Overview of Scope:
It is the aim of this Lifting Plan to define and describe how
the equipment and safe procedures are to be employed to complete
lifting operations in accordance with Legislation and Approved
Codes of Practice;

Safe Lifting Considerations:
Visual inspection must be completed of the lifting accessories
and equipment prior to use ensure minimum SWL is correct and
equipment supplied for use is as per Lifting Accessories &
Equipment above;
Confirm that the SWL of the lifting accessories and equipment
takes account of both the weight and the mode of use [i.e.
downgrade the SWL where chokes are used and number of legs in
use];
Visual inspection must be completed of the lifting points prior
to lifting to ensure they are not damaged;
Protect lifting accessories where applicable to ensure they are
not damaged by sharp edges and free to align correctly;
Ensure the load will stay together during lifting operations,
and all loose materials removed from equipment prior to lift;
Test lift and landing of the load to be completed, to check
balance, stability and bite of accessories and equipment;
Tag line to be used to assist in the control of the load NEVER
wrap the tag line around your hand or wrist and ensure it is free
from obstructions when hoisting;
Keep hands, fingers and limbs free from the lifting accessories
and choke points during hoisting;
Exclusion zone required around lifting and loading areas. No
lifting over the head of personnel, clear a safe lifting path
across the site;
Ensure the landing site is clear and suitable prior to
commencing lift;
Ensure the load is stable when lowered and landed, prior to
releasing the weight and lowering fully. Ensure lifting accessories
are not trapped when landed and are free for removal;
No chandelier lifts are to be completed at any time;

No operations will take place in wind speeds over 9.8 metres per
second;
No operations will take place in wind speeds over the lifting
appliance manufacturers guidance on wind speed; Lifting operations
must stop during storms, or when there is a risk of lightning
strikes;
NOTE: all lifting operations will be assessed at the time of
operations by the Excavator Operator, Crane Supervisor and / or
Slinger Signaller;
NOTE: local conditions and the safety of personnel may dictate
that the Excavator Operator, Crane Supervisor and / or Slinger
Signaller have to stop lifting operations at a lower wind speed
than specified above;
Initial Conditions
[NOTE: This is a GENERIC overview and must be made SITE SPECIFIC
and needs to read through and altered as required DELETE this cell
prior to submission]
1. The working area must be cleared to allow the Excavator, low
loader delivery and lifting operations to take place in a safe
manner without endangering miscellaneous personnel, not directly
involved in the lifting operations;
2. All personnel not directly concerned with the lift must keep
out of the lift appliance operating areas. The working area should
be cordoned off or otherwise secured against personnel entry;
3. The Appointed Person or the Crane Supervisor [where briefed
by the AP previously] shall make himself known to all personnel and
conduct a toolbox talk on the Lifting Plan and Methods prior to
commencing operations;
4. Under no circumstances shall the Excavator be used for
lifting purposes when not under the instruction of the Appointed
Person or Crane Supervisor;
5. All Excavators left unattended shall be left in a safe
condition with no load or lifting equipment on the Quickhitch. The
Excavator will be shut down and isolated as per manufacturers
instructions, secured and locked. The keys WILL be removed from the
equipment when exiting the working cab;
Lift Procedure

1. Excavator together with associated labour will arrive on site
at an agreed date and time for induction and to receive toolbox
talk communication of Lifting Method Statement, Lifting Plan and
Risk Assessment;
2. Excavator to track into position under the control of Slinger
/ Signaller as required for the lift to be completed.
3. Daily checks as required and applicable to Excavator will be
completed at the start of each shift to ensure the Excavator is fit
for purpose and in good working order. Any issues will be
highlighted and reported to Plant Owner for completion of
maintenance;
4. On completion of checks, and when in position for lifting, as
directed by Slinger / Signaller, the Excavator operator will lower
the Quickhitch to ground level, allowing access to the lifting eye
and turn off the engine / apply the dead mans handle so the
excavator cannot move / operate whilst the Slinger / Signaller is
around the Quickhitch;
5. Lifting tackle for executing lift will be connected to the
lifting eye using appropriately sized shackle, checked for
condition prior to work by Slinger / Signaller. Any defects will be
noted and equipment not used if defects found. A record of checks
will be formally recorded and completed weekly by Slinger /
Signaller
6. The Quickhitch of the Excavator will be hoisted into the air.
The Quickhitch lifting eye is designed for straight lifts only,
therefore the Quickhitch must be crowded at the appropriate angle
to achieve this [i.e. adjustment required during lifts to ensure
this achieved during radii changes;
7. The Excavator will now be slewed over item to be lifted and
lifting tackle attached to load as set out and briefed in the LP or
Schedule of Common Lifts;
8. Slinger / Signaller will now instruct operator by hands
signals to slowly hoist up to take the slack out of the tackle;
9. Slinger / Signaller will now plumb the Quickhitch of the
Excavator vertically above lift;
10. Tag lines will now be attached to the lift as required.
Gloves will be worn at all times when using tag lines. Tag line
will NOT be wrapped around wrists or limbs to assist control or
stability;
11. Slinger / Signaller will now instruct Excavator operator
using hand signals to hoist item just clear of transport / ground
to complete a test lift and land;
12. Slinger / Signaller will check for correct slinging and
adjust if necessary;
13. Once all checks completed and happy to proceed, hoist as
required until clear of obstructions. Slinger / Signaller will
instruct the Excavator operator to lift load to required
position;
14. Slinger / Signaller will guide load to required landing area
and instruct Excavator operator to lower lifted item as
required;
15. Once landed, position checked and placed, Excavator operator
will be instructed to lower off all weight of lifted item;
16. Lifting tackle will be released from item and Excavator
operator instructed to hoist up until lifting equipment is clear of
all obstructions;
17. Slinger / Signaller will now instruct Excavator Operator to
slew Excavator back over delivery area and to lower down over the
next item to be lifted;
18. Operations 9 to 17 will now be repeated as required until
all items have been lifted and placed as required;
19. Once all lifting has been completed Excavator operator to
lower Quickhitch to ground and lifting tackle removed from hook and
stowed away;
Variations to Lifting Plan
Any and all significant variation to this Lifting Plan &
Method Statement, including drawings must be notified to the
Appointed Person before proceeding with the lift. The Crane
Supervisor must note details of all variations together with the
comments of the Appointed Person and recorded on the change
log;

Variations Change Log
Any minor changes to the works, obstructions & restrictions
should be noted where these occur. All minor changes should be
reviewed by the Authorised Person and discussed with the Crane
Supervisor [where applicable], Slinger Signaller and Excavator
Operator and signed off by all persons involved in the lift before
proceeding with works. A record of communication must be completed
after each change noted.
Any significant changes require review, revision and re-issue of
the Lifting Plan prior to starting works [e.g. Lifting Appliance
supplied not item Scheduled i.e. due to breakdown or hire
issues];
I have reviewed and checked the above change log items, and
confirm that to the best of my knowledge Excavator is capable of
carrying out the lifts described and within the limits
specified.
