Summer

As well as our daily playschemes for childrenaged from 5 to 11, we’re running play activityweeks for 5 to 13 year olds.

We also have lots of summer holiday clubs,activities, sports courses and more in ourleisure centres and Beacon Park.

So, whatever the activity, we are bound tohave something to make sure your childrenhave a summer of fun!

Playschemes and Play Activity WeeksOur play development team offers a brilliant line-up of holiday activities for children and youngpeople across Lichfield District.

The sessions are great value for families. We also offera number of concessionary places - just look for ‘concession’ in the cost columns.

Concessions10% of our playscheme and play activity places areavailable at a concession of £5.50 a day or £22.50for the week. These places are limited, so make sureyou book early to avoid disappointment. The concessionary rate is available for Lichfield District residents who are: .on income support or job seekers allowance.Play on Prescription members (PoP) .booking for a third or subsequent child in the familyFree placesWe have some free places available for children withdisabilities that need additional support. Useful information for playschemes and play activity weeks

Play leadersWe employ and train our play leaders. We also carry out disclosure barring service checks on all our employees whowork with children. There will be someone trained in first aidat each site.

When attending a playscheme or play activityweek, your child will need to bring a:.packed lunch and a plastic bottle of water. T-shirt for messy activities. sun hat and a long-sleeved top for sun protection. sun cream labelled with child’s name. appropriate footwear and clothing - casual clothes as playing can get messy and active! . prescribed medication if required - you will need to complete a daily permission slip, available at the venue

Can my child take any personal items?We cannot take responsibility for any personal belongings,such as iPods, electronic games and mobile phones, and askthat children do not bring them.

What happens if we arrive late? If your child is going to arrive at a playscheme session after10.15am, please tell the play leader the day before, orcontact the play development team before 10.15am on theday by calling 01543 308786.

Are playscheme places available on the day?You are welcome to come along on the day to see if thereare any places available. If the playscheme is fully booked,but a child has not arrived by 10.15am, you can take theirplace.

Can my child book with a friend?If you put the application forms in the same envelope, we willdo our best to place them on the same playscheme.

How can I book at the concessionary rate?10% of our playscheme places can be booked at aconcessionary rate. However, places are limited so makesure you book early to avoid disappointment.

The concessionary rate is available for Lichfield Districtresidents who are: . on income support or job seekers allowance. Play on Prescription members (PoP). booking for a third or subsequent child in the familyCancellation policy for summer clubs When booking a place on any of our playschemes or playactivity weeks, it is your responsibility to make sure that theyare suitable for your child’s needs.

If you cancel your child’s place on or before 11 July 2013,we will refund your money by cheque, less a £10administration fee.

You can transfer your child’s place up to 11 July 2013, butonly if the alternative place is available.

We cannot offer a refund if you cancel after 12 July 2013.This is because Lichfield District Council subsidisesplayschemes and play activity weeks/days, and it is unlikelythat the place can be re-sold after this date.

How to book playschemes or play activity weeks. Booking starts on Monday 20 May 2013..Complete a registration form for each child. You can either photocopy the form on page 10 or download one from www.lichfielddc.gov.uk/summerfun. Include a stamped, self-addressed envelope with your booking form.. If applying for a concessionary place, please include proof that you receive income support or job seekers allowance (a recent letter will do). If you are a member of Play on Prescription, please write your PoP card number on the bottom of the booking form.

Only fill in the form on the opposite page if you are booking a playscheme or play activity week (listedon pages 2 and 3). All the other summer activities must be booked through our leisure centres or parks.

PaymentYou can pay by cheque or postal order. You can also pay by credit or debit card over the phone.

If you are paying by cheque or postal order, please make it payable to Lichfield District Council and send it to usalong with your booking form. If it’s a cheque, also write your name and address on the back of it.

You can pay over the phone by calling 01543 308786 from Monday to Friday, between 9.30am and 1pm. Afteryour child’s place has been confirmed over the phone, please fill in the booking form and add the booking number inthe payment section and send it to us.
