Hearing protection must be worn Foot protection must be worn Hand protection must be worn Minimum P2 face mask or respirator must be worn when airborne dust or fumes are created Foot protection and spats must be worn High visibility clothing must worn Half face mask respiratory protection must be worn Protective body clothing must be worn Head protection must be worn Welding mask must be worn Face protection must be worn Safety harness must be worn Walk-a-Round POTENTIAL HAZARDS AND RISKS Entanglement Entanglement in moving parts Cutting, Stabbing or Puncturing Injury from sharp edges or burrs Shearing Body parts sheared between items of plant Slip, Trips, Falls Slip, trip, fall from entry/exit of plant and slippery plant surfaces Struck by Moving Object Struck by moving object due to unexpected movement of plant Electrical Shock or Burn Electrical shock or burn from plant contact with live electrical conductors Manual Task Injury Manual task injury from incorrect manual handling techniques Exposure to Dust/Fumes Exposure to Toxic Fumes Crushing Crushing due to being trapped between plant and fixed structure Other Exposure to hazardous chemicals PRE-OPERATIONAL SAFETY CHECKS Complete site-specific risk assessment Complete the appropriate pre-operational plant checklist if applicable Ensure you are familiar with plant operations and controls Ensure that guards are fitted, secured and functional in accordance with manufacturers guidelines If the 2 Person Man-Cage is to be used in a confined space, a Confined Space Permit to Work must be completed with a rescue plan Inspect the 2 Person Man-Cage for damage and is serviceable each time prior to use (See Pre-Use Checklist attached) Ensure the crane used to perform the lift of the 2 Person Man-Cage is rated at minimum of 1000kgs OPERATING PROCEDURES The 2 Person Man-Cage has a maximum working load limit of 250kgs The 2 Person Man-Cage is to be lifted using a 4-leg lifting chain sling with minimum leg length of 1600mm. Ensure workers to be transported in the 2 Person Man-Cage are harnessed and attached to the cage anchor points by a retractable lanyard Ensure any tools and materials being transported are secured or in a tool bag inside the cage After the worker/s, tools and materials (if needed) are secured inside the 2 Person Man-Cage, ensure the gate is closed and the latch properly engaged Using the crane slowly take up the slack on the lifting chains ensuring no shock load is applied to the cage

Raise the 2 Person Man-Cage approximately 300mm above the ground and visually re-check all lifting chains, harnesses, lanyard attachments and all tools and materials are stowed safely

Ensure any other workers are a safe distance away from cage prior to continuation of lift

Using the crane move the 2 Person Man-Cage steadily into position to the designated work area

Workers must remain standing on the floor of the cage and must not stand on the safety rails or handrail during operation

At all times be aware and ensure that the crane, rigging and 2 Person Man-Cage are kept clear of overhead powerlines and other hazards

Keep clear of moving plant parts Operate plant to the conditions of the work area Ensure no person or animal is endangered when

operating plant Before adjusting bring plant to a complete standstill

and isolate (use this when applicable)

ENDING OPERATIONS When work is complete or at the end of a shift,

worker/s will cease all actions and be prepared for the cage lift (All tools/materials must be stowed)

Using the crane slowly lift and move the 2 Person Man-Cage to the nominated resting location

Ensure the cage is gently lowered to the ground to avoid damage and injury to the workers inside

Ensue lifting chains and rigging are in the slack position before workers exit the 2 Person Man-Cage

All tools and material must be removed from cage if it is to be transported from site

A post-use inspection of the 2 Person Man-Cage should be done, and any damage other than normal wear must be reported to the supervisor

Ensure plant is in good working order and stored in the appropriate location

DO NOT Do not use if plant is faulty. Attach an Out of

Service tag and report fault to your supervisor Do not carry personnel in the 2 Person Man-Cage

if it is being moved or transported by a vehicle or Forklift

Do not use the 2 Person Man-Cage for anything other than its intended use.

Do not stand on the safety rails or handrailing during operation

Do not use ladders or other devices during the operation that would enable the worker to gain additional height/reach outside the cage

Do not leave plant running unattended Do not leave keys in machinery whilst unattended Do not wear loose jewellery Do not use mobile phone while operating plant

*Plant in this SWI refers to any machinery, equipment, appliance, container, implement and tool.

PRE-USE CHECKLIST

Is the spring-loaded gate closing securely and engaging the latch properly?Is each harness anchor point secure and free of damage?Is the internal handrailing secure and free of damage?Is the compliance attached to the 2 Person Man-Cage?Is the 2 Person Man-Cage to be inspected by a recognised authorised inspection company (All Ways Rigging)?Has each worker to use/enter the 2 Person Man-Cage read and understand SWI279?Is the lifting chain to be used of sufficient WLL rating and subject to a current inspection by a recognised authorised inspection company (All Ways Rigging)?
