TC-PLUS™ and RT-PLUS™ knee system use the same patellar components
• Patella design The all-poly patellar component is a symmetrical, sombrero design, for optimal tracking throughout range of motion. The reverse side presents a cement pocket and three pegs for anchorage.
• Application The same patella implant can be applied as either “onlay” or “inlay”.
• Patellar groove The femur features an anatomical inclined anterior shield to avoid notching and a 6° patellar groove tracking.

RT-PLUS knee system is part of the seamless TC-PLUS Primary knee system
• Compatible Bone resections (femoral a/p Box) are compatible with the primary prosthesis TC-PLUS Primary knee system.
• Combination The femoral implants are available in five sizes. Each size can be combined with the next larger or smaller size (except for the smallest size 2).
• Stability The rotation peg provides security against luxation and enables a stable distraction up to 20 mm.
• Kinematic The special joint mechanism design enables a physiological roll-back of 9 mm, which improves the flexion capacity of the joint. Furthermore, the rotation peg enables physiological internal and external rotation of 10° from 15° flexion.
• Bone-saving The joint mechanism is contained in a narrow box that keeps an anterior bone bridge and requires minimum bone resection, intended to reduce the risk of femoral condyle fracture.

RT-PLUS™ knee system offer individual intraoperative joint adaptation
• Flexible The modular tibial inserts, with three different heights (8, 11 and 14mm), enable individual intra-operative joint adjustment. The tibial components thickness of 3mm need to be added to the tibial insert height of 8mm resulting in a total tibial height of 11mm, 14mm for a tibial insert of 11mm and 17mm for a tibial insert of 14mm, respectively.
• Stability The special design anchorage of the tibial insert into the tibial component ensures high varus / valgus and hyperextension stability. Combined with the femoral peg, full freedom in rotation and distraction enable minimal joint stresses, without decreasing joint stability.
• Assembling The patented method of the securely fixed tibial insert into the tibial component enables easy and stable coupling and requires only minimal distraction of the knee joint for assembling.
• Modularity As the joint mechanisms are identical, the RT-PLUS Monobloc & RT-PLUS Modular knee system are fully compatible (example; femur RT-PLUS “Monobloc” can be combined with a tibia RT-PLUS Modular).

• Anchorage The femoral anchorage is the same from size 4 to size 10 and use therefore the same reamer to perform the stem anchorage in the IM canal. Only size 2 is smaller and needed to be adapted to the anatomy (same for the femoral PS box rasp).
• Cement Between stems and reamed IM canal, the stem is surrounded by a mantel of ~1 mm bone cement. This allows the component a good stabilization and anchorage.

• Bone compensation The use of 10 mm tibial full augments, give the system the additional option to compensate larger bone defects. In total ~37 mm (10 + 27 mm) joint gap can be fulfilled.
• Fixation The augments are cemented both sides (to the components and the bone). Pay attention during cementing process, that the PE block is always clean of any material (as blood, liquid, fat, bone, etc.).

Re-establishing the joint line
Extensor apparatus needs to be stable in place. If not, a rigid hinge or an arthrodesis needs to be considered.
The figure below shows three anatomical landmarks that can be used during revision surgery to re-establish the joint line. While there are others, these three landmarks can be used as a quick reference to ensure that the new implants reconstruct the joint line at the appropriate level.
The three measurements are:• Epicondyle centre ▷ ~25–30 mm• Inferior patellar pole ▷ ~10–30 mm• Fibula head ▷ ~15–20 mm

Parallel femoral PS box resectionsWith cemented tapered femoral stem, the PS box preparation can be performed flush to the box (parallel resections)
During inserting the femoral component, as the component is not firmly driven by the stem (tapered with cement mantel), the femoral component is placed carefully entering lateral in the femoral PS box! Be aware
• by using cemented conical stem, the PS box can be performed parallel, as the femoral component is not firmly driven by the stem.• by using non-cemented cylindrical stem, especially with long stem, the PS box has to be performed tapered, parallel to stem axis (6° valgus), to exclude medial
condyle impingement with the medial edge of the PS box and eventually risk of femoral medial fracture during femoral reposition.

Femoral and tibial IM canal openingFemoral opening for primary or revision procedure
To avoid notching, for the femur canal opening, we recommend usingthe adjustable IM femoral drill guide. The anterior gauge, which isinserted under the quadriceps, lies flat against the anterior femoralcortical bone in the direction of the femoral anatomical axis. The distalstop of the gauge should lie flush on the condyles. The distal resectionneeds to point in the direction of the femoral mechanical axis andneeds to be positioned medial to “Whiteside’s line” (3 to 4 mm).
Tibial opening for primary or revision procedure
To enable ideal tibial opening, the tibial sizer can be used and placed onto the proximal bone resection for revision procedure. For primary procedures, the intercondylar eminence has to be resected first, in order to enable stable positioning of the tibial sizer. Free hand, the drill is positioned centrally M/L and A/P ⅓ of the distance from anterior.
