performed. Based on that it should be attractive to the facility to help recruit you and give you income guarantees to get you started.

Stockton is in the California Central Valley, just 45 minutes south of Sacramento and 90 minutes from beautiful, vibrant San Francisco.

It is the home of University of the Pacific - a very select small private university with a great reputation.

It hosts the annual Asparagus Festival and is the location of the Haggin Museum. It is located at the head of the Delta - over 1200 miles of navigable waterways and a boater’s paradise. The Port of Stockton boasts ocean going

cargo ships, and it is possible to travel to San Francisco and Sacramento by boat.

Stockton has the best water for competition and recreational rowing in California.

Stockton has its own ballpark, and an arena for sporting events and celebrity performances.

Although we never get snow here we are just 3 hours from beautiful Lake Tahoe and some of the best snow skiing in the world with many resorts close by. The gold country is very close

and Yosemite National Park is relatively close.

Although Napa Valley is not far away, the best Zinfandel in the world can now be found in Lodi, bordering Stockton to the north, where vineyards and wineries abound. Lodi hosts the annual grape festival. There are plans to boost tourism in Lodi to rival that of Napa Valley.

There is talk of a future theme park of the size of Disneyland close to Tracy and a new Amazon Fulfillment Center is being built near Tracy.

Living in Stockton/Lodi

I trained in the UK and did my plastic surgery residency at Emory University in Atlanta. I was on the faculty of UC Davis for two years before establishing my practice in Stockton in 1979. I have been in solo practice ever since.

Over the years as the practice grew certain procedures were dropped. Early on it was hand surgery, then cleft lip and palate, then burns and then facial fractures and in 1995 all insurance was dropped.

Even as the practice became 100% cosmetic rhinoplasty was selectively dropped. The practice now concentrates on aesthetic surgery of the breast, and trunk.

Overhead has been minimized and the office staff have been

extensively trained in handling the cosmetic surgery patient. Extensive forms, procedures, statistics and software are available for the new plastic surgeon.

The practice has developed a thriving injectable clientele and I devote one week a month to injectables.

We use TouchMD, and have a vibrant marketing program, but do not advertise. Significant income is also derived from Ultherapy, and Verju Laser, and we are considering adding Cellfina.

We have an aesthetician and we carry Obagi, Teoxane, skIN3 products.

It is time to think retirement and all possibilities are on the table.

History:The practice was started on January 1, 1979 by Dr. Silverton. It has been a solo practice ever since. It started as 90% reconstructive, 10% cosmetic. As the practice evolved hand surgery was soon dropped. Dr. Silverton remained on a cleft lip and palate panel for a while, but soon dropped it because of lack of volume. Burns were discontinued after a few years. In the early 1990s Dr. Silverton dropped repair of facial fractures, and eventually dropped the reconstruction of major trauma.

Dr. Silverton severed all relationships with insurance companies in 1995. Breast reconstruction was not available after that and the numbers of breast reductions was reduced. During the cosmetic phase as the practice grew further, Dr. Silverton stopped doing rhinoplasty and severely reduced all cosmetic surgery of the head and neck.

With the introduction of liposuction abdominoplasty to the practice in 2009, this procedure has increased in number. Breast augmentation and mastopexy and liposuction has also remained very strong, and this is now more than 95% of all of the surgical procedures.

Although Dr. Silverton injected collagen in the 1980s and 1990s and did minor peels, the non-invasive cosmetic procedures, such as Botox and Hyaluronic acid injectables started slowly in the early 2000s. It was managed by his OR supervisor. This changed when the nurse left and Dr. Silverton took over in 2007. The cosmetic injectable practice has grown

from barely half a day a month to a full week a month densely packed with injectables, which often overflows to Friday. Dr. Silverton has not yet delegated any of this to a trained RN, and attributes much of his success to the fact that he does not have nurse injectors. However, this has resulted in some difficult decisions as it had cut into his surgical practice. 

He has had an aesthetician since about 2005. He has offered the full Obagi line to his patients since then.

In 2011 Dr. Silverton decided to cut the office work week to four days a week, and is now closed on Fridays. He cut his office staff to an office manager, who runs the front office, and a patient coordinator who manages the surgery patients and the surgery schedule. He decided that he no longer needed a nurse on staff. This enables him to provide full service care to his patients, which has encouraged a very close and loyal patient following.

He operates on Monday, Tuesday and Wednesday and sees patients after surgery on Tuesday and Wednesday and all day Thursday for 3 weeks a month. The fourth week is an injectable week. The surgery schedule is full and getting even busier.

He purchased an Ultherapy machine and a Verju laser in 2014. He added Teoxane, Skin3 lines at this time. He also added TouchMD and creates monthly videos highlighting the monthly specials. He uses Insched and Inform and contracts out all dictation to an internet provider. The practice is all cash pay with no external billing.

Now the injectable week frequently spills over to include Friday. Most Fridays are now filled with Ultherapy treatments and Dr. Silverton is now planning to train a nurse to do Ultherapy. The laser treatments are very simple and are already administered by the aesthetician.

He is now starting a loyalty program called Club BEAUTY. He has recently completely revamped the website to a fully responsive website, that fits all mobile screen sizes. He communicates with several thousand patients by eblast twice a month, has regular blogs, monthly specials, an active Facebook page and twice yearly patient events, which are very well attended. His employees have both been with him for about 7 years, are very loyal and extremely well trained, competent, friendly and personable.

He is now back to working 5 days a week as he contemplates ways to delegate some of the injectables and Ultherapy, and pass the practice momentum to an entrepreneurial successor, who will allow him to slow down and retire.

Facility:Dr. Silverton had his own private surgery suite starting in 1982. The first one was accredited for local anesthesia with sedation. In 1990 he purchased a 2500 sq ft office with a purpose built surgical suite that was accredited for general anesthesia by AAASF. In 1998 he purchased a larger 3600 sq ft office and moved to a roomier self contained surgical suite that was accredited for general anesthesia, with

more exam rooms. This suite remained incredibly busy with 15 employees until the financial crisis of 2008. Due partly to the surgery slowdown, and partly due to the need to replace capital equipment and partly in preparation for retirement Dr. Silverton closed the larger office with private surgery suite in 2009 and was able to move back to his 2500 sq ft office, where he converted the surgery suite to offices. He continues operating at a local fully accredited out patient surgery center to this day. However, the earlier surgery suite of the 1990s could be rebuilt and brought back into service within this office if desired. 

The larger 3600sq ft facility is now available again, and Dr. Silverton would very much like this to become a major plastic surgery center again. He will help any interestred plastic surgeon to get started in this facility by sharing space and expenses, and by combining practices. All options will be explored.

FuturePlans for the future will include decisions about bringing in a nurse injector and ultherapist; change from a four day week to a five day week officially; whether to get a full electronic health records system, and which one; decisions on when to develop the operating room; decisions on whether or not to look for a second plastic surgeon, especially if the new plastic surgeon gets too busy by taking insurance and adding facial cosmetic surgery and fat grafting; and another big idea I am thinking about.
