The SQF Quality Achievement Awards Program recognizes the outstanding commitment, support and performance of individuals and organizations who contribute to endorsing and improving the SQFI program and mission around the globe.

Help recognize the achievements of these outstanding individuals or companies by nominating them for this prestigious award.

Include SPECIFIC details about how the individual / company:

• Contributes significantly to achieving the SQF Mission

• Has communicated the goals, mission and benefits of the SQF program to both internal and external stakeholders

• Has proven his/her/their understanding and commitment to the SQF program.

• Has demonstrated an ongoing personal commitment to improving the awareness of food safety within the global community.

RulES & ElIGIBIlITYn All nominators and nominees must

be affiliated with the SQF program. Examples of appropriate affiliations include, but are not limited to registered auditors, consultants or trainers, licensed certification bodies, registered and certified suppliers, licensed training centers, etc.

n All nominees must have shown ongoing and dedicated support for the Safe Quality Food Program during the 2012 and/or 2013 calendar year.

n The entry form must accompany your nomination and all sections must be filled out legibly.

n Narratives must be 500 words or less.

n Nominators must obtain permission from the nominee prior to submitting a nomination.

n Self-nominations will be accepted.

n Specific evidence and supporting documentation should be included for all comments and accolades given by the nominator on behalf of the nominee. Examples of supporting documents include published articles, industry awards, news articles and 3rd party testimonials. These materials will not be returned to you. No videos will be accepted.

JudGInG CRITERIan Only complete, legible entries will be

considered.

n Entries will be scored and judged by a panel consisting of a member of the SQFI staff, a member of the Senior FMI Staff and an executive from a food retailer.

n Final award decisions will be made by members of Safe Quality Food Institute for each specific award category.

n Finalists will be notified on or before September 1, 2013

SCoRInG CRITERIaEntries will be scored on overall content and the nominee’s demonstrated achievements in:

n Contributing significantly to achieving the SQF Mission.

n Communicating the goals, mission and benefits of the SQF program to both internal and external stakeholders.

n Proving his/her/their understanding and commitment to the SQF program.

n Demonstrating an ongoing personal commitment to improving the awareness of food safety within the global community.

EnTRY SuBMISSIonAll nominations must be submitted in the requested format no later than 12:00 AM EST (USA), August 1, 2013. Email is the preferred method of receipt. Send to John Schulz at [email protected]

PRIzESn We will honor the best finalists from each

category at an awards luncheon on Thursday, November 7, 2013 during the 2013 SQF International Conference, where Grand Prize winners (one in each category) will receive a crystal award.

n The Grand Prize winners will receive complimentary registration to the 2014 SQF Conference.

n All finalists will receive a reduced rate registration to the 2013 SQF Conference.

n The Grand Prize winners will be recognized in an SQFI press release, in the winter edition of the SQFI Newsletter and on the SQFI web site.

significantly to achieving the SQF Mission of delivering consistent, globally recognized food safety and quality certification programs based on sound scientific principles, consistently applied across all industry sectors, and valued by all stakeholders.

B. SQFI Training Center (TC) of the Year – Licensed TC who has contributed significantly to achieving the SQF Mission.

C. SQFI Manufacturer/Distributor of the Year – Certified supplier who has demonstrated excellence in the SQF system and has promoted the SQF system.

D. SQFI Primary Producer of the Year – Certified supplier who focuses on agri-business (primary producer) and has shown a commitment and excellence in the SQF system.

E. SQFI Auditor of the Year – Individual who has demonstrated outstanding performance as a credentialed SQF Auditor.

F. SQFI Outstanding Achievement Award - This award category is open to all individuals currently registered, licensed or certified with the SQFI or are somehow affiliated with the SQFI and who has shown some type of outstanding performance in support of the SQF program.
