This will include the redesign of outpatient services, the use of apps in chronic health conditions and improved patient access to Primary Care services. Participants will have the opportunity to learn about developments within HHM and participate in small group discussions related workforce development, implementation support and change management and explore key barriers and challenges to deployment at scale. Local partnerships will also provide an input on their local TEC plans for home fire safety related activities.
