Summer School SUMMER 2018 July 7, 2018 Dear Parents, We would like to welcome you to the 2018-19 school year! As we enter our 16 th year, it is worth reflecting on our school’s accomplishments from this past year. Here’s a sample of some of those highlights: • We had four students receive perfect ACT scores. We also had record high scores in both the ACT and the SAT. • For the first time in a dozen years, we held varsity football games at home creating fun Friday Night Lights. And just as exciting, varsity football games will be held again on campus this fall! • Girls soccer won a CIF championship for the second year in a row, with junior Isabella D’Aquila winning OC, state, and national player of the year recognition. • Boys tennis won its first ever Trinity League Championship. Girls cross-country won their 8 th straight. • Sophomore Anthony Grover was the league champion in cross-country. He was also the OC Championships winner and was selected as the OC Register Cross-Country runner of the year. • Senior Sherry Guo won a silver medal at the prestigious national Scholastic Art Competition. • Our Math team won its division at the UCSD Math Competition. • 16 members of our Business Magnet Program qualified to compete at the state Future Business Leaders of America competition. • In a school first, our spring musical, Les Misérables, drew on the collaboration of all our performing arts programs – instrumental music, dance, choir, and theatre, with several participants receiving recognition at the prestigious MACY Awards held at the Segerstrom Center. Please take a moment to carefully read the documents enclosed in this mailing. Additionally, please make sure to visit our website this summer as we regularly update it with pertinent information for the upcoming year. Personal Counseling
CARING FOR OUR STUDENTS
JSerra offers an exceptional student environment for children by providing an outstanding academic program. However, growing up is not always an easy process and various problems can interfere with a student’s pursuit of success. In order to facilitate the development of the whole person, JSerra offers assistance to students through the Personal Counseling Program so that students can reach their full potential. The services offered within the Personal Counseling Program help lay the foundation for future growth and achievement.
COUNSELING SERVICES
The goal of the JSerra Personal Counseling Department is to provide a safe environment where students can develop as a whole person spiritually, emotionally, intellectually, and physically. We seek to be of constant support to our students, families, teachers, and the administration of our school community. The JSerra Personal Counseling Program provides comprehensive individual and family services to help students meet and overcame challenges, and gain the necessary skills to reach emotional and social maturity. We are able to offer students additional support designed specifically to teach life long learning and coping skills to assist in increasing learning readiness. Our program is staffed with trained and experienced professionals. Cara Morales, MA, LMFT #84336 and Dr. Courtney Harkins, PsyD, MFC #43611 are on campus to provide personal counseling to our students in necessary situation. Based on individual need, our counseling staff can provide:
• Individual counseling• Family conferences• Issue-specific group programs• Referral to outside agencies
OUR PROGRAM
This program will be offered to students who are referred through the guidance counselors, teachers, and in crisis situations. Parental permission is sought, and confidentially observed. The counselor meets with the student to complete a personal interview, make an evaluation, and then offer recommendations.
Services are available daily before, during, and after school hours.
Personal Counseling
STATEMENT OF CONSENT FOR TREATMENT
A clear framework for JSerra’s Personal Counselor and the student can help each party avoid misunderstandings and greatly facilitate a successful working relationship between both groups. The following are policies under which therapists at JSerra Catholic High School operate their services. By reading the sections below carefully and completely, JSerra Catholic High School assumes from your participation in this service that you have a full understanding of the following sections. Please feel free to discuss any of these with your therapist at any time.
PERSONAL COUNSELING/PSYCHOTHERAPY SERVICES
Psychotherapy is a treatment that addresses psychological distress and problems in life. We believe that psychotherapy practice is a collaborative process. The intent is to build a relationship with the student (“him”) in which he feels free to explore his thoughts, feelings, and behaviors — particularly those that are getting in the way of achieving his life goals or ability to find fulfillment and meaning in his life. Since psychotherapy involves the exploration and processing of uncomfortable aspects of life, at times he may experience difficult feelings like sadness, anger, guilt, loneliness, and helplessness. These emotional states can sometimes last for days or even weeks. In such cases, it is usually recommended that one seeks a medical consultation with a psychiatrist or other medical doctor to aid in the process of psychotherapy. On the plus side, psychotherapy has been shown to have clear and important benefits to those who follow through with it. It usually leads to improved relationships, healthy solutions to specific problems, and significant increases in mental clarity and emotional balance.
APPOINTMENTS
Aside from the initial intake session which usually lasts from 60 to 90 minutes, an individual psychotherapy session lasts anywhere between 20 minutes to 40 minutes (unless a longer or shorter session was agreed upon scheduling). Family conferences are available as well, which can last between 60 and 90 minutes or longer if previous arrangements have been made. Sessions can be weekly, bi-monthly, monthly, or as needed. More frequent meetings may be arranged to facilitate a current crisis situation. If there is more intensive work on deeper psychological issues, a referral to an outside therapist or psychiatrist will be given and the therapists at JSerra will work in collaboration with the outside therapist in a supportive role — not as the primary therapist.
PSYCHOTHERAPIST PATIENT CONFIDENTIALITY
By California State Law, our conversations are protected by psychotherapist patient privilege. There are some exceptions to complete confidentiality; however, the content of the sessions will remain confidential unless: a written release of confidentiality is signed by the parent/guardian authorizing the therapist to speak to a specific person or group, a disclosure of court is ordered, or a disclosure is mandated by law.
Personal Counseling
CONTACTING
All JSerra Catholic High School therapists have voicemail in which a confidential message may be left. No one other than the clinician has access to this voicemail. Therapists make every effort to return the call by the end of the business day if they are working that day. JSerra Catholic High School therapists cannot guarantee to respond to you immediately. In case of psychological emergency, particularly one that is life threatening, please go to your local emergency room and ask for the psychologist or psychiatrist on call. You can also call 911 or the suicide prevention hotline at (877) 7-CRISIS.
PROFESSIONAL RECORDS
This notice describes how the student’s medical information may be used and disclosed (shared) and how a parent/guardian may obtain access to that information. By California State Law, HIPAA, and the standards of practice in the profession, JSerra Catholic High School therapists are required to keep appropriate records of all treatment and services rendered, which is confidential and closely safeguarded. JSerra Catholic High School is committed to protecting all medical information, which is personal and private. JSerra Catholic High School therapists are required by law to ensure that medical information about the patient is kept private.
PATIENT’S RIGHTS
The student or the parent/guardian has the right to end therapy at any time, for any reason, or without notice. Psychotherapy never includes a sexual relationship nor does it include sexual contact of any kind. Sexual contact between a psychotherapist and his/her client is both illegal and unethical and should be reported immediately. To make a report to the regulatory board of California, please use the following information:
Board of Behavioral Sciences1625 N Market Blvd., Suite S-200Sacramento, CA 95834
Because of the nature of the therapist-client relationship, JSerra Catholic High School therapists can have no social contact with the family while professional services are being provided, nor can they have any dual relationships with the family. Bartering (exchange of services) and gift-giving/accepting is strictly forbidden. In order to protect confidentiality, if a therapist should see the parents or student in public, he or she will not acknowledge either party unless he/she is acknowledged first.
The student and parent/guardian has the right to question any aspect of the treatment and to expect, if requested, that the JSerra Catholic High School therapist will provide a referral to another qualified therapist for adjunctive or alternative treatment. If the presenting problems and/or symptoms fall outside of the areas of treatment expertise of the therapist you are seeing, you can expect that the therapist will refer you to an appropriate professional for the treatment you need.
Please feel free to talk to with your JSerra Catholic High School therapist about any of the above policies if you have any questions or concerns.
Health Information
The Nurse’s Office at JSerra Catholic High School welcomes you and your student to the new 2018-2019 school year. We provide the students and school staff with basic health care, first aid, and health counseling to promote health while emphasizing illness prevention. The Nurse’s Office is staffed with a full-time RN available during regular school hours and located in Building #1.
Depending on your child’s medical needs, the following information is important to provide the Nurses Office.
MEDICATION(S): • That require a prescription for Asthma Inhaler, EpiPen, ADD/ADHD medications or antibiotics being administered during school hours or on a retreat or field trip must have a Parent/ Physician Request for Medication Form on file with the Nurses Office for this school year. This completed form should be brought in, faxed or emailed to the JSerra Nurse. All medication must be in its original container with the child’s name clearly marked.

Please present the nurse with your student’s physician orders and diabetic supplies at the time of your meeting.
IMMUNIZATIONS: REQUIRED FOR SCHOOL ADMISSION• For all new incoming or transferring students. California State law requires that all students must be fully immunized and provide immunization paperwork before attending any school. Without immunizations the student is not allowed admission. However, if your student has any vaccines that require a catch-up schedule you will need to submit a letter from his/her physician regarding the name and the date the vaccine is to be given before the start of school. • If your student is not immunized a letter from his/her physician must state the medical exemption. Handbook Form
July 7, 2018
Dear Parents and Students:
Please read the 2018-2019 edition of the Parent Student Handbook, which can be found on the JSerra website under the Parent Student tab in the Den. There are a significant number of important policy changes that have been made. After you have reviewed the handbook, please print the student’s name and grade level, and sign the statement below.
This signature page is due at Orientation on August 6th for Freshmen and August 7th for Sophomores, Juniors, and Seniors. If this page is not received by Orientation, your child will not receive his/her final schedule.
We have read the entire contents of the 2018-2019 Parent Student Handbook and agree to abide by the rules, regulations, and policies of JSerra Catholic High School.
PLEASE COMPLETE BOTH SIDES OF THIS FORM AND RETURN IT TO THE FRONT OFFICE BY SEPTEMBER 10, 2018.
Completed forms may also be submitted via email to [email protected]
As parent or legal guardian of _______________________________________________ (Print Student Name)
I authorize qualified personnel of JSerra Catholic High School to conduct personal counseling with the above student. The form authorizes the evaluation and treatment of the above student. As parent or legal guardian, I have the right to request information concerning the above minor’s evaluation and treatment.
If the student participates in treatment, please understand the importance of allowing him/her to develop a confidential relationship with his/her therapist. As such, understand that most personal information the student discusses with his/her therapist will not ordinarily be shared with you. Rather, the student’s therapist will provide you with general summaries of their progress without private details. However, understand that JSerra Catholic High School is committed to informing the parent/guardian about unusual or dangerous symptoms or behaviors (such as violence, child abuse, self abuse, suicidality, or intentions to harm others, harm oneself, drive while intoxicated, etc.). There are four conditions in which disclosure of privileged conversations is mandated by law: 1) any suspicion of child or elder abuse, 2) there is a credible threat to the physical well-being of others, 3) the client is likely to take his or her own life, and 4) if your JSerra Catholic High School therapist is court-ordered to give testimony about your treatment. We will make every effort to resist any subpoenas issued based on psychotherapist-patient confidentiality. Most courts uphold psychotherapist-patient confidentiality, especially in civil matters, and do not compel clinicians to testify. It may occasionally be necessary to consult other professionals about the treatment. During a professional consultation, the therapist will not give any identifying information about the client. The consultant will also be legally (and most often ethically) bound to keep all information confidential. By the standards of practice in psychotherapy, such a consultation is not a violation of the confidentiality. Your JSerra Catholic High School therapist may frequently contact clients by cellular phone and email; however these technologies are not guaranteed of privacy.
Please circle whether you authorize contact by cellular phone and email:

STATEMENT OF CONSENT FOR TREATMENT
I have read, understand, and agree to be bound by the content, terms, and conditions of this informed consent contract.
I understand that it is my responsibility to register with the NCAA Eligibility Center (www.eligibilitycenter.org) by the end of my junior year. I understand that it is my responsibility to request SAT/ACT scores be sent directly from the testing agency to the NCAA Eligibility Center, code 9999. I am responsible for requesting official transcripts to be sent from every high school I attend to the NCAA Eligibility Center.
CORE COURSE GPA (CoreCourseGPA.com):
JSerra is proud to make CoreCourseGPA web-based software available free of charge to all JSerra student athletes and their parent/guardians to assist in tracking progress toward meeting NCAA eligibility center requirements. This innovative software tracks core course requirements and incorporates the JSerra NCAA recognized core courses into their online course entry form. Student’s core course information is saved for the duration of their high school career. It is the student and parent/guardian’s responsibility to check on the NCAA requirements to ensure the student is eligible. Students are also responsible to complete all NCAA Eligibility Center requirements.
UNOFFICIAL TRANSCRIPT AUTHORIZATION:
Coaches and colleges may contact JSerra for unofficial transcripts of prospective student-athletes. I authorize JSerra Catholic High School to release unofficial transcripts to colleges and college athletic recruiting agents. I understand that recruiters will have access to all information on the transcript including: name, address, telephone number and academic history. My signature also authorizes the coaching and administrative staff of JSerra Catholic High School to discuss, e-mail, fax or mail unofficial grade reports for the purpose of athletic eligibility, scholarships or admissions. The JSerra transcript does not include standardized test scores (i.e. PSAT, SAT, ACT). This release shall remain in effect until the student graduates.
I certify that I have read and agree to the statements outlined above.
