Every time there is a drug test you have to intesserpate

In accidents local police may control you about drug and alcohol

Personal need to be aware how to do their job and duty’s

How to avoid risk of getting hurt Obtain information about health precautions (IMDG Code) Obtaining information about health hazards (MIRIS)

Some kind of work needs to have a permit Entering enclosed spaces (closed places with maybe gases)

Working aloft (higher than 2 meters)

Hot work (welding og burning, drilling and grinding, that make a spark)

Cold work (work that cant make an ignition, that job could be, clean up-work on pumps-blanking-disconnecting of pipe work, removing or fitting valves)

Accidents

The three most common places to get hurt!1. Arm, hand finger2. Foreign body in eye3. Leg foot toe

Common injuries in these jobs

Cargo handling - exposure harmful substances, opening and close hatches, entering closed rooms and tanks, hose handling and connecting work at manifolds.

Catering - cuts from knives, slipping, burns, frost bite

Cleaning - slipping - solvents - finger jammed in doors

Corrosion protection - foreign bodies in eyes - abrasive blasting - using chemicals/thinners and falling

Cranes - falling objects, crane hooks, winches

Mooring - hand and legs jammed in wire and ropes, incorrect use of rope and chain stoppers, incorrect used of windlass and winches, burn injures,, broken wires and rope

Painting - falling, toxic, high pressure

Tank cleaning - gas poising, falling, lack of oxygen

Page 3: Safety

Working in engine room - slipping and falling, burns from, steam-hot oil-chemicals, Ear damage, head injuries from falling tools and objects.

Other hazards:

NavigationRisk of explosion/fireLoss of engine and steering powerRough weather navigation

All accident need to be reported

Avoid accidents

a lot of accidents can be avoided if we used the right protection, safety clothing and safety procedure

To prevent injuries we can

Maintenance Following up reports Information and training Protective equipment

How to avoid injuries and accidents

Burns - use glows, boiler suits, the right shoes. Be careful and observant Foreign body - use goggles, face screen Frostbite - use the right clothing (gloves, boots, boiler suits, jackets, face masks) Head injuries - use helmet, when on deck, engine room, in tank or cargo holds. Exposure to harmful substances - use face masks, safety line Falling - fasten the ladder, safety belt, safety net, not carry tools, Slipping - right footwear Injuries to back and shoulders - use lifting equipment, right technic when lifting Damage to hearing - use har protection, earplugs Sprains or cuts in arms, hand or finger - use glowes Sprain or cuts in legs, feet or toes - use the right shows

Manual lifting should be considered by:

Company compliance

ISO14001 mandates that companies voluntarily implement their own Environmental management system or EMS. The EMS Ensure that there is a systematic process for complying with the ISO14001

ISO14001 Encourages companies to integrate environmental considerations in daily work across all levels of the organisation, including the vessels. This is done by promoting the awareness of environmental impact and by seeking and providing concrete solutions to reduce these impacts

Companies will therefor commit to: Pollution prevention Legal compliance Continuous improvement in environmental performance

Africa, which sea area is considered the most dangerous for piracy activity - The waters of the horn of Africa

3. Select one option that is the greatest motivation for most of the pirates - Money4. Which one of the following factors marked the seizure of the ‘’Sirius star’’ by Somali

pirates in November 2008 as unusual - It was the largest vessel seized up for that time

5. Why do pirates believe attacks far out at sea might be more successful? - Because the

target vessel might not expect to encounter pirates away from the coast and be less well prepared against attack

6. Which one of the following features will the pirates consider most advantageous hen planning to attack a merchant vessel - That it has a slow speed and low freeboard

7. When monitoring small craft in the vicinity of a vessel which one of the following manoeuvres taken by a small craft might be considered particular suspicious and possibly indicate the beginning of a pirate attack - A sudden increase of speed towards a vessel, particularly from a craft abaft the beam

8. Which one of the following options is a feature of the small ‘’skiff’’ boats used by pirates to their advantage In an attack on a merchant vessel - They are powered by outboard motors which deliver a relative fast speed

9. How is it recommended that deck lighting is used when transiting an area of know piracy activity - It should be used only in an absolute emergency and otherwise kept switched off.

10. Select one option which explains the strategy used by the military and naval counter piracy organisation in some sea areas to make monitoring nad protection of merchant vessel easier - Organised convoys

11. Initially which kind of message should be broadcast over the VHF, if a small craft is seen acting suspiciously close to the vessel, and it is suspected that a pirate might take place - PAN PAn

12. Select the one option which gives information on preparing a vessel against attacks by pirate - the ship security plan

Page 37: Safety

13. Select one option that correctly defines the meaning of a citadel fitted to a merchant vessel - A pre-prepared secure space or muster point, located at an internal position, to which all person can retreat

14. Which of the option describe what should happen to the last designated accommodation entry point if the pirates get on board - Once the last crew member have retreated into the accommodation, it is sealed from the inside.

15. Select one option that correctly identifies whether the internation maritime organisation recommends that seafarers use firearms to defend themselves and the ship against pirate - Under no circumstances

16. If pirates reach the bridge, or seize a crew member, what is the recommended procedure? Select one of the given option - Do not resist further, as it may lead to reprisals by the pirate

17. IF the military forces mount an attack to rescure the crew and recower the ship from pirates, which of the following are critical rules to follow during the action Six correct - lie down, do not take photographs, answer question, keep hands empty and visible, follow instructions, stay clear

Enclosed space entry Overview and definitions

The first task in any potentially hazardous is RISK ASSESSMENT

To go in a tank you need an ENTRY PERMIT that will be found in the ship safety management system.

The main hazard is the atmosphere. The normal oxygen is 20,9%

Enclosed space means: Limited opening Inadequate ventilation Is not designed for continuous worker occupancy

Adjacent connect space Normally unventilated. Near og next to an enclosed space.

Confined spaces used Enclosed space entry. (Cargo tank, ballast tank, pump rooms, storerooms, chain

lockers, cargo containers, cofferdams, rarely used storerooms..

So to come into that space require an enclosed space entry procedure

Gas free - is an tank or container with safe condition. (testing require an authorised person)

Entry permit - controlled document to carry out a specified task Responsible person - a person authorized to permit entry into enclosed space. Risk assessment - a methodical examination of the circumstances. Asphyxia - the condition arising when blood is deprived of an adequate supply of oxy. Toxic - poisonous to human life Approved equipment. Tested by an appropriate authority to and hazardous

environment. It must have been examined within a designated period Oxygen meter - measure the percentage of oxygen, Explosimeter - An instrument for detecting a combustible gas/air mixture. LFL Resuscitator - equipment to assist or restore the breathing of an casualty Self contained breathing apparatus (SCBA) - portable supply of compressed air. Air line breathing - face mask with air from a small hose

Tank entry

Pumproom needs an mechanical ventilation

Exited - the responsible person

Responsible person - should be appointed to take charge of operation

Page 40: Safety

Proper assessment - a competent person should make an assessment of the space Hazard identification - potential hazards should be identified Secure the space - the space should be prepared and secured for entry Test the atmosphere - the atmosphere of the space should be tested Issue a permit - the required safety management system ‘’permit to work’’ should be

issued Procedure - the appropriate safety and operational procedure should be initiated.

If a person collapsed inside an enclosed space you need to take suitable precautions

A Competent person is a person with sufficient theoretical knowledge and practical experience

The responsible person is a person authorized to permit entry. He will also decide which procedure to use, with risk assessment.

Toxic vapours from:

Vegetable fish, and animal oils - Hydrogen sulphide Chemical tankers - Container vessel - in drums Inert gas - Carbon monoxide, sulphur dioxide, nitric oxide, nitrogen dioxide Bulk cargoes - hydrogen sulphide Sewage - hydrogen sulphide

To entry an enclosed space we should take precaution of:

Standard steps in risk assessment are

Page 41: Safety

Identification of the hazard Assessment of the risk associated Application of controls to eliminate or reduce those risk Documentation of finding and actions Monitoring and adjustment

Question

1. What is the biggest hazard associated with entry into an enclosed space - The potentially harmful atmosphere inside

2. In relation to entry into an enclosed space, what is approved equipment - Equipment of a design tested and approved by an appropriate authority for use in a specified hazardous environment

3. For enclosed space entry, what is the definition of a ‘’responsible person’’ - A person authorized to permit entry into an enclosed space and having sufficient knowledge of the procedures to be established and complied with on board in order to ensure that the space is safe for entry

4. How do the IMO define an enclose space - A space with limited openings for entry and exit or inadequate ventilation or that is not designed for continuous worker occupancy

5. You are working in a tank thas has been previously tested and approved for entry and for which a valid enclosed space entry permit exist. While you are working, you begin to smell cargo vapours. Which of the procedure listed below should be followed. Sel…. - the responsible person, the enclosed space entry permit should be cancelled, the appropriate tag should be placed on the access to prohibit further entrym immediately exit the tank, further test should be made.

6. According to this module, what is the normal oxygen content in fresh air - 20,9%7. In relation to entry into an enclosed space what is ‘’approved equipment’’ -

Equipment of a design tested and approved by an appropriate authority for use in a specified hazardous environment

8. What is the first step in any potentiall hazardous work task on board - Risk assessment

9. What should be the main priority if those who enter an enclosed space to carry out a rescue - Ensure that adequate precaution have been taken to protect the rescue team, thereby preventing additional casualties.

10. Assessment has revealed that the atmosphere in an enclosed space is unsafe but immediate entry is considered necessary, what should those who enter the space be wearing - Breathing apparatus

11. Which of the following are the standard procedures to observe before entry to an enclosed space - The potential hazards are identified, a competent person makes an assessment of the space, a permit is issued, the space is secured for entry, a

Page 42: Safety

Responsible person is appointed to take charge, the atmosphere is tested, appropriate safety and operational procedure are put in place

12. Where there is minimal risk to the life or health of an person entering an enclosed space, what type of procedures will be initiated and followes - Routine safety and documentation procedures

13. A competent person should have which of the following - Sufficient theoretical knowledge and practical experience to make informed assessment of the likelihood of a dangerous atmosphere being present or subsequently arising in the space.

14. A person appointed by the master, or by the company to take charge of an operation, such as entry into an enclosed space is - Responsible person

15. If hot work is to be performed in the middle of a bulkhead between two tanks, what atmosphere test should be performed prior to commencement of work - the atmosphere in the tank to be entered should be tested for flammability and oxygen content, The atmosphere in the adjacent spaces should be tested for flammability and oxygen content

16. Which of the following are properties of hydrocarbon gas - toxic and flammable17. Liquid chemicals can be dangerous to health in which of the following says -

absorption or penetration through the skin, skin contact, ingestion or swallowing the liquid, inhalation of the fumes

18. Some cargoes are carcinogenic, what does that mean - They can generate cancer19. Which one of the following substances is toxic and is released from vegetable, fish and

animal oils during sea water - hydrogen sulphide 20. Which of the following options might cause a lack of oxygen in an enclosed space - ALL

(hydrogen may have been produced in a catholically-protected cargo tank used for ballast, inert gas may have been introduced into space, oxygen absorbing cargoes may have been carried, oxygen absorbing chemicals may have been present, oxygen may have been displaced by the use of carbon dioxide or other fire extinguishing agent, corrosion may have occurred, gases from volatile cargoes may have displaced the oxygen in the space

21. The terms lock out tag out refers to which one - The securing labelling of equipment against inadvertent use

22. A crew member is to be sent into an enclosed space to remoce remaining sludge deposit from between the frames. Fumes are present, which of the following could the crew member use - A self-contained breathing apparatus, an externally-generated air supply

23. In safety terms, why might you suspend cargo operations that involve spaces adjacent to a tank intended for entry?- Because poisonous fumes expelled from those spaces might be drawn into the space by mechanical ventialtors, because a valve on common piping might be opened in error and liquid or fumes might run across

24. When is an enclosed space considered safe and fit for unprotected entry - Only when it has been tested and confirmed as gas free by a competent person and approved for entry

Page 43: Safety

25. What preventative measure might be taken to prevent a valve being opened by mistake while entry an enclosed space is taking place - be applying a heavy duty cable tie, by fixing a safety notice or isolation tag, by fitting a specialised locking device, by fitting a numbered seal, by fitting a chain and a padlock

26. In safety terms, why might you suspend cargo operations that involve spaces adjacent to a tank intended for entry - because a valve on common piping might be opened in error and liquid or fumes might run across, because poisonous fumes expelled from those space might be drawn into the space by mechanical ventilators

27. Which one of the following statement is correct - Personal monitors only provide a warning against oxygen deficiency and hydrocarbon concentrations when there is a change in conditions in the immediate vicinity of the wearer

28. Can an Explosimeter detect the presence of flammable vapour in a space filled with gas - Some types can, and some types cannot. So this has to be checked

29. In what parts of an enclosed space should the atmosphere be tested prior to entry - at multiple levels and different sections of the space, to increase the possibility of detection pocket of gas that might remain in less well centilated areas

30. When should testing be carried out on the atmosphere in an enclosed space listed for entry - At regular and frequent intervals whilst entry continues , before entry

31. Can a combustible gas detector be used to detect gas other than standard gas on which it is calibrated - yes but only for a limited range of similar gases and reference must be made to the calibration curves supplied with the meter

32. Which instrument gives readouts of gas concentration in terms of the lower flammable limit - A combustible gas indicator

33. If the conditions at the exact job lovation within enclosed space cannot be properly assessed…… -Assessment and planning should be done to allow an intial entry and then further planning will be done once the necessary information has been obtained from inside the space

34. If a lifeline is to be used, which one of the following is an essential feature - That it can be easily detached by the wearer, in the event that it becomes tangled

35. How is the attendant assigned to enclosed space entry to be employed - The attendant must not be assigned to other duties and must remain at the entrance to the space while it is occupied

36. How can illumination safely be provided or improved in a confined space - Install temporary lighting, remove additional manhole covers and lids, issue flashlights

37. An emergency has occurred in an enclosed space - the attendant should not, under any circumstances, enter the space alone or the before the rest of rescure has arrived

38. If the attendant hears a low level audible alarm whistle from a self contained brathing apparatus coming from inside an enclosed space, what should they do - Look for the user moving toward the exit and assist them once they emerge

39. If the compressor feedin an air line breathing apparatus fails an the emeregency supply is engang, what should the warer immediately to - stop work and immediately exit the space
