ANCIZAR DE LA PEÑA SILVAANESTESIOLOGIA Y REANIMACIÓN
UNIVERSIDAD DE CARTAGENA2012
SATURACION VENOSA CENTRAL DE OXIGENO
(SvO2)
Una Mirada al balance Entre El Aporte Y El Consumo..!
OBJETIVOS
• Comprender los aspectos Fisiológicos del Aporte (DO2) y Del Consumo (VO2) De Oxigeno
• Interpretar la relación de balance entre el Aporte y el consumo de oxigeno
• Utilizar Extracción Tisular (ERO2) como fracción del aporte de oxigeno.
• Analizar ERO2 como una variable relacionada con la SvO2
• Comprender la Utilidad de la SVo2 como meta Hemodinámica
• Evaluación Del Aporte y La demanda de O2
• Gasto Cardiaco por Método de Fick Inverso
OBJETIVOS
Aporte y Consumo
El Aporte de Oxigeno (DO2)
DO2
GASTO CARDIACO (GC) [Arterial de Oxigeno] (CaO2)
(HB x 1.34ml) SaO2 0.0031 x PaO2
(9gr/dl X 1.34mlO2) 0.98 = 12ml (0.0031 x 115mmHg) = 0.35ml+
CaO2: 12.35ml/dl
DO2: 43 dl/min x 12.35ml =530ml/min
The Optimal Hematocrit. Crit Care Clin 26 (2010) 335–354
Y el Consumo?? (VO2)
Oxigeno Residual SvO2
la cantidad de oxígeno residual después que los tejidos lo consumen
OXIGENO RESIDUAL
CONSUMO DE OXIGENO (VO2)
RO2
GASTO CARDIACO (GC) [Venosa de Oxigeno] (CaO2)
(HB x 1.34ml) SaO2 0.0031 x PaO2
(9gr/dl X 1.34mlO2) 0.62 = 7.5ml (0.0031 x 55mmHg) = 0.17ml+
CaO2: 7.7ml%
The Optimal Hematocrit. Crit Care Clin 26 (2010) 335–354
VO2: 43 dl/min x 7.7ml =331ml/min
CONSUMO DE OXIGENO (VO2)
• VO2 =
• VO2 = 530ml/min - 331ml/min
• VO2 = 199ml
• Relación DO2 / VO2 = 530/331 = 1.6
DO2
RO2
DO2 critico 3 veces el VO2
Y Donde Consigo el
Gasto Cardiaco…??
Extracción de O2 (ER O2) = VO2 / DO2
(GC x Da-vO2) / (GC x CaO2) x 100
= (CaO2 - CvO2)/ CaO2
= (SaO2 - SvO2)/ SaO2
Extracción de O2 (ER O2) = 98 – 62 = 0.36 98
ER O2 α -1 SvO2

Método de Fick
• Vo2 = (CaO2 –CvO2) x GC
• IGC = _____IV02________ CaO2 - CvO2
• IGC= (10/Hb) / SaO2 - SvO2
• IGC = (10 / 9gr%) / (0,98 – 0,62)
• IGC = 1.111 / 0,36 = 3.08 Lt/min

Cuando es Baja o Alta la SvO2 ???
ER O2 α -1 SvO2
• ER O2 = 30% - 50%
Use of Central Venous Oxygen Saturation to Guide therapy. Am J Respir Crit Care Med Vol 184. pp 514–520, 2011
• SvO2 = 70% - 50%
ER O2 α -1 SvO2
• ER O2 < 25%
• SvO2 = > 85%

Limitaciones..!
SvO2 = SvO2 ??

LOCALIZACION DE LA PUNTA DEL CATETER
Use of Central Venous Oxygen Saturation to Guide therapy. Am J Respir Crit Care Med Vol 184. pp 514–520, 2011
• NO es parámetro de Evaluación Aislada• SvO2 > 70% ?
• En Algunas Circunstancias SvO2 es superior SvO2
• En situaciones de BAJO CG pequeños cambios = Grandes Cambios en SvO2
