St. Mary School

St. Mary School is a pre-Kindergarten through Grade 8 Catholic Elementary and Middle School under the Oklahoma Archdiocesan Schools Office.

The curriculum stresses academic achievement within a Christian community where the child feels that he/she is loved and respected by his/her peers as well as the teacher. Vatican II texts are used so that our theology is in compliance with the bishops of the world. United with each other in meaningful liturgy and prayer, the students can further come to an understanding of the Christian life. At St. Mary, we are attempting to "teach as Jesus did."

The Diocesan curriculum guidelines, consistent with the State of Oklahoma guidelines, are followed for the teaching of all secular subject areas. The curriculum is marked by current content and fresh approaches to methodology. There is emphasis on principles rather than fact, on learning through problem solving rather than by precept. We strive to offer a program which makes use of many sources of reading material, a wide variety of audio-visual and technology tools and a multi-text approach to the content areas.

History

St. Mary School opened on September 26, 1889 as the first Catholic school in the Oklahoma Territory. Sister Alphonsa Reid, the founding Principal of St. Mary, along with two other Benedictine nuns braved the challenges inherent to Oklahoma Territory. Over the next 100 years the educators and volunteers of St. Mary have continued to offer Benedictine hospitality to all who enter.

Mission Statement of St. Mary School

St. Mary School is a vital part of the mission of St. Mary Catholic Church. St. Mary School mission is to instill in each child a desire to seek and share God's wisdom, knowledge, and truth.

Philosophy

Rooted in the Benedictine tradition upon which St. Mary was established, we exist to provide the best academic and spiritual formation to all. Placing God first in our lives and loving others as we love ourselves, we endeavor to teach the Gospel message by modeling it for our students so that they will worship more genuinely and embrace lives of service within our community.

1. To develop the total Christian personality by instilling in the students a respect for life and reverence for Christian living.

2. To develop within the school a strong religious education program based on Catholic Theology, Scripture, Liturgical experience, and service so that each child will grow and develop in the Christian way of life.

3. To facilitate the intellectual, social, and physical development of each child through a variety of instructional methods such as small and large group instruction, departmental classes, and other tools for basic knowledge and formation.

4. To aid students in evaluation of their own capabilities and vocational possibilities.

It is the consensus of the faculty that every human being has a right to pursue knowledge corresponding with his/her dignity and God-given ability, regardless of background or gender. In the early stages of life, education is often of the utmost importance because through education and guidance the children of today become the Catholic citizens of tomorrow.

The Parent-Student Handbook is arranged in alphabetical order beginning on page 4.

Absence

When a student is absent from school, a parent should call the office by 10:00 AM each day of the absence. If the office does not receive a call, a parent will be contacted. This policy is for the protection of the St. Mary students and is aligned with the state statutes of the state of Oklahoma. (Oklahoma School Law 1998)

Students who become ill at school, are vomiting, have diarrhea and/or a temperature of 99.5 or more will be sent home. In order to protect the health of the entire school community, students who are sent home during the school day must be symptom free for the 24 hours before they are allowed to return to school.

A written statement giving reasons for the absence or tardiness must be brought to the student’s teacher upon the student’s return. These notes/letters will be retained in the office for one year. Should absence for any reason other than illness seem imperative, parents are requested to consult with the Principal and present a written reason for the absence.

The school calendar provides for extended weekends throughout the school year. Parents are encouraged to schedule trips or family outings during these times so as to eliminate the need to interrupt a child’s learning process. Missed assignments are the student’s responsibility.

Students who are absent due to illness have one day for each day of absence to make up the missed assignments, quizzes or tests. For example, a student who was absent three days would be given three school days to complete the missed work.

When a student is absent for two or more days, a parent may call the school office before 9:30 AM to arrange for homework assignments. Homework assignments may be picked up at the school office between 3:00 PM – 3:30 PM.

For short absences, students should make arrangements with classmates regarding assignments. Students may also receive missed assignments from their teacher when they return to school.

Arrangements for regular classroom tests missed because of an absence are to be made with the individual teachers. These tests must be taken within one week of the original test date.

Excessive absence (30) days or the equivalent of 30 days including tardies, can be cause for a student to be retained in the current grade for another year. *See next section for fees associated with excessive tardies.

Absence During the School Day

Students needing medical appointments during school hours require a written note by the parent. Parents are required to sign out their child. If the child returns to school during the same school day, he/she must be signed back into school in the office. Students who are away from school for an appointment for 3 ½ hours or more will be counted as absent for ½ a day. Three (3) early withdrawals each of which are less than 3 ½ hours are considered a one-half day absence. Students who are tardy more than 40 times will be charged an administrative fee of $5 per tardy or early withdrawal due to the excessive time and paperwork involved.

Allergy Policy

St. Mary School recognizes that asthma is a serious condition affecting many school children and positively welcomes all pupils with asthma. The school encourages children with asthma to achieve their potential in all aspects of school life by having a clear policy that is understood by school staff and pupils. Teachers and new staff are also made aware of the policy.

1. Asthma MedicationImmediate access to reliever inhalers is vital. Children are encouraged to carry their reliever inhaler. Parents are asked to ensure that the school is provided with a labeled spare reliever inhaler. All inhalers must be labeled with the child's name by the parent.

2. Record KeepingAt the beginning of each school year, or when a child joins St. Mary Catholic School, parents are asked to submit a child’s medical record. From this information the school keeps its asthma register which is available for all school staff. If medication changes in between times, parents are required to inform the school.

3. The School EnvironmentThe school does all that it can to ensure its environment is favorable to children with asthma or allergies. The school is an e-cigarette, vape and tobacco-free environment. This policy extends to all clergy, congregants, employees, volunteers, clients, customers, visitors, vendors, contractors, students and others at St. Mary. As far as possible the school does not use chemicals in science and art lessons that are potential triggers for children with asthma.

4. Food Allergy Policy

St. Mary School recognizes that life threatening food allergies are an important condition affecting many school children and positively welcomes all pupils with food allergies. In order to minimize the incidence of life threatening allergic reactions, St. Mary School will maintain a system-wide procedure for addressing life threatening allergic reactions and maintain an Emergency Action Plan for any student(s) whose parent/guardian, and physicians have informed the school in writing that the student(s) has a potentially life threatening allergy.

Discipline

Christian Due Process

The students’ interest in receiving a quality, morally based education can be served if students, parents/guardian and school officials work together. Normally, differences between these entities can be resolved. In some rare instances, however, the school may find it necessary, in its discretion, to require parents/guardians to withdraw their child. It shall be an expressed condition of enrollment that the parents/guardians of a student shall also conform themselves to standards of conduct that are consistent with the Christian principles of the school, as determined by the school in its discretion. These principles include, but are not limited to, any policies, principles or procedures set forth in any student/parent handbook of the school.

These Christian principles further include, but are not limited to, the following: 1. Parents/guardians are expected to work courteously and cooperatively with the school to assist the student in meeting the academic, moral and behavioral expectations of the school. 2. Students and parents/guardians may respectfully express their concerns about the school operation and its personnel. However, they may not do so in a manner that is discourteous, scandalous, rumor driven, disruptive, threatening, hostile, or divisive. 3. These expectations for students and parents/guardians include, but are not limited to, all school-sponsored programs and events (e.g., extended care, athletics, field trips, etc.)

Discipline cont.

St. Mary Catholic School understands self-discipline in the light of the Gospels and Jesus’ relationship with his disciples. Jesus empowered his disciples to reflect on their attitude, actions, behaviors and feelings in such a way that they were able to change and grow in the Good News of God’s Love. In the same way, our style of discipline is an approach toward discipleship with Christ. Through discipline, we empower our students to reflect on their behaviors, attitudes, decisions and actions that are Christian or unchristian so that they can choose to change, establish healthy boundaries and limits, and grow in God’s love and become productive citizens.

With that goal in mind, our school implements the 15 skills of the developmental process called, “Discipline with Purpose.”

Basic Skills: Listening, Following Instructions, Questioning, Sharing, Social Skills

Constructive Skills: Cooperation, Reasons for Rules, Completing a Task, Leadership, Communication

Generative Skills: Organization, Resolving Problems, Initiating Solutions, Fact versus Feeling, Service to Others

Opportunities to teach students the 15 self-discipline skills abound within any school day. The teachers model the skills, pre-teach ways to demonstrate the skills, infuse skill language in other curriculum, and they use prepared lesson plans to directly teach the skills. An excellent time to teach the skills is when disruptive behavior occurs. Because teachers believe that confrontation (coming face to face) is a part of their task as disciplinarians, they welcome opportunities to help students help themselves. All disruptive behaviors are routinely handled on the spot by the adult witnessing the behavior. Each teacher works with the students to make sure rules are understood and the reasons for the rules are taught. Students learn that each adult may have different procedures in helping them follow the rules.

Code of Conduct

No discipline code can ever be written which covers all possible acceptable and unacceptable behaviors. However, this is the main outline of all school and classroom rules: 1. Respect yourself, others, and things. 2. Contribute to the learning environment. 3. Follow school and classroom procedures.

Classroom Management Cycle

Each teacher will have a personal classroom management cycle that is used in dealing with minor infractions of the rules listed above. This management cycle will be taught to the children.

School Discipline Cycle

Occasionally removal from the group or externally imposed discipline is necessary to protect a student and/or the learning environment of the school. This is necessary when a student: 1. Puts self or others in physical or psychological danger. 2. Is irrational or unreasonable. 3. Persists with unacceptable behavior after completing the classroom management cycle. 4. Goes far beyond bounds of respect.

The following pages are discipline policy/procedure for Prek - 2nd and 3rd - 8th :

Note: A transitional policy is used for the second semester of 2nd grade and the first semester of 3rd grade. For a copy of the policy please ask the classroom teacher.

Preschool-2nd Grade Discipline Policy

St. Mary’s teachers meet the children at their individual stage of development and help each child reach challenging and achievable learning goals. In order to help teachers provide meaningful, relevant, and respectful learning experiences for each child. Our students are expected to follow certain behavioral guidelines including, but not limited to the following: follow directions given by all school staff, respect the rights and property of others in actions and words, keep hands, feet, and objects to self, and use all areas and equipment safely.

Discipline Guidelines

The following general guidelines are to be implemented when a student purpose-fully engages in behaviors that are against the expectations set forth above.

The administration and staff of Saint Mary’s Catholic School recognizes that two situations are rarely the same in every respect, and that it is important to maintain flexibility and to take into account individual circumstances. Factors to be consid-ered include, but are not limited to:  developmental stage of student (age, maturity, etc.), seriousness of the offense (length, intensity, severity), the frequency of mis-conduct, the effect of the misconduct on the school environment, the requirements of a student’s Individual Education Plan ( for students with disabilities).  Excep-tions may be made by the teacher, in consultation with the principal, based on spe-cific student needs. These will be outlined in an Individual Behavior Plan.

Verbally hurtful behavior can be defined as deliberately harmful speech that is typically both unprovoked and repeated.

If a student engages in mild verbal aggression towards another student or staff member, consequences may be implemented at the discretion of the teacher (con-sidering all relevant circumstances): removal of student from situation, redirection, loss of privileges, lunch detention, behavioral training, in house suspension, age appropriate chores, parental notification etc.

If the above strategies are ineffective, further steps or actions may be implemented; including, but not limited to: behavior intervention plan, behavioral education, sus-pension, expulsion.

OUT OF SCHOOL SUSPENSION: Suspension is a step that will be taken only as a last resort when a student’s behavior, even with the implementation of a be-havior intervention plan, continues to significantly disrupt the educational program of other students and/or presents a danger to the student or to other individuals. The principal will take into consideration all circumstances when making the deci-sion to suspend a student. There is no appeal of the Principal’s decision to suspend a student.

EXPULSION: Students may be expelled from the school for serious violation af-ter the above suspension procedures are followed. Expulsion can occur without first using suspension. The principal will take into consideration all circumstances when making the decision to expel a student. There is no appeal of a decision to expel a student.    EXCEPTIONS TO THE DISCIPLINARY PROCEDURESThe Principal may make exceptions to the written disciplinary procedures where in his/her judgment such exception is warranted.

3rd-8th Grade Discipline Policy

The objectives of St. Mary’s Catholic School are to inspire Christian values and re-spect for others, stimulate a love for learning and intellectual capacity, acquaint children with church teachings and gospel values, nurture and foster the diverse gifts of each child, and form a community that teaches respect, compassion, and service. St. Mary’s teachers meet the children at their individual stage of develop-ment and help each child meet challenging and achievable learning goals. By con-tinually observing children’s play and interaction with the physical environment and others, our teachers learn about each child’s interests, abilities, and develop-mental progress. We get to know the children’s families and learn about the values, expectations, and factors that shape their lives at home and in their communities. In order to help teachers provide meaningful, relevant, and respectful learning experi-ences for each child, our students are expected to follow certain behavioral guide-lines. Students are required to follow directions given by all school staff, respect the rights and property of others in actions and words, keep hands, feet, and objects to self, and use all areas and equipment safely.

Field Trips1. Field trips are designed to correlate with teaching units and to achieve

curricular goals.2. Field trips are re-evaluated each year to determine the trips compatibility

with curricular goals.3. A field trip is a privilege and not a right.4. All grades do not always have the same number of field trips.5. Field trips are permissible for all grades when advanced planning, location,

and the experience insure a successful learning opportunity.6. Individual teachers, in consultation with the Administration, reserve the

right to restrict or deny student participation on any field trip due to, but not limited to, poor academic performance and/or poor conduct.

7. A written official permission slip, signed by the parent, is required before a child will be permitted to attend a field trip activity. Verbal permission cannot be accepted. Permission slips are due in the office forty-eight hours after the parent or guardian has received the permission slip.

8. A field trip permission slip is printed at the end of this book. This is the only format that may be used to allow a student to leave school during school hours. If your child fails to bring his/her permission slip home, you may tear out and use or copy the form provided. Call the school for information needed to complete the form. The student will not be allowed to return to school until all nits have been removed and the possibility of infecting other students is gone. Students should not share hats, combs, coats and other items that may encourage the spread of head lice. Contact your child's physician, the school secretary, or the Logan County Health Department for assistance in treating head lice.

Health Screenings

A health screening, conducted by Logan County Health Department, is held once a year to test for hearing difficulties for students in Pre-Kindergarten through Grade 8. Oklahoma Senate Bill 1765 requires students in Kindergarten, first, and third grades to undergo a vision screening. The parents should arrange this screening with a qualified doctor. Scoliosis screening is conducted for students in Grade 5. Parents are notified of the outcome.

HIV/AIDS

St. Mary follows Archdiocesan policy regarding HIV/AIDS. Copies of the policy are available by request through the school office.

Home-School Communication

In order to insure that all communication from school reaches home in a timely manner, St. Mary School uses a Thursday folder and an email system. Official folders containing correspondence and graded assignments are sent home on Thursdays and should be returned the following school day. Your child is responsible for emptying the contents of his/her envelope when it is returned to their homeroom. There is a $2.00 replacement charge for envelopes that are lost. Information is not sent home if the Thursday folder has not been returned. Weekly agendas and homework information is sent via email on Mondays. Official school-wide emergency communications are sent using the School Connect® push message system. Teachers may also use School Connect® to send out reminders of upcoming events and projects.

Homework

Formal home-study is assigned to help students become self-reliant and self-directed. Assignments are designed to reinforce daily lessons, to supplement and enrich class work, and to prepare for certain lessons through various experiences.

Since each student has different capabilities and interests, it would be difficult to denote the specific amount of time to be spent on an assignment. If a problem arises, the teacher should be contacted.

Homework may be assigned to students Monday through Thursday. Although homework will not be assigned to elementary students on Friday evening, students in Middle School may expect some weekend homework.

Homework due to Vacations/Planned Absences

The school calendar provides for extended weekends throughout the school year. Parents are encouraged to schedule trips or family outings during these times so as to eliminate the need to interrupt a child’s learning process. Missed assignments are the student’s responsibility.

Teachers are not required to give make-up tests or assignments for absences due to vacations. No assignment will be given in anticipation of the vacation.

Homework Policy Due to Illness

When a student is absent for two or more days, a parent may call the school office before 9:30 AM to arrange for homework assignments. Homework assignments may be picked up at the school office between 3:00 PM – 3:30 PM.

For short absences, students should make arrangements with classmates regarding assignments. Students may also receive missed assignments from their teacher when they return to school.

Students will be allowed one day for each day of absence due to illness. For example, a student who was absent three days should be given three school days to complete the missed work.

Meningococcal Disease and Vaccine

The following information is provided to families in compliance with Oklahoma State Law.  Meningococcal disease is a rare, but sometimes-fatal disease caused by a bacterium called Neisseria meningitides. The disease causes either meningitis, severe swelling of the brain and spinal cord, or meningococcemia, a serious infection of the blood.

Teenagers and young adults, aged 15 to 22 years, are at increased risk because of behaviors that spread the disease.  On average two to three people in this age group get meningococcal disease every year in Oklahoma.  More than half of these cases could be prevented by vaccine. Vaccines can prevent many, but not all, types of meningococcal disease.  There are two vaccines available in the United States that protect against four of the five most common strains of the meningococcal bacteria.  The newest vaccine, called Menactra, or MCV4, is currently available for: adolescents entering high school (15 years of age), college freshmen who live in dormitories, and other people at high risk eleven through fifteen years-of-age.

Parent’s Role in Education

We, at St. Mary School, consider it a privilege to work with parents in the education of children because we believe parents are the primary educators of their children. Therefore, it is your right and your duty to become the primary role models for the development of your child’s life---physically, mentally, spiritually, emotionally, and psychologically. Your choice of St. Mary School involves a commitment and exhibits a concern for helping your child to recognize God as the greatest good in his/her life.

Good example is the strongest teacher. Your personal relationship with God, with each other, and with the Church community will affect the way your child relates to God and others. Ideals taught in school are not well rooted in the child unless these are nurtured by the example of good Catholic/Christian morality and by an honest personal relationship with God in your family life.

Once you have chosen to enter into a partnership with us at St. Mary School, we trust you will be loyal to this commitment. During these formative years (Pre-K to 8), your child needs constant support from both parents and faculty in order to develop his/her moral, intellectual, social, cultural, and physical endowment. Neither parents nor teachers can afford to doubt the sincerity of the efforts of their educational partner in the quest of challenging, yet nourishing, the student to reach his/her potential. It is vital that both parents and teachers remember that allowing oneself to be caught between the student and the other partner will never have positive results. To divide authority between school and home or within the home will only teach disrespect of all authority. If there is an incident at school, you as parents must make investigation of the complete story your first step. Evidence of mutual respect between parents and teachers will model good mature behavior and relationships. Talking negatively about a child’s teacher at home will only create an attitude of distrust toward the teacher, the school, and the parent.

Students are naturally eager to grow and learn. However, sometimes in the process of maturation new interests may cause them to lose focus. As this natural process occurs, the student needs both understanding and discipline. At times, your child may perceive discipline as restrictive. However, it is boundaries and limits which provide a young person with both guidance and security.

It is essential that a child take responsibility for grades he/she has earned and be accountable for homework, long-term assignments, major tests, service projects, and all other assignments. Parents are encouraged to let their child experience a logical consequence for an inappropriate action or behavior. This responsibility also extends to times of absence. Together, let us begin this year with a commitment to partnership as we support one another in helping your child to become the best person he/she is capable of becoming.

Parties are held between 2:30 and 3:00.

Pet Policy

Individuals with a documented need for a service animal or service animals as employed by the school are welcome on the school campus and at school-related activities.  All other pets or animals are not allowed on the school campus at any time unless for an ad-ministration approved activity. Administration approval is only intended for events in which the presence of animal encourages the learning process or specific, Church spon-sored pet blessings. Pets or other animals are not allowed on any school field trip or other school sponsored event, including school-sponsored social events.

Promotion Policy and Retention Policy Advancement to the next grade in St. Mary School is based on a student’s daily performance, test results, recommendations of teachers, and the student’s ability to complete work successfully on a more advanced level. Promotion to the next grade depends on successful completion of all subject areas. The Administration may recommend the repetition of a grade, tutoring, or summer school classes as requirement for promotion when, after conferences with teachers and parents, it is believed that such action will better prepare the student academically or emotionally for the next grade. Students may also be Transferred to the next grade. This simply means that a student is moving to the next grade against the recommendation of the teacher.

Sacramental Program

The sacramental life of the children of the Catholic tradition is an important component of the religion program at St. Mary School. Preparations for two sacraments, Reconciliation and Eucharist, form the core of instruction in Grade 2. In accordance with the diocesan guidelines, candidates for First Eucharist will receive the sacrament of Reconciliation prior to First Eucharist.

Parents are required to be active partners in the preparation of their children for these sacraments. The sacraments of Reconciliation and Eucharist are only conferred to students baptized in the Roman Catholic tradition.

School Hours

Grades Pre-K through 8: 8:00 AM – 3:15 PM. Students not in the school at 8:00 AM are considered tardy even on Wednesday during Mass.

The school doors are opened for students at 7:00 AM. Students arriving between 7:00 and 7:45 will go to Before School Care located in the gym.

Prayer and afternoon announcements begin at 3:05 PM each day. Dismissal immediately follows. Please check the school calendar and weekly newsletter for early dismissal dates.

Technology Concerns

Electronic Readers (E-readers): Electronic readers, simply called “e-Readers”, are digital devices that can store books, periodicals, magazines, and other electronic media. e-Readers like Amazon’s Kindle®, Barnes & Noble’s Nook®, Apple’s iTouch®, and Apple’s iPad® are quickly becoming ubiquitous in our digital culture and they simply cannot be ignored. St. Mary School, in striving to maintain technological relevance in education, is providing the opportunity for students to use these devices in accordance with our e-Reader Acceptable Use Policy. This opportunity is a privilege that requires extra caution and responsibility both on the part of the students and their parents.

e-Reader Acceptable Use Policy: The wide variety of hardware and software capabilities of available e-Readers makes them challenging to monitor and control in a school environment in contrast with school owned technology assets like computers, etc. Therefore, our e-Reader Acceptable Use Policy needs to be specific and clear. A student who violates any portion of the e-Reader Acceptable Use Policy may immediately lose the privilege to use their e-Reader at school for a length of time commensurate with the nature of the violation. 1. All e-Readers must be registered with the St. Mary School Office and accompanied by the Acceptable Use Agreement Form signed both by the parents and the student. 2. e-Readers are to be used only for the reading of school approved material (books, etc.) and not for other purposes such as communication, entertainment, music, gaming, etc. 3. All material on the e-Reader must comply with the spirit and policies of St. Mary School. Please refer to the Parent-Student Handbook for more details. 4. All e-Readers must have cellular and network capabilities disabled (turned off) while the device is at school.

50

e-Reader Acceptable Use Policy (cont):

5. e-Readers must be used at appropriate times in accordance with teacher instructions. The e-Reader must not be a distraction for the student or those around him/her, nor be a source of any classroom disruption. 6. e-Readers are not to be used during lunch, middle-school breaks, or during playground/recess time. 7. The student is responsible for knowing how to properly and effectively use their e-Readers and this should not be a burden for the teachers.

Telephone

Permission to use the telephone must be obtained from the school secretary. Students must submit a note from their teacher. The office phone is a business phone and students are permitted to use it only in case of an emergency. The telephone in a Teacher’s Lounge is for the use of the teachers only. Forgotten homework, athletic equipment, etc. do not constitute emergencies. Arrangements for after-school visits with friends should be made at home.

Testing

There will be out-of-uniform days (free-choice) which will be announced during the course of the school year (a dress code for such days is included below). If there is a time when the prescribed uniform cannot for some legitimate reason be worn, a note from the parent/guardian must be written to the Principal. Students who are out of uniform without this excuse will be sent to the office and the parent will be called to bring the proper clothing to the student.

Attire for P.E.: Middle school students must change into St. Mary Spirit Shirt and shorts. The girls must wear long, “basketball” style shorts for P.E. ImmunizationsImmunizations are required for all NEW students for the 2017-2018 school year. Families with continuous enrollment in the Archdiocese of Oklahoma City prior to the 2016-2017 school year are accepted with their current immunization status.Every new student enrolled in a Catholic school in the state of Oklahoma shall be immunized against vaccine preventable diseases caused by infectious agents in accordance with the immunization schedule adopted by the Oklahoma Department of Health Services. A student who fails to present the required evidence shall not be accepted for enrollment. The only exception to the forgoing requirement is a medical exemption signed by a licensed physician (M.D. or D.O.) authorized to practice in the State of Oklahoma.*Procedures for a Child Needing a Medical Exemption (or for students of continuously enrolled families prior to 2016-2017 school year)Parents are required to submit a waiver request for immunization exemptions in writing (Oklahoma Department of Health Form #216A). Parents are referred to schools to obtain the forms. A physician must sign medical exemptions. Parents should then return the entire form to the school. properly completed, is legible, and signed. A copy of the form or statement must be kept on file in the student’s cumulative record and a copy must be sent to the Immunization Service for approval. Parents should be informed that their child might be excluded from school in the event of an outbreak of a disease for which their child is exempted. Each exemption submitted to the Immunization Service is reviewed for validity and accuracy. If the exemption is determined to be invalid, the form or statement will be returned to the school along with a letter explaining the reason why the exemption is invalid. If the exemption is valid the form is returned to the school marked with an approval stamp. The Immunization Service does not keep a copy of the exemption certificate.Oklahoma’s immunization law does not require a new exemption form to be filed every year. It is the policy of the Immunization Service to accept exemption forms filed when the child entered the school system. Exemption forms should accompany children when they transfer between schools. Schools should not keep the exemption form of a transferring student. If a student transfers into Oklahoma schools from another state, an exemption form must be completed to comply with Oklahoma law. If an exemption form has been misplaced, a new form should be completed, but if an exemption form is on file, an additional form is not needed unless vaccine requirements change for the grade level of the child, such as when new vaccines are added for students entering the 7th grade to which the students does not have a previous exemption. If a parent needs a copy of a previously filed exemption form, they must obtain it from the school. If a copy of the original exemption is not available, the parent must complete a new form. The Immunization Service does not provide copies of previously filed exemption forms for parents. The exemption process must be completed, approved and on file at the school prior to the first day of school or students will not be admitted. *See Dignitas Personae from the Congregation for the Doctrine of the Faith.
