedicine LHIN Investments

In 2010, eight telemedicine nursing positions were funded through the Central West LHIN Telemedicine Nursing Initiative.

In 2012, eleven telemedicine systems were funded through the Central West LHIN Narcotics Strategy.

In 2013, one Central West LHIN Regional Narcotics Strategy Lead role was created to increase community capacity by:• Coordinating Narcotics Strategy services within the Central West LHIN• Building community partnerships and cross-sector collaborations• Integrating the use of telemedicine into addiction and mental health services• Developing a local addictions strategy in the Central West LHIN

In 2013, one Central West LHIN Regional Telemedicine Executive Administrator role was created to:• Supporting agencies in the integration of telemedicine in clinical operations• Provide administrative and operational support regarding telemedicine

utilization• Support organizations in achieving telemedicine goals

Traditional Room-Based System

Personal Computer Videoconferencing (PCVC)

OTNconnect is an app available on iOS devices

OTNinvite is a new feature allowing consultants to connect with clients from personal devices anywhere they have an Internet connection.

Benefits to Healthcare Providers

Why do healthcare providers use telemedicine?

Efficient method to serve patients/clients from anywhere in Ontario

OHIP billable at a premium rate

Visits count as face-to-face contact

Increases ease of case coordination

Increases clinicians’ efficiency and productivity

Benefits to Clients & Patients

Why do clients use telemedicine?

Increased access to care

Eliminates the need to travel long distances to appointments

Saves money and time for clients and/or family members

Reduces the stress of travelling outside local community

For some individuals, increases confidentiality and comfort level

Telemedicine Statistics

Telemedicine helped save $61.8 million in Northern Health Travel Grants in 2013/14

Saved 418 million kilometers of travel in 2013/14

Patients report saving as much as $242 per telemedicine session (i.e. including mileage, parking, taxi, transit, lost wages, food, childcare, accommodations, etc.)

92% of patients reported being satisfied with telemedicine and 91% of patients indicate they would use it again.

Why did the LHIN develop Telemedicine Strategic Objectives?

1. Align with and support the objectives of Health Links by increasing access and care coordination for complex patients in the Central West LHIN

2. Align with the Patient First Action Plan by expanding technology to provide additional ways for patients to connect with healthcare providers

3. Provide a LHIN strategy for organizations in order to increase the utilization of Telemedicine and improve virtual access to healthcare bringing care closer to home

4. Align the LHIN’s Telemedicine plan to the LHIN’s strategic plan (IHSP) and with that of the Ontario Telemedicine Network (OTN)

5. Improve performance of previously invested Telemedicine studio-based and nursing resources

1) Ensuring currently funded sites are appropriately open to the community

2) Enabling all organizations to utilize local Telemedicine sites through partnerships and collaborations

3) Exploring centralized scheduling to ease administrative burden within organizations

Your organization has the opportunity to bring care closer to your location by connecting with partner agencies that lead programs beneficial for your clients. You also have an opportunity to share your resources and sessions by allowing other agencies to connect to your programs and offerings via OTN.

Telehomecare supports patients with COPD or CHF through health

coaching and remote monitoring.

Telehomecare is a free, six-month program.

Derived from evidence based guidelines, and approved by a

provincial clinical expert committee– Primary care provider refers patient

– Technician installs home monitoring equipment

– Specially-trained registered nurses or certified respirologists provide weekly

health coaching by telephone.

– Patients use home-based technology to measure blood pressure, oxygen and

weight and to answer simple questions about how they are feeling
