WorkstreamsThe SPSP MH programme is centred around 5 work streams:
Travelling Teams and Ward LeadsTravelling teams have been identified in all participating wards. This is the term used to describe the three members of the ward team that have been selected to attend the National Learning Sessions to develop their skills and knowledge on aspects of the Patient Safety Programme. The role of the travelling team members is to provide leadership and support to the wider clinical team in the ward and drive the programme forward. Each travelling Team should consist of a Consultant Psychiatrist, Nurse and one other member of the clinical team. In addition some Ward Leads has been identified to have protected time to progress the work.
Staff Safety Climate Culture SurveyAStaffSafetyClimateSurveyhasbeendevelopedby the SPSP MH Programme in order to gain information about the perceptions of safety from front-line clinical staff in their clinical area as well as management’s commitment to safety. The Survey has been carried out in all participating wards in NHS GGC. The next step is for wards to meet to discuss the findings from the survey, to acknowledge existing good practice and identify areas where the team can improve and take appropriate actions. These survey reports will also be looked at on an NHS GGC wide basis to identify themes for action and share learning.
Patient Safety Climate Culture SurveyAPatient SafetyClimate Surveyhas alsobeendeveloped and has been used in most of the participating wards. Plans are in place to extend this to all of these wards. Glasgow Mental Health Network have been working with NHS GGC staff to facilitate this survey. This work has been hugely informative and again will be looked at across GGC to identify themes for action.

Local Learning Event 6 March 2014ALocalLearningsessionwasheldon6thMarch2014.Theeventwas extremely successful with staff attending from all wards involved in the programme. HIS (Healthcare Improvement Scotland)werealso inattendanceat the session. Aworldcafé session took place with teams sharing good practice and hints and tips on how to progress the work. Evaluation of the event showed staff were keen to have more group sessions to share learning. In response to this local workshops will be taking place over the coming months, bringing together a few wards and SPSP staff to problem solve and share learning.

OverviewOur programme in now well underway and we are beginning to see a lot of positive activity in our 14 pilot sites.
6 of the sites have completed the staff and patient surveys and have either begun or are about to begin collecting process data.
5 sites have still to complete the surveys and are anticipated to do so in the first quarter of 2014.
3 sites, including 2 newly identified wards, have yet to complete any activities.
We have successfully tested process bundles on Risk Assessment and Transition. These Bundles will be rolled out in the second quarter of 2014.

3. NHS Greater Glasgow & Clyde Patient Safety Programme in Mental Health
ANational LearningEventwasheldon19thMarch2014atHiltonDunblaneHydro. Thiswasattended by over 300 participants. NHS GGC was well represented with over 40 members of staff in attendance. Staff attending found the event very useful, hearing good practice in other areas andsharinglearning.AposterwassubmittedfromNHSGGConourworkintheprogrammesofar.
Measurement PlanParticipating teams have been collecting monthly outcome data which is shared with the National Team. Phase two of the programme is looking to:
• Consolidate and support existing good practice around improvement and harm reduction.
• Promote and support the use of data for improvement
• Test areas that have not yet been tested across the workstreams.
AspartofPhase2anewmeasurementplanhasbeen issuedwithclearerdefinitionsondata itemsforcollection.ThishasbeenissuedinresponsetofeedbackfromNHSBoards.Dataissubmittedviaspreadsheets on the team site and collated by Clinical Governance Support Unit for submission to HIS. Inadditioneachwardisencouragedtousetheirdatalocallytoidentifyareasforimprovement.Datacollection has been a major challenge for the programme and Clinical Governance staff are working closely with the wards to improve this.
Safety DialoguesLeadership walk rounds are an improvement tool that connects senior leaders with their frontline staff to help build a culture of safety within an organisation. In NHS GGC a decision was made to call theseeventsSafetyDialoguesinordertoembracethefocusofthembeingatwo–waydialogueratherthanatopdownprocess.SafetyDialogueswillbecarriedoutinallwardsparticipatingintheSPSPprogramme over the next year.
