Executive Summary

Project Name: Soweto/Kayole Primary Healthcare Clinic

Location: Nairobi Kayole is one of many slums located on the outskirts of Nairobi, Kenya’s capital. With an estimated population of 700000, most of its residents live in abject poverty in homes that lack electricity and running water. In addition, more than 60% of the population are unemployed with most families living on less than one dollar a day.

Description: Soweto Kayole PHC Clinic was established by African Enterprise, Kenya in 1996 as a community health centre with the aim of providing affordable health services to the residents of Soweto Kayole & the larger Embakasi District. This not-for-profit Clinic is a beacon of hope for many who cannot afford to access quality medical care and are often forced to turn to quack medics and ill equipped unlicensed medical facilities.

Proposal: To add a 20 bed maternity facility to the existing Clinic that can provide safe and adequate maternity care for all concerned. It will not come as a surprise to hear that mortality rates in the slums are higher than the country’s average. To address this imbalance the clinic provides high quality outpatient services in Prenatal and Ante-natal care (ANC), various HIV prevention & management focussed health care programs such as voluntary counselling & testing (VCT), prevention of mother-child transmission (PMTCT), Lab services (TB & routine tests), TB diagnosis & treatment, administering of ARVS etc., and on occasions receives emergency cases that require specialized care. While this Clinic is now viewed as being the most efficient, affordable and highly respected community based health care centre within the area, it still does not have the facilities to assist mothers giv-ing birth or to offer full medical attention to the babies being born.

Soweto Kayole Primary Health Care Project, Nairobi.Soweto Kayole PHC Clinic was established by African Enterprise, Kenya in 1996 as a commu-nity health centre with the aim of providing af-fordable health services to the residents of the neighboring slums. This clinic has since grown to be a reputable healthcare facility that offers high quality integrated medical services such as general illness treatments, pre and post natal health services, health education and aware-ness campaigns, and many other services.In the 18 years since inception it has steadily grown from a little known clinic into its current form and now offers the following curative & preventive services: 1. Ante-natal care.2. Voluntary counseling & testing (VCT)3. Prevention of mother-child

transmission. (PMTCT)4. Lab services (TB & routine tests)5. TB diagnosis & treatment.6. Administering of ARVS.7. Notification of home births8. Emergency ambulance response9. Immunization services.10. Family planning.11. Children nutrition programs.12. Spiritual nurture.

In the first six months of 2013, the Clinic managed to serve fourteen thousand, six hundred patients suffering from various illnesses while the clinic’s emergency ambulance services rescued and transferred two hundred and sixty four (264) patients to various referral hospitals in Nairobi. Most importantly, many of these patients have had an opportunity to listen to the Gospel of the Lord Jesus Christ during the daily morning devotions and prayers held at the clinic.Since this clinic serves very needy and vulnerable people who cannot afford health

services in other private health facilities, many of our patients are unable to pay for the highly subsidized services and yet they cannot be turned away unattended. This scenario truly drains the scarce resources and hence we must constantly rely on the goodwill of our partners and well-wishers such as drug suppliers, individual supporters, churches and the local community to keep this clinic operational.Moreover, the growing need for maternal interventions in the neighboring slums has forced us to open the clinic doors to the local women for maternity services even before we have the resources to build and equip a proper maternity wing.Of the approximately 1.2 million babies born each year in Kenya between 50,000 and 60,000 infants are thought to become infected with HIV as a result of mother to child transfer (MTCT).Even without a maternity section, the clinic is having great success in reducing the transfer-ence of HIV from mother to child albeit during the post natal phase so the hope is that an almost 100% non-transference rate could be achieved if the mother is in treatment before and during the birth. An additional benefit from this development would be that the clinic would become more financially independent as this service can charge a small fee for the service.Attached please find some photos of the clinic as well as a copy of a testimony shared by the Soweto Kayole clinic ambulance driver who pro-vides rescue & emergency first aid care to the residents before they are received at the clinic or referred to any other referral medical facility for more specialized care.

Soweto PHC Clinic priority needs;1. Provision of resources to construct a

maternity wing with a 20 bed capacity at the clinic. (See detailed budget included)

2. Financial Resources to purchase medical supplies and key medical equipment. (on-going)

3. Resources to fully computerize the facility.

SOWETO KAYOLE PRIMARY HEALTHCARE PROJECT

Ambulance Report

The following is a recount of one of the experiences and encounters of Mr. Henock Machoka (the AEE Soweto Kayole Community Ambulance Driver).

“On 24th May, 2013, I was at the clinic when I received a distress call and rushed to rescue an expectant woman who had developed complications while delivering at home. I picked up the patient and sped off towards the Pumwani Maternity Hos-pital which is the nearest public maternity hospital located about 20 Kilometers away. The patient’s blood pressure was quite high at 224/146 which posed grave danger to the patient but my assistant was at hand to

help out and stabilize the patient. Due to the heavy traffic, it took us half an hour to reach the maternity hospital despite speeding and flashing the ambulance siren.

On arrival at the hospital casualty in Pum-wani, we were informed that their main theatre was closed due to technical prob-lems and we were directed to Kenyatta National Hospital another 20 kilometers away. By now the patient was in a very bad condition and we almost lost hope but thank God we managed to get there before it was too late. The patient was quickly received and moments later she delivered a healthy baby girl. I always thank God for this fam-ily since that case was extremely risky not only for the mother but also for the baby. The Lord saved both of them and we are always excited to see them when they come to the Child Welfare Clinic at Soweto Kayole”.
