• ราวๆ 40-50% ของโรงพยาบาลทั้งหมด จะมีการให้บริการ Telemedicine อย่างใดอย่างหนึ่ง

• โดยในปี 2016 ที่ผ่านมา โรงพยาบาลในเครือ Kaiser Permanente ได้รายงานว่า ปริมาณการให้บริการ Telemedicine มีปริมาณสูงกว่าการพบผู้ป่วยโดยตรงแล้ว

• หลายๆรัฐ เริ่มให้ประกันสุขภาพครอบคลุมบริการ Telemedicine เหมือนประกันสุขภาพทั่วไปแล้ว

• ระบบประกันสุขภาพจากนายจ้าง เริ่มมีการครอบคลุม Telemedicine แล้ว

• ปัญหาการขาดแคลนแพทย์จะเป็นตัวเร่งให้บริการเหล่านีเ้กิดขึ้นมาแทนที่

• Affordable Care Act (ACA) ซึ่งให้สถานพยาบาลลดค่าใช้จ่ายในการรักษามากขึ้น ท าให้มีการน าTelemedicine ที่มีราคาต่ ากว่ามาใช้มากขึ้น

• ความคาดหวังของประชาชน ในการเข้าถึงบริการสุขภาพที่รวดเร็วและตลอดเวลา เป็นอีกปัจจัยที่เร่งการเติบโตของ Telemedicine

• กระจายการรักษาจากผู้ให้บริการ (โรงพยาบาลและคลินิก) ไปเป็นที่บ้านและอุปกรณ์เคลื่อนที่ (mHealth/IoT) ท าให้ลดความแออัดและระยะเวลาในการรอคอยได้

Benefits

• แนวทางการ claim ค่าใช้จ่ายจากประกันสุขภาพยังไม่นิ่ง • ความสัมพันธ์ของแพทย์และผู้ป่วยที่เปลี่ยนไป• การรักษาไม่ต่อเนื่อง และเสี่ยงต่อเกิดการรักษาที่เกินความจ าเป็น เช่น ยาปฏิชีวนะหรือ

ยาแก้ปวด• ยากแก่การติดตามการรักษา และควบคุมคุณภาพของแพทย์• ยังใช้กับกลุ่มโรคที่ต้องอาศัยการตรวจร่างกายมากๆ ไม่ดีนัก เช่น Acute Abdomen• ยังมีหลักฐานทางวิชาการในระดับ RCT สนับสนุนไม่มากนัก• กฎหมายใบอนุญาต และข้อบังคับที่ต่างกันไปในแต่ละรัฐ• Malpractice• การเข้าถึง Internet ความเร็วสูงในบางพื้นที่ยังคงจ ากัด• กลุ่มผู้สูงอายุซ่ึงไม่สามารถเรียนรู้และใช้งานเทคโนโลยีเหล่านี้ได้ดีนัก

Challenges

• จากรายงานของสมาคมประกันภัยเวชกรรม Physician Insurers Association of American (PIAA) ด้วยข้อมูลกว่า 10 ปี พบว่า

– แนวทางยังคงเป็น case-by-case

– แทบไม่มีรายงาน malpractice จาก Telemedicine เลย“Even acknowledging that the number of virtual care claims will likely have increased since 2013 given the greater prevalence of Telemedicine, the PIAA data shows almost no malpractice activity related to Telemedicine. ”Medical Professional Liability Risks of Telemedicine, February 2017

Malpractice

• อนุโลมให้แพทย์ให้บริการข้ามรัฐ ผ่าน Telemedicine ได้ (Interstate Medical Licensure Compact)

• ตัวชี้วัดคุณภาพระบบ Telemedicine อยู่ระหว่างการจัดท าโดย NQF

• ATA อยู่ระหว่างด าเนินการมาตรฐานการเชื่อมต่อระบบ Telemedicine กับ ระบบโปรแกรมเวชระเบียน EHR และระบบช่วยตัดสินใจของแพทย์

• ราชวิทยาลัยต่าง ๆ เริ่มมีการออกแนวทางในการใช้ Telemedicine มากขึ้นโดยเฉพาะ AHA

• ก าลังมีการพิจารณาออกมาตรฐานความปลอดภัยให้มีความชัดเจนมากขึ้น

• JCI ถอนข้อก าหนด Audit Telemedicine ออกไปเนื่องจากซ้ าซ้อนและขัดแย้งกับ ข้อก าหนดของหลายๆ รัฐ

• AMA อยู่ระหว่างการจัดท าข้อปฏิบัติทางจริยธรรมและปรับปรุงร่างแนวทางในการclaim ค่าใช้จ่ายใน Telemedicine
