EDTA ve %2.5’luk sodyum hipoklorit ve steril serum fizyolojik ile her enstrüman
arasında irrige edildi. Paper-pointlerle kurutularak kök kanal dolgusuna hazır hale gelen
kanallar rasgele seçilerek her grupta 10’ar diş olacak şekilde 6 gruba ayrıldı:

Grup 2: Guta-perka ve AhPlus kullanarak System-B
Grup 3: Guta-perka ve AhPlus kullanarak Obtura II
Grup 4: Resilon ve Epiphany kullanarak lateral kondensasyon
Grup 5: Resilon ve Epiphany kullanarak System-B
Grup 6: Resilon ve Epiphany kullanarak Obtura II ile dolduruldu.
Tüm köklerden radyograf alınarak lateral kanalların dolgu durumları kaydedildi.
Doğal ve yapay olarak oluşturulan lateral kanalların tıkamasının değerlendirilmesi için
kökler %5’lik nitrik asitle 72 saat dekalsifiye edildi. Kökler %80, 90 ve 99’luk alkol
serilerinden sırasıyla 12 saat, 1 saat ve 3 saat geçirilerek dehidrate edildi ve metil
salisilat ile şeffaflaştırıldı. Şeffaflaştırılan köklerden stereomikroskoba (Olympus BX50,
Tokyo, Japonya) bağlı olan dijital kamera (Olympus DP 70, Tokyo, Japonya) ile ×10 ve
×30 büyütmede görüntüler alındı. Değerlendirme apikal, orta ve koronal üçlülerde ayrı
ayrı yapıldı. Lateral kanalların tamamı boş ise “0” ile, parsiyel olarak pat ile dolmuşsa
“1” ile, tamamı pat ile dolmuşsa “2” ile, parsiyel siman ve dolgu maddesi ile dolmuşsa
“3” ile ve tamamı dolgu maddesiyle dolmuşsa “4” ile skorlandı. Elde edilen veriler
Kruskall-Wallis H-testi ve Mann-Whitney U-testi ile analiz edildi. Ayrıca, bulgular
“kabul edilebilir” ve “kabul edilemez” şeklinde kategorize edilerek, değişkenlerin
karşılaştırılmasında Ki-kare analizi kullanıldı. Lateral kanalların dolmasını etkileyen
faktörlerin çoklu analizinde lojistik regresyon analizi kullanıldı.

4. BULGULAR
4. 1. Sodyum Hipoklorit ve Klorheksidinin Resilon Konlar Üzerindeki
Etkisinin Değerlendirilmesi: Atomik Kuvvet Mikroskop Çalışması
NaOCl ve CHX gruplarının 1 ve 5 dakikadaki üç boyutlu AFM görüntüleri Şekil
4.1.1’de gösterilmektedir. Bu görüntüler örneklerin yüzey yapılarını ve yüzey
pürüzlülüğünü ortaya koymaktadır. Bu görüntülerde kontrol grubuyla
karşılaştırıldığında hem NaOCl hem de CHX’in 5 dakikada Resilon kon yapısında
düzleştirici bir etkiye neden olduğu görülmektedir. Kontakt mod görüntülerinin RMS
ortalama değerleri Çizelge 4.1.1’de görülmekte ve Şekil 4.1.2’de grafikle
özetlenmektedir. Kontrol grubu, 1 dakika NaOCl grubu ve 1 dakika CHX grubu
arasında istatistiksel olarak anlamlı bir fark bulunmamıştır. 5 dakika NaOCl ve 5 dakika
CHX gruplarının RMS değerleri kontrol grubundan anlamlı derecede düşüktür (p<0.05).
5 dakika NaOCl grubu diğer tüm gruplardan anlamlı derecede düşüktür (p<0.05).

Grup 1: Yapılan SEM incelemesi sonucu lateral kondensasyon ile doldurulan
10 örneğin 6’sında tübül penetrasyonu gözlenmezken, 3 örnekte birkaç tübülde sınırlı
penetrasyon gözlendi. Örneklerden bir tanesi analiz için uygun bulunmadı. Bütün
örneklerde dentin duvarına yapışmış pat ve tıkalı tübüller görüldü. Pat kalınlığının
oldukça değişken olduğu yer yer kalınlaşmaların olduğu görüldü (Şekil 4.3.1). Şekil
4.3.2’de soğuk lateral kondenzasyonla sıkıştırılan Resilon kon yüzeyinde bozulma
olmadığı görülmektedir. Yüzeye yapışan globüler yapıda pat artıklarının yanı sıra
yüzeyden kopan gevşek bağlı plaka şekilli doldurucular da görülmektedir.

Grup 2:Yapılan SEM incelemesi sonucu Obtura II ile doldurulan 10 örneğin
3’ünde penetrasyon gözlenmezken, 7 örnekte penetrasyon varlığı görüldü. Örneklerde
pat bazı bölgelerde dentin duvarlarında bazı bölgelerde dolgu yüzeyinde gözlenmekte
olup, pat kalınlığının oldukça az ve homojen olduğu görüldü.

Grup 3: Yapılan SEM incelemesi sonucu System B kullanılarak sıcak vertikal
kondensasyon ile doldurulan 10 örneğin yalnızca birinde dentin penetrasyonu
görülmezken, 8 örnekte penetrasyon gözlenmiştir. Örneklerden birinin analize uygun

olmaması nedeniyle değerlendirmenin dışında tutulmuştur. Tüm örneklerin dolgu
yüzeyinde derin penetrasyonu gösteren tübül replikasyonu olan deliller mevcuttur.
4. 4. Epiphany Sistemi İle Doldurulan Simüle Eğri Kanallarda Pat ve Dolgu
Dağılımının Değerlendirilmesi
Araştırmamızda, koronal üçlüde metodlar arası istatistiksel olarak anlamlı fark
görülürken (p<0.05), orta üçlü ve apikal üçlüde anlamlı farklılık görülmemiştir
(p>0.05). Bununla birlikte, yalnızca System B grubunda kesit alınan düzeyler arasında
fark görülmemiştir (Şekil 4.4.1). Obtura II ile doldurulan örneklerde apikal üçlüde
oldukça yüksek pat oranına rastlanmış, hem koronal hem de orta üçlü üçlüye göre
istatistiksel olarak anlamlı fark bulunmuştur (Şekil 4.4.2). Çizelge 4.4.1’de kullanılan
tekniğe göre koronal, orta üçlü ve apikal üçlüdeki dolgu oranları sunulmaktadır.

Resilon ve Epiphany patının pembemsi rengi ayırt edilmesini zorlaştırsa da,
tonlar arasındaki farklılık, alanların hesaplanmasını sağlamıştır. Lateral kondensasyon

grubunda koronal, orta üçlü ve apikal üçlülerde bazı kesitlerde Resilon konlar arasında
birleşme görülürken, bazı kesitlerde konları tek tek görebilmekteyiz (Şekil 4.4.3).
Obtura II grubunda koronal ve orta üçlü üçlülerde homojen bir Resilon yapısı ile
duvarlarda yok denecek kadar az miktarda pat varlığı görülmektedir. Bununla birlikte,
apikal üçlüde oldukça fazla örnekte yalnız pat varlığı görülmektedir. Obtura II grubunda
apikal üçlünün pat oranı, koronal ve orta üçlü üçlülerden istatistiksel olarak anlamlı
derecede yüksek bulunmuştur (p<0.05). System B grubunda ise düzeyler arası anlamlı
bir farka rastlanmamıştır (Çizelge 4.4.2). Çizelge 4.4.3, dolgu oranlarının istatistiksel
değerlendirme sonucu tekniklere ve düzeylere göre p değerlerini göstermektedir.

Kullanılan materyallerle lateral kanalların tıkanması karşılaştırıldığında koronal
ve orta üçlüde istatistiksel olarak anlamlı fark bulunmazken, apikal üçlüde guta-
perkanın Resilona göre lateral kanalları anlamlı derece daha iyi doldurduğu
bulunmuştur (p<0.05). Şekil 4.5.2’de grupların ortalama değerleri ve lateral kanalların
kullanılan materyalle olan ilişkisinin grafiği görülmektedir.

Obtura II yöntemi çoklu regresyon analizi sonuçlarına göre, yaygın olarak
kullanılan lateral kondensasyon yöntemine göre lateral kanalları doldurmada 0.44 kat
iyi bulunurken (p<0.05), System B yöntemi ise 6.98 kat iyi bulundu (p<0.05).
Kullanılan materyaller karşılaştırıldığında guta-perka esas alındığında Resilon 0.87 kat
iyi bulundu (p>0.05). Apikal üçlü esas alındığında, orta üçlüdeki lateral kanalların
doldurulma olasılığının 1.34 kat fazla (p>0.05), koronal üçlüdeki lateral kanalları
doldurma olasılığının 1.66 kez fazla olduğu bulundu (p>0.05). Çoklu regresyon analizi
sonuçları Çizelge 4.5.2’de sunulmaktadır.

Yapılan istatistiksel analizler sonucu apikal üçlüdeki lateral kanalların
doldurulabilmesinde guta-perkanın Resilona göre daha iyi olduğu, orta ve koronal
üçlüde kullanılan materyaller arasında fark olmadığı görüldü (Şekil 4.5.2). Şekil
4.5.2’de guta-perka ve Resilonun apikal, orta ve koronal üçlüde lateral kanalları
doldurma kapasitesinin ortalama değerleri sunulmaktadır.

çalışmalara ihtiyaç vardır. Böylece yüzey bozulmasına ve yapısal değişikliğe neden
olmadan etkili dezenfeksiyon elde edilebilir.
3) Kanal-içi düzensizliklerin doldurulmasında ve boşluk oluşumunda farklı
teknikler ve dolgu materyallerinin değerlendirildiği çalışmamızda, guta-perka ve
Resilonun boşluk oluşturma yönünden istatistiksel olarak anlamlı bir fark göstermediği
bulundu. Obtura II’nin istatistiksel olarak anlamlı derecede az boşluk oluşumuna neden
olduğu belirlendi. Geniş kanallarda dar kanallara göre, daha az boşluk oluşurken,
defekt replikasyonunda fark görülmemiştir.
4) Kanal-içi defektlerin replikasyonunda, kullanılan materyalin önemli olduğu
ve guta-perkanın defektleri daha iyi replike ettiği sonucuna varıldı. System-B’nin defekt
replikasyonunda Obtura II’den daha iyi olduğu saptandı.
5) Yapılan analizler sonucu, klinik olarak kanal içi düzensizliklerin
replikasyonunda kullanılan tekniğin önemli olduğu, kanal içi defektlerin
replikasyonunda System-B’nin daha iyi sonuçlar verdiği, Obtura II’nin daha az boşluk
oluşumuna neden olduğu sonucuna varıldı. Bu da System-B’nin daha fazla teknik
hassasiyet gerektiren bir yöntem olduğunu düşündürdü.
6) Resilon ve Epiphany’nin dentin tübül penetrasyonu ve kanal duvarına
adaptasyon gösterdiği ve tekniğe bağlı bazı farklılıkların olduğu belirlendi. SEM
görüntülerinde, System-B ısı taşıyıcı sistemle doldurulan kanalların dentin tübül
penetrasyonunun diğer tekniklere göre üstünlüğünü gösteren bulgular mevcuttur.
Resilonun ısıtılması ve vertikal basınçla kanal dolgusunun yapılması daha ileri
penetrasyon ve adaptasyon sağlamaktadır.

7) Resilon ve Epiphany ile doldurulan simüle eğri kanallı rezin bloklarda dolgu-
pat dağılımının değerlendirilmesi sonucunda, lateral kondensasyon grubunda en yüksek
pat oranına rastlanmıştır. System-B ve lateral kondensasyon teknikleri apikalden
koronale kadar oldukça homojen bir dolgu sağlarken, Obtura II apikal üçlüde yalnızca
pat bulunan buna rağmen orta ve koronal üçlüde oldukça az pat yüzdesine sahip bir
dolgu ortaya koymuştur. Sonuç olarak, Obtura II enjektör uçlarının çok ince olmaması
klinik olarak, apikal 3 mm’ye ulaşılmasını zorlaştırmakta ve böyle kanallarda
kullanılmasında bir kontrendikasyon oluşturmaktadır.
8) Lateral kanalları doldurmada, guta-perka ve Resilon koronal ve orta üçlülerde
benzer sonuçları verirken, apikal üçlüde guta-perka daha iyi tıkama göstermiştir.
System-B, tüm lateral kanallarda en iyi tıkamayı sağlarken, Obtura II ve lateral
kondensasyon yöntemleri arasında fark görülmemiştir. Klinik olarak oldukça önem arz
eden ve başarıyı etkileyen lateral kanalların doldurulmasında Resilon veya guta-perka
kullanımının fark yaratmadığı, buna rağmen kullanılan yöntemin önemli olduğu
sonucuna varıldı.

1978 yılında Şanlıurfa’da doğdu. İlköğrenimini Ankara’da, orta ve lise
öğrenimini Konya’da tamamladı. 1996 yılında Çukurova Üniversitesi’nde başladığı
mesleki eğitimini 2001 yılında bitirdi. 2003 yılında Ç. Ü. Sağlık Bilimleri Enstitüsü Diş
