Sample Transition Book
(All pictures and names are fake)
A copy of the transition book can be kept for the child at the school and another copy can be kept at home. As children grow older and mature, you can move from using the
transition book to a laminated picture of the youth and his homeroom teacher (Grade 9, 10, etc).
