Storia e filosofia
Lo stelo Revision nasce dall’esperien-
za del sistema LCR: differenti disegni
di stelo a sezione circolare, accop-
piabili meccanicamente ad una serie
di colli modulari fissati mediante cono
Morse e vite di sicurezza.
La protesi da revisione non cemen-
tata LCR (1982) ha dimostrato come
la modularità sia efficace nei casi di
re-intervento.
Dal punto di vista clinico, l’impiego
di una sezione circolare alettata per
lo stelo conico, ha dimostrato ottimi
risultati nel successo delle protesi da
revisione di tipo Wagner.
Tale riscontro, ampliamente riportato
in letteratura, e l’esperienza maturata
con la protesi LCR, hanno condotto,
dalla fine del 1994, allo studio di una
nuova protesi femorale da reimpianto
denominata Revision.
La prima posa dell’impianto risale al
1996.
Partendo dall’esperienza della protesi
di tipo Wagner, il progetto dello stelo
Revision, si caratterizza per la ricerca
di un’adeguata soluzione modulare
in grado di superare i limiti chirurgici
e risolvere quelli funzionali, tipici del
disegno monolitico.
Nello stelo Revision la tecnica ope-
ratoria è analoga a quella di una
tradizionale protesi da revisione non
cementata di tipo Wagner, tuttavia
l’approccio in due tempi e la mo-
dularità, la rendono estremamente
The Revision stem is the result of
the experience gained with the LCR
system: different round section stem
designs, mechanically connectable
to a series of modular necks, fixed by
Morse taper and safety screw.
The LCR (1982) cementless revi-
sion prosthesis demonstrated the
efficiency of the modular design in
revision cases.
From the clinical point of view, the
use of a tapered stem with round
finned section gave very good results
with the success of Wagner revision
prostheses.
These positive results, widely con-
firmed by the literature, as well as
the experience gained with LCR
prosthesis, lead to the study of a new
femoral prosthesis for reimplantation
called Revision.
The first application of the implant
dates back to 1996.
Starting from the experience acquired
with Wagner type prostheses, the
Revision stem project was character-
ised by the search for an appropriate
modular solution to overcome the
surgical and functional limits typical
of the monolithic design.
The surgical technique applied with
the Revision stem is similar to that
used with a traditional Wagner ce-
mentless revision prosthesis. How-
ever, the two-time approach and
the stem modular design make it

extremely flexible: the primary fixa-
tion objective is separated from the
restoration of the articular function
and can be easily obtained without
the risk of precluding or modifying the
final articular anatomical frame, thanks
to the availability of proximal modules
of different heights.
The flexibility of the implant is also sup-
ported by the search for overall reliability.
Particular attention was paid to the
design of the coupling taper.
Different approaches were adopted.
The anatomical aspect was analysed
to allow the implant adaptation to the
configuration normally taken by the
femur with respect to the lateromedial
plane, thus making the Revision stem
unique in its gender, though having a
straight design.
The structural mechanical behaviour
was calculated and simulated in a
virtual environment (finite element
analysis). Severe experimental labora-
tory tests were carried out to check the
dynamic performance in the worst load
and posture conditions, to guarantee
the reliability of the coupling in time.
The technological know-how acquired
in the metallurgical field allowed Lima
Lto Spa to develop and industrialise a
specific heat treatment for the Ti6Al4V
titanium alloy which increases the alloy
general mechanical performance and
in particular its resistance to fatigue,
flessibile: l’obiettivo della fissazione
primaria è separato da quello di ripri-
stino della funzione articolare, e può
essere agevolmente raggiunto senza
il rischio di precludere, o alterare a
priori, il quadro anatomico articolare
finale grazie alla disponibilità di moduli
prossimali in differente altezza.
Alla flessibilità d’impianto si affianca la
ricerca dell’affidabilità totale.
Particolare attenzione è stata posta al
disegno del cono d’innesto.
Diversi gli approcci in questo senso.
L’aspetto anatomico è stato analizzato
al fine di consentire l’adattamento
dell’impianto alla configurazione nor-
malmente assunta dal femore rispetto
al piano latero-mediale, rendendo
unico nel suo genere lo stelo Revision
sebbene di disegno retto.
Il comportamento meccanico strutturale,
è stato calcolato e simulato in ambiente
virtuale (analisi agli elementi finiti). Rigide
prove sperimentali di laboratorio sono
state eseguite per verificare le prestazioni
dinamiche nelle condizioni peggiori di ca-
rico e postura, a garanzia dell’affidabilità
dell’accoppiamento nel tempo.
Il patrimonio di conoscenze tecnologi-
che acquisito in campo metallurgico ha
permesso a Lima Lto Spa di sviluppare
ed industrializzare nel corso degli anni
un trattamento termico specifico per la
lega di Titanio, Ti6Al4V, in grado di in-
crementare le prestazioni meccaniche

increasing it by 20% with respect to
that of the alloy in its starting condi-
tions.
The mechanical machining of the
tapered surfaces is also critical as
it determines the roughness, a pa-
rameter which affects the tribological
behaviour.
In the Revision stem, the typical
roughness of the tapers prevents the
onset of corrosion due to mechanical
reasons.
The number and the configuration of the
components supplied intraoperatively,
with the support of a simple and efficient
instrument set, allow the operator to
easily manage surgical-related limits, by
dividing the femoral operating time into
two segments, one dedicated to the
choice of the stem and the other to the
choice of the articular portion, always
having the possibility of checking the
best overall dimensions.
The flexibility of the surgical act guides
the operator with safety during the
operation phases and the implant
modular design widens the treatable
morphological range, allowing a bal-
anced recovery of articular motions.
The restoration of a correct bio-
mechanics associated to optimum
primary stability are possible in all the
revision cases presenting consider-
able loss of bone stock and alterations
of the proximal anatomical frame.

La lavorazione meccanica delle superfici
coniche è altrettanto critica in quanto ne
determina la rugosità, parametro influen-
zante il comportamento tribologico.
Nello stelo Revision la rugosità ca-
ratteristica dei coni previene l’insor-
genza di fenomeni corrosivi d’origine
meccanica.
Il numero e la configurazione dei com-
ponenti forniti intra-operatoriamente,
con il supporto di un semplice ed
efficacie strumentario permettono
all’operatore di gestire agevolmente i
limiti chirurgici, dividendo il tempo fe-
morale in due segmenti, uno dedicato
alla scelta dello stelo, ed uno alla scelta
della porzione articolare, in ognuno
dei quali è sempre possibile verificare
separatamente gli ingombri ideali.
La flessibilità dell’atto chirurgico guida
l’operatore con sicurezza nelle fasi d’in-
tervento, e la modularità dell’impianto
allarga lo spettro morfologico trattabile,
rendendo possibile un recupero equili-
brato dei rapporti articolari.
L’opportunità di ripristinare una corretta
bio-meccanica associata a un’ottima
stabilità primaria, sono offerte per tutti
i casi di revisione presentano importanti
perdite di sostanza ossea ed alterazioni
del quadro anatomico prossimale.

Stem, neck and screw are manufac-
tured in Ti6Al4 titanium alloy, accord-
ing to severe production criteria and
improved by the high level technologi-
cal know-how of LimaLto.
Thanks to a preliminary phase of drop
forging of the neck and a special
thermal treatment of all the compo-
nents, it is possible to obtain the high
mechanical performance required by
the modular stem.
The subsequent mechanical machin-
ing by means of modern numerical
control operation centres completes
the final geometry that characterises
the general biomechanical behaviour
of the implant.
The neck and stem external surfaces
feature a macro-roughened finishing
obtained by projecting a controlled
flow of corundum particles.
Pressure, speed and granulometry of
the projected material make it possible
to obtain an even and calibrated sur-
face texture with biologically appropri-
ate roughness values (Ra 3÷4μm) in
order to support and stimulate the
osteointegration.
The neck portion not involved in
the osteointegration has a polished
finishing to reduce the polyethylene
debris formation in case of acci-
dental contact with the acetabular
component.
Stelo, collo e vite sono realizzati in
lega di Titanio Ti6Al4V, secondo
criteri produttivi severi, ed affinati
dall’elevato know-how tecnologico
di LimaLto.
Una prima fase di foggiatura a caldo
per il collo ed uno specifico tratta-
mento termico per tutti i componenti,
permettono di ottenere le elevate
prestazioni meccaniche richieste allo
stelo modulare.
La successiva lavorazione meccanica
mediante moderni centri di lavoro a
controllo numerico, completa la geo-
metria finale, caratterizzante il com-
portamento bio-meccanico generale
dell’impianto.
Le superfici esterne di collo e stelo pre-
sentano finitura macrorugosa ottenuta
attraverso la proiezione di un flusso
controllato di particelle di corindone.
Pressione, velocità e granulometria
del getto permettono di ottenere
un’uniforme e calibrata tessitura della
superficie a valori di rugosità biologi-
camente adeguati (Ra 3÷4μm ) per lo
stimolo ed il supporto all’integrazione
ossea.
La porzione del collo non destinata
all’integrazione ossea è lucida, per
ridurre la probabilità di formazione
di particolato di polietilene nell’acci-
dentale contatto con la componente
acetabolare.

The modular system is characterised
by the mechanical coupling of the
proximal module (neck) to the stem
by means of a “Morse” taper stabilised
during the implantation phase by a
safety screw.
When the load is applied, the coupling
self-locking feature is more than suf-
ficient to ensure the complete immo-
bility in time of the tapered coupling.
However, the accidental interposition
of foreign bodies or the stagnation of
organic fluids during the implantation
may hinder the final result.
The safety screw is necessary to
ensure intraoperatively the coupling
of components. To avoid the loos-
ening of the screw, which may be
caused by the vibrations produced
while walking, a self-locking device
is provided.
The stem has a 1°50’ tapered pro-
file, with round finned section and
typical diameters ranging from 14 to
24mm.
The diameter increases by 2mm.
Four landmarks are visible on the stem
larger base, below the coupling taper,
indicating the main quadrants resulting
from the ideal intersection between fron-
tal and sagittal planes, and necessary
for the selection of the stem position.
The neck, available in 7 different
heights, is designed with a single 135°
cervico-diaphyseal angle configura-
tion, with a resulting offset of 35mm.
Il sistema modulare è caratterizzato
per l’accoppiamento meccanico del
modulo prossimale (collo) allo stelo,
tramite cono di ‘Morse’, stabilizzato
nella fase d’impianto da una vite di
sicurezza.
A carico applicato, la natura autobloc-
cante è più che sufficiente a garantire
all’accoppiamento conico la totale
immobilità nel tempo, tuttavia l’acci-
dentale frapposizione di corpi estranei
o il ristagno di fluidi organici durante la
manovra d’impianto, può pregiudicare
il risultato finale.
La vite di sicurezza si rende necessaria
alfine di assicurare intra-operatoria-
mente l’incastro dei componenti. Per
evitare che con il passo le vibrazioni
possano nel tempo causare il rilascio
della vite, la stessa è dotata a sua volta
di un dispositivo autobloccante.
Lo stelo ha un profilo di 1°50’ di co-
nicità, con sezione circolare alettata
di diametri caratteristici da 14mm a
24mm.
L’incremento sul diametro è di 2mm.
Alla base maggiore dello stelo, sotto il
cono d’innesto, sono visibili in risalto quat-
tro reperi ad indicare i quadranti principali
risultanti dall’intersezione ideale tra piano
frontale e sagittale, e necessari a guidare
la scelta della posizione dello stesso.
Il collo, disponibile in 7 altezze, è
disegnato in un’unica configurazione
d’angolo cervico diafisario, 135°, per
offeset risultante di 35mm.

However, the orientation of the cou-
pling taper with respect to the ana-
tomical reference planes allows a vari-
ation between the 31 mm minimum
value (131° varus) and the 39mm
maximum value (239° valgus).

The stem geometry and surface
finishing guarantee an excellent
primary stability as well as an even
and permanent secondary oste-
ointegration.
The stem and modular neck axes
are not parallel: they cross in a point
at the base of the coupling taper,
determining a 4° angle. This feature
can be used in any plane common to
the direction of the ideal diaphyseal
axis thanks to the total freedom in
choosing the degree of neck antiver-
sion allowing the implant adaptation
to the femoral morphology and the
proximal anatomical frame, in order
to attain an appropriate recovery of
the articular motions.
There are two main ways to use the
implant modules:
- given a 135° neck cervico-diaphy-
seal angle, this can be changed within
the range between 131° varus and
139° valgus;
- depending on the stem length, either
long (200mm) or short (140mm), and
the approach (transfemoral or not trans-
femoral), the taper position can follow
femoral antecurvatum or the procurva-
tum, adjusting the mutual positions of the
two components to the proximal femoral
morphology in the sagittal plane.
The stem tapered design ensures
the required distal coupling while the
round finned section ensures a high
rotational stability.
Geometria dello stelo e finitura su-
perficiale garantiscono un’eccellente
stabilità primaria per il raggiungimento
di un’uniforme e duratura osteointe-
grazione secondaria.
Gli assi di stelo e collo modulare non
sono paralleli tra loro: intersecano in
un punto alla base del cono d’accop-
piamento, determinando un angolo di
4°. Questa caratteristica può essere
sfruttata in qualsiasi piano comune alla
direzione dell’ideale asse diafisario gra-
zie alla totale libertà di scelta del grado
d’antiversione per il collo, permettendo
l’adattamento dell’impianto alla morfo-
logia femorale e del quadro anatomico
prossimale, al fine di un adeguato
recupero dei rapporti articolari.
Sono due le principali modalità di
gestione dei moduli d’impianto:
- dato l’angolo cervico diafisario del
collo di 135°,questo può variare tra
tutti i valori compresi fra gli estremi di
131° in varo e di 139° in valgo.
- in funzione della lunghezza dello stelo,
sia lungo 200mm, o sia corto 140mm,
e dell’approccio (via transfemorale, o
cielo osseo chiuso), la posizione del cono
può seguire l’antecurvato o il procurvato
femorale, adeguando le posizioni mutue
dei due componenti alla morfologia fe-
morale prossimale nel piano sagittale.
Nello stelo, il disegno conico assicu-
ra il desiderato incastro distale e la
sezione circolare alettata garantisce
un’elevata stabilità rotazionale.

To improve the primary stability the
implant angular design - which, as
already said, can approximate the
diaphyseal curvilinear axis to two
concurrent straight lines - determines,
in its turn, a constraint reaction, a
further contact point to balance the
axial load.
Thanks to the experience in Wagner
applications, the particular combina-
tion of diameters and lengths has
been studied to ensure an adequate
width of the distal portion in contact
with the diaphysis.
The conformation of the longitudinal
fins evenly distributes the load and
stimulates the endosteal tissue, fa-
vouring the reconstruction of the lost
bone stock.
The neck metaphysis is adequately
rounded for the anatomical adaptation
of the prosthesis to the calcar area.
The distal end of the stem is tapered
in order to avoid any anomalous peak
stress.
The reduction in length of the 12/14
coupling tapers makes it possible to
increase the range of motion, which in
the case of couplings with a cephalic
diameter of 36mm is close to 150°.

che come detto può approssimare
l’asse curvilineo diafisario a due rette
concorrenti, determina a sua volta
reazione vincolare, un ulteriore punto
di contatto a contrastare il carico
assiale.
Alla luce dell’esperienza di Wagner, la
particolare combinazione di diametri e
lunghezze, è studiata per garantire sem-
pre un’adeguata ampiezza della porzio-
ne distale in contatto con la diafisi.
La conformazione delle alette longi-
tudinali ripartisce il carico uniforme-
mente e stimola il tessuto endosteale,
favorendo la ricostituzione del patri-
monio osseo perso.
La metafisi del collo, è adeguatamente
arrotondata per adattare anatomi-
camente la protesi alla regione del
calcar.
L’estremità distale dello stelo è ra-
stremata, ad evitare anomali coni di
pressione.
La riduzione in lunghezza nel cono
d’accoppiamento 12/14, permette di
incrementare l’escursione articolare,
che per accoppiamenti di diametro
cefalico 36mm è prossima a 150°.

The stems are available in 12 sizes:
6 diameters from 14mm to 24mm,
with sizes varying by 2mm; two
lengths are available for each diam-
eter, 140mm and 200mm.
The diameter is always calculated
making reference to a distance of
140mm from the distal end.
The necks are available in 7 sizes from
50mm to 110mm of height (measured
from the base of the trunk at medium
level from the rotation centre), with
sizes varying by 10mm.
By coupling the two modules, it is
possible to obtain 14 implant lengths
for each diameter, from a minimum
of 190mm (stem 140+neck 50) to a
maximum of 310mm.
The instrument set guides the opera-
tor in the choice, simulation and cor-
rection of the overall dimensions.
Using the 19 modules it is possible to
obtain 84 different implants, thus hav-
ing a wide range of solutions for the an-
atomical adaptation of the prosthesis
to the hosting femur and an adequate
recovery of articular motions.
Gli steli sono disponibili in 12 taglie:
6 diametri da 14 mm a 24mm, con
incremento di 2mm; per ogni diametro
sono previste due lunghezze, 140mm

La misura del diametro è sempre valu-
tata in corrispondenza alla distanza di
140mm dall’estremità distale.

La strumentazione guida l’operatore
nello stabilire, simulare e correggere
gli ingombri.
Mediante i 19 moduli si possono
comporre 84 diversi impianti, offren-
do un ampio spettro di soluzioni per
l’adattamento anatomico della protesi
al femore ospitante ed un adeguato
recupero dei rapporti articolari.
