This Statement of Qualifications is intended for your use in considering tpmgroup Group for upcoming projects. It specifically addresses tpmgroup’s capabilities and experience in environmental, emergency response, industrial maintenance and aviation fueling systems.

For over two decades tpmgroup has provided its clients a broad range of problem solving expertise, profession experience and in-house equipment to complete both emergency and planned projects that have protected public safety, facilities and property as well as the environment. tpmgroup will bring this same dedication to quality & professionalism, experience and resources to assist its clients on going and future site, remediation, hazardous waste removal, and abatement needs. tpmgroup maintains a variety of heavy equipment for construction, vacuuming, demolition and other field activities.

T.P.M., Inc. DBA tpmgroup(tpmgroup) was established in 1993 has offering services to its clients for over 22 years as a Small Business Concern which operates in the form of a Corporation, organized in the Commonwealth of Kentucky. The Corporate headquarters are located at 2040 Old Louisville Road, Bowling Green, Kentucky 42101.

tpmgroup originated in 1991 as a subsidiary of Hanks Construction Company of Bowling Green, Kentucky. tpmgroup initially provided Underground Storage Tank (UST) removal and installation services. In 1993 tpmgroup became a Kentucky Corporation and began a new challenge of providing professional full service environmental and construction services. The addition of a drilling program and 24 hour emergency response environmental team further increased tpmgroup’s capabilities to clients in 1994. In 1998 tpmgroup added property and land and proceeded to permit 9 (1) acre landfills for C, D & D. In 2002, Apex Environmental was formed with locations in (I) South Union, KY and (II) Bowling Green, KY. Apex I primary function is handling waste solidification while Apex II is a centralized waste treatment (CWT) facility.

tpmgroup has a southern office located at 635 Hangar Lane, Nashville, Tennessee 37217 and expanded again in 2015 by establishing an office in Louisville, KY to service a five year contract with Louisville-Jefferson County Metro Sewer District.. Over the last 20 years tpmgroup has grown from a local services company, to a company that has a reach to serve all of Kentucky, Tennessee, Southern Indiana, in addition to areas of Alabama, Illinois and Mississippi. tpmgroup has also provided support for both the BP/Deepwater Horizon-Gulf Coast Spill and the Enbridge Pipeline spill in Michigan.

tpmgroup has built a solid, regional reputation for construction management, environmental and geotechnical engineering service excellence. The company is structured to deliver responsive, client-orientated services at a reasonable cost. tpmgroup is fully insured, financially secure, and has a diverse and expanding client base from both the private and public sectors.

tpmgroup not only responds to emergencies and disasters for containment and cleanup but alsoprovides prevention services. tpmgroup is pleased to have A&A Enterprises as a strategic partner. A&A isthe owner of the patented Guardian Retention Systems (GRS) line of products. The GRS GuardianSystems’ barrier bags makes an ideal, quickly assembled barricade against floods, water retention, andvarious threats of encroaching waters. The unique, patented trapezoidal design of Guardian bags, unlikestandard rectangular sandbags, employs gravity in creating downward pressure to form a tight seal.Because the Guardian bags are held in place by the Guardian unit and filled with a large bucket machinesuch as a backhoe, the system uses just a fraction of the time and manpower needed to fill traditionalsandbags. In fact we can deploy 100 lft of the Flood Guardian with a crew of 4 in only 20 minutes!

Unlike many other products on the market the Flood Guardian has been tested by the US Army Corps ofEngineers Engineer Research and Development System which found that the system is “an expedient andcost-effective solution for a temporary flood-fighting barrier.”. The Flood Guardian is also a provensuccess having held back Mississippi River Flood waters threating the Harlow Casino in Greenville,Mississippi where over 1.7 miles of 8’ high flood wall was constructed with the system in 72 hours andprotected the casino for 30 days. Harlow Casino estimated the system saved the casino over $10,000,000in property damage.

The UST services provided by tpm group are essential for upgrading existing UST systems to comply with current and future regulations. They are also necessary when converting property from UST sites to other real estate applications.

tpmgroup operates various types of drill units with fully trained drill teams. TPM has local base resources in Bowling Green and Louisville, Kentucky and Nashville, Tennessee. These services include utilizing methods such as hollow stem auger, air and mud rotary, diamond rock coring, casing advancement and direct push methods.

Our drilling services include soil sampling, drilling and monitoring well installations, boring and well abandonments, and well head protections, grouting remedial treatments and various rock and mineral exploration.

INDUSTRIAL SERVICES

tpmgroup provides industrial cleaning and plant maintenance services for a variety of industries including transportation (trucking, rail and aviation) Averitt Express CSX Railroad and Nashville Metropolitan Airport, large utilities on the scale of TVA, and LG&E and Municipalities such as Metropolitan Nashville and White House Tennessee.

A good cross section of our client base would include automotive casting and tooling manufacturing, commercial and residential contractors and local state and federal government waste water systems. We serve many National Manufacturers and Independently owned small business in the state of Tennessee and Its not uncommon on any given day tpmgroup could begin the morning at Advanced Composites and finish up in the evening serving a “Mom and Pop” Feed and Seed or independently owned Gas Station.

With a history in industrial cleaning that dates to 1993, tpmgroup has developed customized industrial cleaning solutions in water blasting, vacuuming, hydro excavation and a myriad of other services that quantify tpmgroup as the industry leader in its field.

With more than 30 highly-trained field service employees, a mobile fleet of than 9 units, and complete engineering and tooling capabilities, there is not a cleaning challenge that is too large or complex. Tpmgroup’s trucks range in capacity from 3,000 to 5,000 gallons. The high-velocity air vacuum trucks handle liquid, solid and sludge removal needs while wet vacuum trucks are designed for the removal of liquids, such as acids, caustics, oil and water as well as some light sludge. High-velocity air vacuum trucks also have a blower system for additional vacuum loading and unloading. Our trucks are designed to remove both wet and dry materials, including flammable liquids, paint sludges, oily sludges, powders and other hard-to-remove materials.

Slope Stabilizationtpm group understands that erosion is a problem that frequently develops on highly erosive soils, steep slopes, and areas with weathered rock which presents a high risk for slope instability resulting in landslides. Highways, railways, power plants and transmission facilities as well as residential and commercial developments all require long-term stable slopes to control land slides. Traditionally this has been an expensive and time consuming proposition with project schedules ranging from several weeks to months depending upon the size of the slide.

The Kentucky Department of Transportation has discovered the GRS system allows these slides and stream/river bank stabilizations to be completed at a fraction of the cost and a construction schedule measured in days instead of weeks!

Mr. Hanks has over 20 years of experience operating in the environmental contracting field. His experience includes overseeing soil remediation projects, underground storage tank removal and remediation, installation and repairs to leachate collection systems, final capping and closure of landfills, and grade and drain projects. Mr. Hanks has also lead tpmgroup into being a full service environmental contractor that includes a fleet of vac-trucks, roll-off trucks, and earth moving heavy equipment, in addition to operating facilities for the proper treatment and disposal of liquid wastes and a recycling center.

Jill Johnson Chief Financial Officer

Mrs. Johnson has a Bachelors of Accounting (BACC) from the Mississippi State University. She has 20 years of experience as a tax specialist and accountant for several multi-disciplinary corporations focused on the management of corporate assets. She has worked extensively with clients directing and overseeing their tax and investment portfolios, Individual, Corporate, Estate and Fiduciary tax returns and the preparation of GAAP and OCBOA Financial Statements. Mrs. Johnson has several years of experience with engineering, environmental, and geologic consulting firms. She has proven herself to be an experienced and competent financial manager and honed her skills by overhauling and resurrecting the delinquent tax status of several companies. Additionally, Mrs. Johnson has experience focused on agriculture, forestry and government.

Jimmy Gardner, Operations Manager

Mr. Gardner is a graduate of Western Kentucky University with a B.S. in Industrial Technology. He has 15 years’ experience in project management most of which has been in the construction industry. Mr. Gardner has been involved in land development projects in which he coordinated the design and layout of grading plans, street layout and utility placement. He has been heavily involved with day to day site work activities ensuring that aims, goals and objectives are accomplished in accordance with outlined priorities, time limitations, and funding conditions. Mr. Gardner has over 20 years’ experience in drafting schematic drawings for both the construction and manufacturing industries. At tpmgroup Inc. Mr. Gardner prepares and assists in the preparation of Site Investigation Reports, Intermediate Site Investigation Reports and Closure Assessment Reports. Mr. Gardner works closely with staff geologist to produce potentiometric ground water surface maps, borehole logs and geologic cross sections of environmental contaminated sites. Mr. Gardner is responsible for coordinating all operations of tpmgroup.

Gabe Weaver, P.E., Engineer

Mr. Weaver has a M.B. A. from the University of Louisville and a B.S. in Civil Engineering from the University of Kentucky. He began his career working for the Commonwealth of Kentucky’s Transportation Cabinet where he supervised grade and drain projects, designed grade and drain projects for bridge and roadway construction. He has over 10 years in the civil engineering field where he has become a leader in erosion engineering. He has worked in the design and testing of new products for erosion control and has worked with a vast number of clients in implementing procedures and design for erosion control. Mr. Weaver has worked in developing standards and practices for ECTC and ASTM.

Mike Hull, Sr. Project Manager – Aviation Fueling

Mike has more than 30 years of experience repairing, servicing and installing aboveground andunderground fuel tanks, piping and dispenser equipment. As a certified installer, he has worked at numerous GA airports in Kentucky and has become the preferred contractor for Averitt Express, installing and serving fuel systems at 26 different service centers throughout the Southeast. He was also heavily involved in the east fuel facility design-build project at the Blue Grass Airport installed in 2011.

Raymond Hagan, SR Project Manager

Mr. Hagan is a student at Western Kentucky University majoring in Civil Engineering. He has 10 years experience in project supervision and management in the construction industry. Mr. Hagan has been involved in land development projects in which he coordinated the design and layout of grading plans, street layout and utility placement. He has been heavily involved with day to day site work activities ensuring that aims, goals and objectives are accomplished in accordance with outlined priorities, time limitations, and funding conditions. Terry Breshears, P.G. (TN), Environmental Specialist

Mr. Breshears has a B.S. in Geology and Physics from Middle Tennessee State University and M.S. in Geology from University of Missouri-Columbia. Mr. Breshears has 15 years of experience with UST site investigation and remediation. He has worked extensively with RCRA, CERCLA and Phase I, II & III site assessments. Mr. Breshears has 20 years of experience with engineering, environmental, materials testing and geologic consulting firms. He is an experienced driller in exploration, site investigation, monitoring & recovery. Additionally, Mr. Breshears has 8 years of experience in manufacturing facilities.

Tim Slattery, Hydrologist, P.G. (TN)

Mr. Slattery has BS (1986) and MS (1994) Degrees in Geography from Western Kentucky University. His graduate work focused on environmental problems associated with karst terranes. Mr. Slattery has over 21 years experience in the environmental field. His experience includes: Phase I, II and III environmental site assessments/remediation, emergency response, waste management, UST compliance, regulatory compliance, erosion prevention and sediment control, karst hydrogeologic inventories, dye tracing, and operations management. He is a certified professional geologist in Tennessee (TN 3697), a Certified Professional in Storm Water Quality (CPSWQ #0168), Kentucky Erosion Prevention and Sediment Control certified (KEPSC # 120200836), and a Certified Landfill Manager in Kentucky. Mr. Slattery is a Desert Storm veteran having served with the 5th Battalion, 62d Air Defense Artillery for 5 years, 9 months (1986 to 1992). Mr. Slattery will be heavily evolved with work related to this RFQ, in some instance 30-40 hours per week could be devoted to projects listed in this RFQ

Jeff H. Jones, Federal Services

Mr. Jones has over 25 years of Business, Construction and Agriculture management and marketing experience. Experience includes market evaluations, development and implementation of marketing and business plans, cost estimation, and proposal preparation. Additional marketing experience includes proposal presentations and developing and maintaining strong client relationships.

Management experience encompasses all phases of project and administrative management activities. Project Management experiences includes responsibilities for completion of project up to ten million dollars. Administrative activities have included employee supervision, recruiting and hiring, serving as Owner/President for startup environmental/construction firm employing over 65 employees.

Extensive experience in “Start Up” enterprises including new business startups and new market penetration. This experience includes the construction, agriculture and aquaculture industries. SHEA STILES, Project Manager

Ms. Stiles graduated Magna Cum Laude from Oglethorpe University in Atlanta, GA in 1992 with a B.A . in Psychology and a minor in Sociology. Shea Stiles has 22 years of experience in the environmental field. She started with Laidlaw Environmental Services in 1993 which was acquired by Safety-Kleen Corporation where she was employed until 2012. She worked for two oil companies before her employment with tpmgroup Group, Inc. Her major focus at tpmgroup is industrial waste management. She is heavily involved with developing and sustaining strong customer service relationships. She has been involved with Project Management since 2006.

Clarke Willey, Business Development, Landscape Architect

Mr. Willey has a B.S. from The University of Massachusetts in Landscape Architecture and Regional Planning. He has 12 years of Civil Site Project Management with a focus on Commercial, Residential, Transportation Facility Development and Planned Community Development. He has also been employed as a Land Development Project manager for a production home builder. Throughout his career he has chosen to focus on Hydraulics, Wetlands and Erosion and Sediment Control. Additionally he has 15 Year of Sales and Business Development experience and has acted as a general contractor and owned a small contracting business.

Lisa Peterson, Accounting Manager

Mrs. Peterson has an Associates Degree in Accounting from Madison Tech located in Madison Wisconsin and a B.S. in Accounting from Kaplan University located in Dallas Texas. Mrs. Peterson has 16 years of experience performing a multiple roles focused on education accounting and departmental financial management of several service oriented companies throughout the central United States. She has worked extensively with companies which perform service work with a focus on consulting. She has performed many roles in her professional development performing as a staff accountant, financial manager, human resources and trainer within industry segments in field of banking, medical and non-profit.Mrs. Petersons day to day focus is assisting with the growth and stability of TPM by providing accounts receivable and payable supervision, analytical and relocation oversight and internal allocation and protocol of human resource management.

Aaron Bagley, Project Manager

Aaron Bagley has been involved in the environmental remediation and emergency response field for almost 10 years. Mr. Bagley has served as project manager on projects ranging from small fuel releases, to large scale hazardous materials incidents. He is a specialist at; waste profiling and manifesting, hazardous waste transportation and disposal, handling hazardous waste and removal of bio-hazards as well as Emergency Response. Aaron attended Tusculum College in Greeneville, TN in 2002. His focus was Elementary Education. He currently holds many certifications in the environmental field, including 80-Hour HAZMAT Technician (HMTO). Mr. Bagley is also currently a certified OSHA instructor for several courses including 40-Hour HAZWOPER, 24-Hour Technician, 8-Hour Supervisor and 8-Hour Incident Commander.
