As a safety officer I was responsible for the following duties:

Conduct daily safety and health inspections and maintain a written deficiency tracking log for an $800, 000

military warehouse modification project.

Conduct mishap investigations and complete required reports. Maintain the OSHA Form 300 and daily

production reports for prime and sub-contractors.

Attend the pre-construction conference, pre-work meetings including preparatory inspection meetings, and

periodic in-progress meetings.

Implement and enforce APPS and AHAs.

Risk Solution Partners – Honolulu, Hawaii Risk Control Consultant / (SSHO)

As a Risk Control Consultant I am responsible for the following duties:

Maintain and enforce clients’ Voluntary Protection Program (VPP) Star Status health and safety program for

Honolulu Builders on their Kunia expansion project for Monsanto. Project Safety Person of a 20+ million

dollar warehouse and farm process equipment project. The contract consisted of all phases of construction

and included trenching, crane safety, rigging, steel erection, electrical, mechanical, lockout, scaffold, fall

protection, confined space, etc. The project had as many as 60 tradesmen on site at any given time. I

generated a positive effect on every single worker on site allowing them to be more safety conscious; Gather, assess, and organize information on clients’ health and safety problems and issues;

Conduct on-site observation to ascertain unit functions, work performed, and methods, equipment, and

personnel used;

Analyze data gathered and develop solutions or alternative methods of proceeding;

Identify accident causes and help clients determine how such accidents might be prevented in the future;

Evaluate workplace environments, equipment, and practices, in order to recommend compliance with safety

standards and government regulations;

Conduct safety training sessions and education programs to management, and demonstrate the use of safety

equipment;

Confer with clients concerned to ensure successful functioning of newly implemented systems or

procedures; Recommend measures to help protect client’s personnel from potentially hazardous work methods,

processes, or materials;

Recommend and assist the clients in the implementation of health & safety solutions in order to help them

reduce their expenses related to workplace injuries and illness and thus enhance profitability;

Fluor Daniel Canada Inc. (Ford Paint Facility) - Oakville, Ontario Safety Specialist

Consultant work involving the maintenance and enforcement of existing, all-encompassing construction safety

programs and procedures at the 825,000 square foot, grass roots paint facility construction site. Issues regarding all

process equipment including paintspray booths and associated robotics, ovens and conveyor systems, water recirculation systems, ventilation systems, fire protection and security systems, etc., were supervised. I chaired

multi-union Joint Health & Safety Committee meetings, conducted site inspections, confined space analysis as well

as various other construction safety related tasks.

Education Safety Association of Ontario – Toronto, Ontario Field Consultant – South Central

I was responsible for servicing the Museums, Universities, Colleges, Libraries, Schools and Art Galleries in the Greater Toronto Area. I assisted the education sector in reducing workplace injuries and illnesses through activities

such as Certification training, audits, health and safety program development and quantitative measurements of air

quality, noise and vibration.

June 1998 to November 1998:

Health Care Health and Safety Association – Toronto, Ontario Project Team Leader

Reporting to the Executive Director/CEO, I was responsible for organizing and managing my division and

establishing procedures to meet the Health Care Health and Safety Association’s objectives. I coordinate and

monitor the delivery of Health Care Health and Safety training programs and consulting services of my division. I

was a liaison with senior managers in the health care sector. I acted as a team leader by directing and advising Consultants and Associate Consultants on a project-by-project basis to ensure they meet the needs of the Health

Care Health and Safety Association’s clients. I also performed consulting duties as required to utilize my area of

experience within any one of the divisions of the Association. I assisted the Executive Director/CEO in the hiring,

training and managing of Consultants and Associate Consultants. I was responsible for meeting with trade and

professional associations and other groups to discuss the health and safety needs of my division and to promote the

services offered by the Health Care Health and Safety Association. I also assisted the Executive Director/CEO and

Board of Directors in strategic and operational planning. My responsibilities spanned throughout the province, and

included all long term care facilities.

July 1995-June 1998:

Boart Longyear Inc. - Mississauga, Ontario Safety & Personnel Administrator (Mississauga & Sudbury facilities) I was responsible for assisting in the investigation of all work related incidences, including lost-time injuries,

medical aids, first-aids, near-misses and accidents involving property damage. I conducted regular inspections of the

steel manufacturing plants, all geo-technical and a number of mining projects. I also administered and monitored all

Workplace Safety and Insurance Board claims, acted as the company’s representative for appeals and ensured

compliance of the Occupational Health & Safety Act and all environmental regulations. I conducted and/or analyzed

all industrial hygiene monitoring. I was responsible for the development and implementation of all environmental,

health & safety programs. I successfully conducted training needs analysis and provided training programs for all

plant and management staff. I developed and administered all Human Resource procedures and programs and

conducted investigations regarding all matters related to Human Resources.

May 1993-July 1995: Xerox Canada Inc. - Oakville, Ontario Environment, Health & Safety Technical Advisor

I successfully developed and implemented policies and procedures regarding numerous factors of the environment,

health & safety program using the International Safety Rating System. Some of these developments included:

evacuation procedures, fire/property protection procedures, lock-out procedures, safe job procedures,

plant/management safety training analysis and scheduling, Joint Health & Safety Committee training and

reorganization and safe work/fire permits and policies. I also analyzed work incidents and their causes,

recommended corrective actions and followed these through to completion. Individually and as advisor of the Joint

Health & Safety Committee, I identified unsafe working conditions and prepared technical reports for unsafe

equipment and processes. I updated and organized all Material Safety Data Sheets and completed general and

specific workplace WHMIS training. I was the official company representative in all communications with

Government agencies regarding environment, health & safety matters.



March 1992-May 1993

Xerox Canada Inc. - Oakville, Ontario Safety Specialist

Consultant work involving the enforcement and development of environment, health and safety standards during the construction of the second phase of Line #1. I conducted inspections, incident investigations and reported on

substandard conditions. I was also the liaison for all permitting and approvals between Xerox and various agencies

during the construction project. I conducted hazard analysis operability studies (HAZOPS) and made

recommendations based on my findings.
