Successful Management of Acute Dental Pain
This evidence-based course is designed to provide effective and practical strategies for managing acute dental pain emergencies. The latest information on NSAIDS, acetaminophen-containing analgesics and local anesthetics will be provided with the objective of having immediate application to your next patient emergency. Want to know how to anesthetize that hot tooth? How to predictably manage severe acute pain after surgical or endodontic treatments? How to combine common medications to improve analgesia? This course will answer these practical tips and more using a lecture style that emphasizes interactions with the audience in answering common pain problems with useful solutions.
Objectives:
At the completion of this course, the practitioner should be able to:
1. Describe a fast and efficient routine for managing dental pain emergency patients
2. Select the best combination of analgesics to manage dental pain
3. Provide effective local anesthesia to the classic “hot” molar case
Beginning this month, the Texas State Board of Dental
Examiners will begin a lengthy and often arduous process knows as a Sunset Review. The Texas Sunset Act, passed in 1977, allows for the
Sunset Advisory Commission to review the purpose and effectiveness of about 130 different state agencies. Anything from the Texas Commission on the Arts to the Texas Low-Level Radioactive Waste Disposal Compact Commission (which meets next month in Montpelier, Vermont…you can’t make this stuff up). The purpose of the Sunset Review process is to keep the government accountable to the constituents and prevent any agency from becoming a useless budget drain. In its most basic sense, the Sunset Commission simply asks, “Is this
agency needed any longer?” Any state agency subject to Sunset Review can be automatically abolished unless a new legislative bill is passed that “renews” the agency. If the Commission feels the agency is still needed, then they will introduce a new bill, often with recommended changes to the operating policies of the agency, which must pass through the Legislature in order for the agency to continue to exist. If an agency is abolished, either by recommendation from the Commission or failure of a renewal bill, then that agency has one year to conclude its business.
The Sunset Commission consists of twelve people: five Senators, five Representatives, and two public members, one appointed by each the Speaker of the House and the Lieutenant Governor. The Commission also maintains a Director and staff. Beginning this September, the process begins with agencies submitting self-evaluation reports to the Sunset staff. The staff then spends the next year working closely with agency representatives and conducting their own evaluations. Public hearings will be held and the staff will prepare
its final report to submit to the Commission. Final recommendations and Sunset Bills will not be introduced until the next Legislative session, in 2017.
For the TSBDE, the last Sunset Review was finalized in 2003. As you know, because you have been paying them to renew your dental license, they passed and were renewed for 12 more years. Changes that were made to the Board policy and operations then included a reduction in the size of the Board, reduction in experience required for licensure by credentials, and a strengthened enforcement process. Interestingly enough, as recent as last year, the Board was still making policy changes to unburden the violation enforcement process. The Board was established in 1911, and it is unlikely that it will be abolished in 2017, but there could be some major changes made that could affect us. Any TSBDE Sunset Bill for the 85th session will certainly be on our radar, so we will keep you informed of any changes that would affect your practice.

Ken M. Hargreaves, DDS, PhD Ken Hargreaves received his DDS from Georgetown University, his PhD in physiology from the Uniformed Services University of the Health Sciences in Bethesda, MD, and his certificate in Endodontics from the University of Minnesota. Ken spent 5 years at the Pain Clinic of the NIDCR and 7 years as an associate professor of Endodontics and Pharmacology at the University of Minnesota. He joined the University of Texas Health Science Center at San Antonio in 1997, as professor and Chair of the Department of Endodontics and is cross-appointed as professor in the Departments of Pharmacology, Physiology and Surgery. He maintains a private practice limited to endodontics and is a Diplomate of the American Board of Endodontists. Ken has received an NIH MERIT Award for research, two IADR Distinguished Scientist Awards and the Louis I. Grossman Award from the AAE. He has published more than 160 articles and, with Harold Goodis and Frank Tay, co-edited the 2nd edition Seltzer and Bender’s Dental Pulp, and, with Steve Cohen and Lou Berman, co-edited the 10th edition of Cohen’s Pathways of the Pulp. Ken also serves as editor of the Journal of Endodontics.

Summer has come and gone, sort of. It is still bordering
hundred degree days and I’m still sweating as I walk from my car into the office but that carefree feeling of summer has been surely stuffed back into the classrooms along with all the kids in Austin. I came across this little
story about education and it reminded me that we need to keep learning, keep growing and to keep moving.
A college class was graduating on a hot and humid day.
As the graduates walked across the platform and received their diplomas from the president of the college, he smiled, shook their hands, and said loudly, “Congratulations!” Then, in a much lower voice, one that was firm and could be heard only by the graduates, he would say, “Keep moving.”
He was only trying to keep the line moving across the stage, but his words were good advice for a lifetime: “Keep moving.” After every
achievement growing up, graduation, marriage, job promotion, even retirement the best advice is, “Keep moving. Don’t stop. Don’t stagnate.”
There is more to life than you have found thus far. This is not the end; it is only the beginning. “Keep moving.”
When we approach life with this attitude, we look forward to the future and what God has in store for us.

School will be back in session soon. Kids go back to school and traffic here in Austin will of course get worse. For some reason I miss those days of learning, cramming my brain with interesting, useful and useless information. But I’m a dork and I LOVED school. Most of you liked school or at least did well in school or you wouldn’t have become a dentist.
Did we stop learning at the moment we crossed the stage at graduation? Absolutely not. We have the opportunity to continue learning every day about

dentistry whether it be about new innovative techniques, upcoming technology, advancements in materials, or how to approach a complex patient. The avenues are endless. Reading JADA or other publications, attending lectures, going to conferences, viewing webinars, finding a mentor, participating in local organized study clubs, even getting together with a group of your peers reviewing cases and discussing is an incredible way to learn and grow.
We have twelve mandatory continuing education (CE) hours required by the Texas State Board but there are many more hours of learning that we do whether or not it is certified CE. Things like practice management philosophies, how to use new computer software, what’s the most efficient way to extract a tooth are things we don’t necessarily learn in class. Sometimes we can best treat our patients and run our practices from asking questions of other dentists, specialists and representatives in the companies that we utilize.
At CADS we are deeply committed to serving our members. In this coming year we are going to start a mentorship program, have a list of local study clubs posted on our website, start a HIPPA compliant discussion forum and pilot area specific CADS study
groups. We are very excited about these projects and if you would like to become involved please contact me at [email protected] or call at 512-940-8267. Please visit our new and improved website often to see when these will become available.
Education is important because it keeps us thinking about better ways to do things. At CADS we care about educating our members but also finding better ways to serve our members. As Walt Disney said, “Keep moving forward.”

Regulatory Updates
A reminder that the Opt-In/Opt-Out dates for Medicare have changed. The Centers for Medicare and Medicaid
Services have announced an extension for the deadline to either enroll or opt-out of Medicare for providers prescribing Medicare Part-D drugs. The new deadline is June 1, 2016, but the CMS encourages you to take action by January 1, 2016 to allow time for paperwork to clear.
The Texas State Board of Dental Examiners recently held Stakeholder Meetings to discuss possible changes to current rules pertaining to the advertising of a “specialty.” Check the CADS website (www.capitalareadental.org) for the most up to date information on this topic.
The Austin City Council Public Utilities Committee met recently to decide the fate of a proposal to remove fluoride from the community’s water supply. A full report will be made available on the CADS website and in the October issue of The Capital Star.
The EPA’s final rule on minimizing environmental mercury in the dental office is due next summer. The rule will have an effective date of three years after it is finalized (summer 2019). Under the proposed rule, dental offices (some specialty practices not included) will be required to install amalgam separators if their waste water is discharged to public treatment facilities.

Dr. X, who is a couple of years away from retirement, wanted to discuss the value of her
practice and potential transition strategies. She had heard from various “experts” that dental practices are worth approximately 70% of their previous year’s gross revenue. While this “rule of thumb” sometimes holds true, there are many other factors that influence market value, with the most significant factor being profitability. When we brought up the tremendous impact that overhead and cash flow have on practice value, Dr. X’s positive demeanor began to change into one of concern.
Dr. X went on to explain that, while her practice’s gross revenue had been steadily increasing, her net income had not followed the same trend. After examining her practice financials for the past few years, it quickly became apparent that several overhead expenses were much higher than industry standards. This was particularly obvious in her lab expenses, which were running at 14% of gross income as compared to the industry norm of 8-10%, and dental supply costs, which stood at 10% of gross revenue as opposed to the normal range of 5-6%.
Dr. X related the inflated lab expenses to the fact that she did a large amount of crown and bridge, but our investigation revealed that the true culprit was a high lab cost per unit. Dr. X went on to reveal that her lab technician is a close friend and she had not shopped around in years. By using a more competitive price per unit (without sacrificing quality), we calculated that Dr. X could easily reduce her lab expenses to approximately 8% of gross income, a savings of over $24,000 per year.
We also found that the assistant who ordered dental supplies for the office had developed a close relationship with one of the supply representatives and was not competitively shopping for the best buys. After reassigning this task to another person, the supply expense immediately dropped to a more acceptable level of 6% of gross revenue, reducing overhead by an additional $16,000 per year.
From this experience, Dr. X learned that her profit and loss statement can serve as a valuable tool to compare her expense levels to industry norms, identify problem areas, and make changes that will lead to increased net income and practice value. She also realized that while gross income certainly influences market value, the net income generated from those
revenues has a much greater impact and plays a far greater role in gauging the health of an office. Through making a couple of minor changes, Dr. X increased the profitability of her office by 10% or $40,000, which made a huge difference in her quality of life as well as the value and marketability of her practice. That marketability can be achieved and applied to all dental practices, thus increasing their value.
We understand that dentists are busy people and don’t typically enjoy spending a great deal of time analyzing numbers or pouring over income/expense statements. Therefore, we suggest that doctors simply become familiar with the normal range for major overhead expenses (dental supplies, lab, staff, etc.) so that they can quickly notice if something is out of line and move to correct the problem, thus making their time and their practice more valuable.
