II. SCOPE OF SPECIAL INSPECTIONS This project, located at 255 Broadway Street, has a mat slab and 2 concrete podiums with 1 floor of metal stud framing and 7 floors of wood framing. When complete, the building will contain 75 residential units comprised of studios, 1, 2 and 3 bedroom units. The basement and level 1 will contain some common area, retail space, and offices. There will be roof decks at levels 6 and 7 and an exterior courtyard at level 1. Total area is approximately 88,000 SF.
This project is funded in part by the Mayor’s Office of Housing (MOH) / Human Rights Commission (HRC). As such, the HRC’s Local Business Enterprise (LBE), Construction Workforce and Prevailing Wage policies apply. The majority of the inspections are anticipated to occur from March 2013 thru May 2014. A construction schedule (Attachment A) is included for your information and use. The scope of inspections will be as dictated by the San Francisco Department of Building and Inspection. Your fee proposal shall be based upon the following scope of work. Reinforced Concrete

Wood Framing k. Wood Framing – Include Framing, Shear Wall, Nailing and other
inspections as required by the City and County of SF. The wood framing will occur over approximately 69,000 sf and involves the construction of 75 units. It is anticipated that much of the wood framing will be performed offsite at Bay Area Construction Framer’s yard located in Livermore. Wall panels will be assembled at this location and then hoisted into place and nailed off.
Dowels & Bolts in Concrete l. Epoxy Dowels – Allow for the inspection and requisite number of pull
tests for 100 epoxy dowels m. Anchor Bolts / Embeds for Structural Steel – Allow for the inspection of
100 anchor bolts and / or embeds. n. Sill / Anchor Bolts for Wood Framing – Allow for the inspection of 1,700
sill bolts o. Tie-Down Anchor Bolts – Allow for 300 tie down inspections.

All inspection reports shall be submitted to the Owner, Architect, Engineer, San Francisco Department of Building and Inspection, and Cahill Contractors. Any items that fail inspections shall be re-inspected and a revised report referencing the original failed inspection will be included for reference. All Final Affidavits or final inspection reports that are required by the San Francisco Department of Building and Inspection or Owner shall be included in the scope. III. EQUAL OPPORTUNITY GOALS The Developers of the project (Broadway Sansome Associates, LP) have committed to meeting the San Francisco Human Rights Commission (HRC) equal opportunity contracting and work force participation goals. In general, the project goals can be summarized as follows:

Local Business Enterprises (LBE): LBE status will be considered in the award of this work. To claim LBE status, a business must be certified by the MOH / HRC on or before the bid date. Please provide your HRC Certification letter with your bid. Please list the involvement of any suppliers or lower tier-subcontractors that may be included in your bid as well as their LBE status. Prevailing Wage Provisions: This project is subject to federal and state prevailing wage requirements. Subcontractors must pay all trades in their employ the higher of the state or federal prevailing wage determination. Please see the California Department of Industrial Relations website (www.dir.ca.gov) or go to the links below for more info. If you do not understand which wage to include in your pricing, please contact Cahill Contractors.

Payroll Requirements Inspection Firms will be required to submit certified payrolls every week, through the Elations System [or LCP Tracker].
Preconstruction Meeting Inspection Firms shall be required to attend a preconstruction meeting with Cahill, the Mayor’s Office of Housing and City Build. This meeting must be held prior to the commencement of any subcontractor’s work onsite.
IV. PROPOSAL CONTENTS AND DEADLINE Proposals are due on Wednesday February 20th at 2:00 pm to Casey Kasten. Proposals shall include the following information which will be used to evaluate the proposals:
1. A brief statement of your firm’s qualifications in completing the scope of work described above. Please provide references
2. Fee schedule, as follows:
a. Fees will be paid on a time and materials basis. Please provide i. Your rate schedule (hourly and overtime rates) including any
escalation during the project period, ii. Any minimum number of hours per inspection or minimum trip
charges. iii. Rates for reimbursable expenses.
Please note that all rates are to be good for the duration of the project. Escalation will not be allowed. b. Total estimated fee for the scope of work described above. Please itemize
your estimated fees for each inspection type listed in the scope. All fees or rates quoted shall be good for the duration of the job. Escalation will not be allowed.
3. Insurance – Please provide a sample insurance certificate with your proposal.
Minimum insurance limits are as follows:
Workers Comp - $1M Auto - $1M General Liability - $1M Umbrella - $5M Professional Liability - $1M The Owner (Broadway Sansome Associates, LP) shall be named as the primary insured on all insurance certificates. Inspection Firms will be required to include the following entities as additional insured:
• Mayor’s Office of Housing (MOH) / Human Rights Commission (HRC) • Chinese Community Development Center & their agents & consultants • Mithun Solomon (Architect) • Design Studios Gonzalo Castro (Construction Manager) • Cahill Contractors, Inc (Contractor)
4. If applicable, evidence of status as Local Business Enterprise as certified by the
Human Rights Commission or potential eligibility for Local Business Enterprise status and willingness to apply if your firm meets the requirements as spelled out by the Human Rights Commission.

Broadway Sansome Associates LP will enter into a contract with the Special Inspections and Testing firm submitting the best overall proposal. The proposed fees, experience, insurance, LBE status and references will be evaluated to determine the successful firm. Broadway Sansome Associates, LP reserves the right to waive any irregularities in the proposals submitted.
