The most significant event of the past month was publication of “Saaol Heart Study” - a five year result of 5000 Saaol patients in the international journal “ B i o m e d i c a l & P h a r m a c o l o g y ” i n t h e i r September ,2018 issue( volume 11(3) p 1275 -1289). This research results were collected and collated by Mr. Vikram Singh, the PhD student who worked under Dr. Chhajer for the past three years. This study not only proved the efficacy of Saaol's lifestyle intervention but also leads to an international acceptance of “Saaol Safety Circle” and “Heart Attack Protection Score” by the modern medical fraternity. The study conclusively shows that 98% of our patients improved significantly by Saaol advocated
lifestyle changes. Dr. Bimal Chhajer will be in USA for three weeks to attend “International Conference on L i f e s t y l e M e d i c i n e ” i n Indianapolis and there after hold some public awareness talks in Los Angeles, USA. Medicines used by the Doctors/Cardiologists for Heart Failure
Heart failure cases mostly require lifelong medical management. T h e y e v e n n e e d r e p e a t e d hospitalizations ranging from once or twice a year to every month. During hospital they are given oxygen, intravenous Lasix to drain water from the lungs and other emergency medicines. Once the failure is controlled patients become alright and return back to normal life. The principle of ideal treatment of Heart failure is to make sure that
the damaged heart pumps blood properly and water does not accumulate in the Lungs. Most patients can be living comfortably if they follow optimum medical drug treatment, follow lifestyle changes and take precautions. Natural Bypass or EECP treatment of 35 days has been shown to i m p r o v e C H F p a t i e n t s b y i n c r e a s i n g E F a n d r e d u c e breathlessness and improve the quality of life.

The number of heart patients in India in absolute numbers is anywhere between 8-10 crore (80 to 100 million) It is estimated that total Heart failure cases in India would be about Ten million or One crore. More than half of these patients need repeated hospitalization and the cost of the treatment in private hospitals is very high. In USA there are about 5 million or 50 lac Heart failure patients and they spend billions of dollars to keep them alive. The major cause of Heart failure is coronary artery blockages which lead to heart attacks and lowering of Ejection Fraction. This accounts for more than 60% of the cases. These cases are preventable - if we can educate the heart patients,
screen the common high risk population (those who have high lipids, consume tobacco, have high BP or Diabetes, have family history of heart disease) with CT Coronary Scan and induce them to healthy lifestyle. About 10% each of heart failure c a s e s a r e d u e t o Va l v e defects(Rheumatic Heart Disease) and Cadiomyopathy. These cases can be only managed by medicines and hospitalization in acute cases. High blood pressure contributes to about 7% cases of Heart Failure cases and are preventable. If not properly managed, the chances of death is very high in Heart Failure cases. Of those who are hospitalized with heart failure 33% die during admission and 25% die within next 3 months after they are discharged from the hospital. Most of the heart attacks are preventable by lifestyle changes and medical management. Once the heart failure is diagnosed a proper care and medicines can save these patients from hospital admissions.In a poor country like India this is the best strategy.

S h o r t n e s s o f b r e a t h o r breathlessness is a major symptom of Heart failure. Many patients report severe breathlessness on lying down and improvements on sitting up.Any experienced doctor can
diagnose heart failure by just placing a stethoscope in the patients's chest. A typical sound called Crepitations can be heard in the lower part of the back of the chest. Heart sounds are also altered. Echocard iogram can eas i ly confirm Congestive Heart Failure and is a simple non invasive test. Chest X ray and ECG can also be done to confirm.

The blood oxygen level will fail and severe breathlessness results. This is called a heart failure. Because of this congestion - now heart failure is also called CHF or Congestive Heart Failure or Congestive Cardiac Failure (CHF). In many cases the effect of this congestion
reaches the liver, abdomen and the legs also. There may be swelling of the legs. In the initial phase there will be a mild breathlessness and the system tries to overcome it by increase the heart rate. This can carry on for months even. But in the severe cases, the breathlessness is very acute and needs hospitalization. This can now be called Acute Heart Failure.
Symptoms of Heart Failure and Diagnosis
The most common cause of heart failure is Heart Attack leading to low Ejection Fraction. If the left ventricle is damaged it can be called LVF or Left Ventricular Failure. In some cases Right Ventricle Failure are also seen - where the congestion is more on the abdomen, legs and feet. Heart failure results usually after the EF goes below 35%, but many patients with even 25% do not get a failure. The chances of failure is specially more if the patient do not
control their blood pressure. High blood pressure is one of the important causes of heart failure. The other causes are Valve defects (from birth or damaged heart valves), long standing lung disease, irregular heart beats. A very uncommon disease called Cardiomyopathy - where the heart damages without any reason is also a cause of Heart failure in many cases. Obesity, Diabetes, Thyroid problems, alcohol can also lead to Congestive Heart Failure.
My name is Mr.Mahavir Kambli and I am aged 44 years. I stay in Ponda Goa. In August,2017 I developed some chest pain, especially on exertion. I was referred to the local heart Hospital where they performed a Coronary Angiography by using a catheter. There were six blockages - ranging from 70 to 90 to 100%. As already explained, a 100% block in the Coronary tubes will lead to death or damage to the heart muscles as they go without blood or oxygen. This is called a heart attack. The amount and the area of heart m u s c l e d a m a g e w i l l d e p e n d o n t h e respective t u b e s which are blocked and the location of the blockage. If the 100% blockage happens in the lower part of the tubes less will be the damage. This can be called minor attack. The Ejection Fraction or pumping power of the heart will only reduce by 5-10%. This can be measured by a simple test called Echocardiography. In case of minor heart attack the report of LVEF or
EF will be 50-58%But if the blockage of 100% happens in the beginning of the tube- more area will be damaged and the EF will go down to 30-49%. This n o w can be called a major

mostly the biggest of the three - the EF will be more depressed(towards 30%) compared to the other two tubes. Massive heart attack occurs due to blockage in the LM tube which gives origin to both LAD and LCx - leading to death of 70% muscles of the heart and is the cause of all the sudden deaths due to heart attacks.

Heart FailureHeart failure is a term used by Cardiologists to mean that the heart muscles are not able to pump adequate blood to the body per beat and its consequences. As it appears from the English language heart failure does not mean the patient will die instantly. They can live with heart failure for years as the failure on part of the heart is not complete but inadequate. The most immediate reason of such failure is low EF or low Ejection Fraction of
the Left Ventricle.In Heart Failure, the difficulties mainly arise from back pressure of the blood. If the chamber four can not pump completely and empty into the Aorta - more blood will remain in the LV and now chamber three (Left Atrium) can not send complete blood to the LV. Thus there is a congestion. Next in the sequence is Lungs - it can not send all the purified blood to the chamber three. Thus the lung gets

Heart is a pump made of muscles (called Myocardium in Medical Science). It has a size of a small apple and it keeps on pumping blood to the body without any stop throughout our life. It pumps approximately 72 times in a minute, 4300 times in an hour and about 100,000 times in a day. It is so strong that it can last for 100
years - pumping about 3.5 billion times without fault. The heart is located in the middle of the chest - little on the left side. It is protected by the sternum (breast bone) and ribs from all sides. Two lobes of the lungs on both sides gives a cushion of sponge to the heart. Heart has four chambers - two on the right side for the impure blood and two on the left side for the pur i f ied blood. Two upper chambers are used to receive the blood and two lower chambers are
pumping chambers. The upper chambers are called Right and Left Atria and the lower chambers a re ca l l ed R igh t and Lef t Ventricles. The chamber one (Right Atria) receives the impure blood from the body and then pump it to chamber two (Right Ventricle). The chamber two will now pump
this blood to the lungs for f i l l u p oxygen or purification. This blood, now filled with oxygen f r o m t h e lungs, will
come to chamber three (Left Atrium) which again pumps the blood to the final or chamber four(Left Ventricle or LV). This Left Ventricle or chamber number four is the most important chamber of the heart - as it has to pump the blood to the whole of the body with a pressure so that it can reach the furthest parts of the body. It has to create a pressure of 120 mm of Mercury - which is good enough to throw a water jet to the height of 1.5 meters in a fountain.
The muscles of the Left Ventricle are very thick and strong - the strongest of the four chambers. In each pumping the LV or Left Ventricle pumps about 60% of the blood present in the chamber - this is called Ejection Fraction or EF or LVEF. A tube called Aorta, about one inch in diameter, receives the
blood from the chamber four/Left Ventricle and distributes this blood to the whole body. After giving oxygen to the whole body this blood again becomes impure and returns to chamber one via the veins. This way the heart completes one c i rc le ca l led Circulatory System.
The heart - made of muscles and consisting of all the four chambers- needs oxygen/blood for its own consumption. This is supplied to it by three small Coronary Arteries - called LAD, RCA and LCx. The LAD or Left Anterior Descending A r t e r y i s t h e biggest o f t h e t h r e e a r t e r y w h i c h supplies blood to a l m o s t 50% of t h e muscles o f t h e h e a r t and mostly to the walls of Left Ven t r i c l e . Th i s i s the mos t important tube of the heart. The RCA or Right Coronary Artery supplies about 30% of the heart muscles - mostly on the right side of the heart. The back of left is supplied by the LCx or Left Circumference Artery which
supplies the rest 20% of the heart muscles. The LAD and LCx are on the left side and arise from the LM tube or Left Main tube. Blockages inside these arteries c a u s e d b y t h e f a t d e p o s i t s (Cholesterol and Triglycerides)
l e a d t o shortage of blood to the h e a r t muscle s . T h e patients do n o t know about
the blocks till they close 70% of the tube. Thereafter the heart gets Angina (chest pain or heaviness, breathlessness)- when the blocks become 70-90%. When the block becomes 100% - no blood reach the heart muscles and they start dying. This is called Heart Attack, Cardiac Arrest or Myocardial Infarction.
